We describe the first documented case of life-threatening upper airway obstruction due to retained secretions around a nasogastric (NG) tube in a patient suffering from this disease who was managed with prolonged continuous positive airways pressure (CPAP). We report a case wherein the volume and concentration of local anaesthetic used was well within the safe limits. Despite the safe precautions taken, the patient developed a profound motor and sensory block which took nearly 20 weeks to resolve.\u0000Case presentation: A 72-year-old female patient scheduled for a total shoulder arthroplasty received a single shot interscalene block under ultrasound guidance. A total volume of 10mls of ropivacaine 0.375% was injected under direct ultrasound guidance. The patient experienced no pain on injection and neither was there any paraesthesia on needle placement. We encountered no high pressures during the injection. The patient had a profound block which lasted for over 20 weeks. The patient was followed up at regular intervals which showed a slow recovery pattern.\u0000Conclusion: Local anaesthetics are known to cause neurotoxicity. A traitor! A wasted year! Worst year! Funny memes were created around it. Nevertheless, it has indeed been a tough year for all of us.\u0000However, in my experience, this year has been one of my best teachers so far. I sit down at my desk to reflect upon the events this year which has significantly reshaped my life.\u0000Here I enumerate the lessons which have empowered me for a lifetime.\u0000Acceptance- I accepted the reality without complaining, however deep down I was worried about the uncertainty of the disease, effects of the disease on myself, my family and humanity in general. However, this never came in my way of performing my duties as a doctor at the frontline.\u0000Resilience- This year was an emergence of an endurable and resilient version of myself. The surreal circumstances I faced during the lockdown compelled me not to lose my cool and carry on, albeit with moments of frustration. I have definitely emerged out stronger.\u0000Realisation- I wasn’t alone in my journey. God has blessed me with beautiful parents, a loving husband, a doting brother and gems whom we call friends who kept checking on my physical and mental wellbeing. I am eternally grateful to one and all for the support.\u0000Health and Wellbeing– Self-care should not be equated with self-indulgence. Rather it should be a priority as one cannot fill from an empty bucket. I decided to transform myself on the fitness front and bring out my strongest version.\u0000Love– Family first always! Distance makes love grow fonder, I have experienced it in a real sense.\u0000Art of Giving- The fact that everything is non-permanent in this world, and nothing belongs to you, has reinforced my faith in charity.\u0000Opportunities- while we were crippled with restrictions, conferences looked like a distant reality. Technological advancements however created a land of opportunities. For these patients requiring general anaesthesia and endotracheal intubation, maintaining spontaneous breathing throughout induction should be considered foundational to safety. Spontaneous respiration using intravenous anaesthesia and high-flow nasal oxygen (STRIVE Hi) has been previously shown to safely induce patients with difficult airways. Other commercially available phantom models like gelatin based phantom and agar based phantoms though cheap has less shelf life .We devised a novel and cost effective learning phantom using Igel for needling training. These causes include endocrine, toxin induced, nutritional disorders, infectious diseases and many more. One extremely under reported cause is Neurotoxicity due to antibiotics.\u0000Case Presentation: Here we present a case of a 48-year-old woman, with known Systemic Hypertension and Chronic Kidney Disease. She developed fever which was treated with a fourth-generation cephalosporin following which she showed signs of altered sensorium. Various investigations were done which lead towards the diagnosis of Cephalosporin Induced Encephalopathy (CIE).\u0000Conclusion: Our Case Report highlights the risk factors for developing CIE and the correct investigations to be done for any case of Encephalopathy. A happy go lucky person with a large group of admirers consisting largely of fellow doctors, both young & old. He is a great party man, drinks & smoke on the house with his devoted fanfare partaking his lavish offerings.\u0000And then, one fine day, he contacts COVID-19 & is admitted in the very hospital where he had spent 30 years of his life. He develops respiratory distress… As I rush to attend the call, I am appalled by the look of the deserted corridors & waiting areas. I don’t see any known faces, none of his fanfare for sure. He lies struggling for breath on non invasive ventilation, confused & hypoxic. As he is intubated & put on ventilatory support at the insistence of his son, I reflect back on those days when he used to be surrounded by his friends, treating them to drinks. But today, when he needs their support the most, he is left alone, in his suffering, to fend for himself.\u0000Truly, we are indeed a death denying society.\u0000\u0000‘O death, where is your victory? O death, where is your sting?’ (St.Paul, I Corinthians, 15:55)\u0000\u0000When it began, people thought, ‘It can’t affect me’. However, with time, they were increasingly confronted with the looming threat which threatened to disrupt the ‘web of illusion’ that they had created for themselves…an illusion, for the very world of existence is actually akin to an illusion, characterised by its transient nature. No wonder, our ancient sages called the world of existence ‘maya’. Then came the period of ‘lockdowns’ & the web of uncertainity surrounding it. People were in a fix, for in today’s tech-savvy existence, fostered by science & reasoning, they were now unprepared to spend time with ‘themselves’! The very Self was now seen as an alien being & was avoided.
