Gastrointestinal fonksiyon bozukluğu olmayan hastalarda öncelikle enteral beslenme tercih edilmektedir. Enteral beslenme, hastanın yutma eylemini gerçekleştiremediği durumlarda ilaç uygulanması amacıyla da kullanılmaktadır. Fakat beslenme tüpünden ilaç uygulanması karmaşık bir yoldur. Beslenme tüpünde tıkanmayı ve ilaç etkisinde azalmayı önlemek, ilaç toksisite riskini en aza indirebilmek için, ilaç uygulanması sırasında uygun tekniklerin kullanılması gerekmektedir. Ağızdan ilaç alamayan hastalarda, alternatif uygulama yolları olarak ilacın intravenöz, intramusküler, subkütan, inhale, transdermal, rektal yollar ve dil altı tabletler gibi farklı dozaj şekilleri mevcut olduğunda uygulamada öncelikle tercih edilmektedir. Alternatif uygulama yollarının elverişli olmadığı durumlarda, enteral beslenme tüpü ile ilaç uygularken Amerikan Parenteral ve Enteral Beslenme Derneği önerileri, beslenme tüpünün boyutunun etkisi, beslenme tüpünün konumunun etkisi, farmasötik dozaj şeklinin özellikleri ve uygulama yöntemleri göz önüne alınmalıdır. Nütrisyon desteği alan hastalarda ilaç etkileşimi olasılığı ortaya çıkmaktadır. Etkileşimleri önleyebilmek için ilacın mekanizması ve özellikleri konusunda daha detaylı eğitim alan bir eczacının ekipte olması ve mesleki bilgilerinden faydalanması gerekmektedir. Anahtar Kelimeler: Enteral beslenme, dozaj şekilleri, enteral ilaç uygulama SUMMARY A caloric requirement of the patients who do not have an access for feeding through an oral route is supported by enteral or parenteral nutrition. In the patients who have suitable gastrointestinal function, enteral feeding is preferred initially. Enteral feeding is also used for administration of medications in patients who cannot swallow. However, an administration of drugs via a feeding tube is complicated; appropriate techniques should be used in order to prevent obstruction of feeding tube and thereby avoid reduction of the drug effect and to minimize the risk of toxicity of given pharmaceuticals. In the patients who are not able to take medication orally, alternative routes such as intravenous, intramuscular, subcutaneous, inhaled, transdermal, rectal, and sublingual administrations are available and should be preferred at the first place. According to the American Society for Parenteral and Enteral Nutrition, an effect of a size and the position of the feeding tube, characteristics of pharmaceutical dosage form and methods of administration should be considered when applying medication via enteral tube. The risk of the drug interactions arises in patients who have nutritional support. The protection methods are important in HAIs as well as other infection diseases (1). Hand hygiene practice is one of the most effective measures to prevent HAIs (2). Although hand hygiene is an effective and simple method, hand hygiene compliance among health care workers is very low (3). SUMMARY Objective: The most efficient and most cost effective method for preventing health care associated infections is hand hygiene. Although hand hygiene is the most effective and simple method, compliance rates are very low among health care workers. It was aimed to evaluate the rates of compliance of healthcare workers in a state hospital. Material and Method: In this study, totally 112 healthcare workers (31 doctors and 81 nurses) were evaluated with the 5-indication observation method in a period between January and July 2013. Results: A total of 754 (65.9%) out of 1.144 cases were resulted in accurate hand washing and hand-rubbing. When the intensive care unit and surgical clinics were evaluated together, it was found that hand hygiene compliance rates were 51.26% in 199 cases and 66.85% in 591 cases for doctors and nurses, respectively.
