Conservative treatment is preferred.\u0000Objective: Evaluation of the effectiveness and safety of «Apicold Mint» and «Apicold Propo» nasal spray in children with adenoiditis.\u0000Material and methods: A study of the action of «Apicold Mint» and «Apicold Propo» Nasal Spray (manufactured by APIPHARMA doo) in the treatment of adenoiditis. The clinical study involved 2570 children aged 4-17 years.\u0000Results and discussion: The use of «Apicold Mint» and «Apicold Propo» nasal spray helps reduce the symptoms of adenoiditis in the absence of side effects. This involves the formation and presence of complications that are not usual for the main disease. The influence of comorbidity is particularly increasing with age and also due to the presence of concomitant diabetes and (or) metabolic syndrome. These data and our previous studies (determining the effect of hyperglycemia on the formation of a complicated course of acute otitis media) make the study relevant.\u0000Aim: to create a typical \"clinical portrait\" of a patient with type 2 diabetes on the basis of clinical and laboratory indices, taking into account pathogenetic links of bone resorption.\u0000Materials and methods: The study included 45 patients aged 38 to 75 years with type 2 diabetes without acute middle otitis media who underwent examination and treatment at the \"V.Y. Danilevsky Institute of Endocrine Pathology Problems\" during the period from 2019 to 2021. Patients were included in the study according to the randomized simple sampling method in the order of admission to the hospital and were divided into groups for further analysis according to the level of glucosylated hemoglobin (HbA1c): 1st group – 15 patients with HbA1c up to 7%; 2nd – 21 patients with HbA1c levels of 7-10%; 3rd – 9 patients with HbA1c levels higher than 10%.\u0000The examined patients were subjected to clinical examinations of blood, urine, blood glucose level with determination of the average glucemia and its fluctuations, level of glucosylated hemoglobin (HbA1c), total and ionized calcium in the blood, lipidogram, the level of interleukin 6 (IL-6) and tumor necrosis factor (TNF-α), the level of parathormone in the blood.\u0000Results: According to the received data, the general portrait of a patient with type 2 CD is as follows: a patient of a certain age group (over 50 years old) with increased body weight (BMI for the whole group was 31.07±0.84 kg/m2 with fluctuations from 22 to 44 kg/m2), with comorbid pathology (the most frequent were metabolism encephalopathy with liquor-venous distension (95.5%), hypertonic disease and polyneuropathy (91.1%), retinopathy (86.7%) and cardiopathy (60%), thyroid pathology and fatty hepatosis (55.6% and 57.8% respectively), obesity and cardiosclerosis (46.7% and 44.4% respectively), and the presence of a diabetes in the family history (62.2%). This symptom can be caused by many diseases. One factor which is contributing to dizziness is mental disorders (including depression and anxiety). The problem of psychoemotional stress in recent decades is becoming increasingly important in the world. The COVID-19 pandemic, which was declared by the WHO in 2020, substantially deepened that problem.\u0000Numerous studies show that persons of working age suffer most from psychoemotional stress. An urgent task is a selection of scales and questionnaires for persons with dizziness caused by chronic stress.\u0000Aim: to increase the effectiveness of diagnosis and treatment of cochleovestibular disorders that caused under stress conditions and develop the rehabilitation events.\u0000Materials and methods: 95 patients aged 18 to 59 years were examined with complaints of dizziness caused by stress conditions and 20 people in the control group. 52 patients from main group also complained of tinnitus and 35 of hearing loss.\u0000The following studies were carried out for patients: audiometry in full, listening adaptation, detection tests for central auditory disorders, impedance measurement, short-patent auditory induced potentials (CSVP), vestibulometry.\u0000The questionnaire was conducted on the following scales: the questionnaire \"Comprehensive Stress Assessment\" (by Shcherbatykh YuV), the Hospital scale of Anxiety and depression (HADS), the Quality of Life Assessment Scale (by Chaban AS) and the Holmes and Reich social adaptation scale.\u0000Non-parametric methods were used for statistical analysis. The difference in comparing of the two independent aggregates was considered valid at p < 0.05.\u0000Results: All patients with psychogenic dizziness were divided into three groups depending on the severity of stress. The first group included 21 people (22.1%) with moderate stress, the second group – 35 patients (36.8%) with fairly pronounced stress, and at the third group were 39 people (41.1%) in a state of severe stress (but 10 of them are on the verge of exhaustion of the body's adaptive forces). The average indicators obtained during the questionnaire for the inquirer \"Comprehensive stress assessment\" in the 1st group were 9.0 (10.5-7) points, in the 2nd group – 17.5 (21-14.5) points and in the 3rd group – 34.0 (28-41.5).\u0000The results of the Hospital Anxiety Scale and Depression (HADS) questionnaire are: in 1st group more than half of the subanxiety scale studies was normal; none of the subjects showed clinically pronounced depression and only 3 of them (14.3%) showed subclinically pronounced depression. In group 2nd the anxiety component is more pronounced with a predominance of clinically expressed anxiety; depression was diagnosed only in 37.1% with a predominance of subclinically pronounced depression. However, the regeneration of tissues damaged during surgery remains important. It has three stages – alteration (primary and secondary), exudation and proliferation – and takes place with the formation of reactive oxygen species. Therefore, determination of total NO synthase activity (NOS), inducible synthase activity (iNOS) and constitutive (cNOS) isoforms are biochemical markers of the inflammatory response. The activity of the alteration process and the degree of postoperative damage is characterized by the concentration of glycosaminoglycans and L-oxyproline.\u0000Materials and methods: This study determined the multimodal (multi-vector) effect of Flu-Acyl rhino spray (6% N-acetylcysteine in 3% hypertonic pH-controlled saline) on the inflammatory response of the nasal mucosa in the postoperative period. The study was conducted at the Department of Otolaryngology of NMU named after O.O. Bogomolets on the basis of \"Alexander Clinical Hospital of Kyiv\". Clinical and laboratory examinations of 52 patients aged 18 to 60 years who underwent FESS were performed. Patients were randomized into two groups. 24 patients received standard treatment (control group), 26 patients – with the addition of nasal spray Flu-Acyl rhino (main group).\u0000Results: It was found that the indicators of total activity of NO synthase (NOS) and its isoforms, as well as the concentration of glycosaminoglycans and L-oxyproline were lower in patients of the main group on days 7 and 14 of the postoperative period compared with patients in the control group (р<0,05). These results can be explained by the anti-inflammatory effect of Flu-Acyl rhino spray due to the presence of N-acetylcysteine, which is able to inhibit the activation of the transcription factor NF-kB, which leads to decreased iNOS activity on the 7th and 14th day of treatment. Poymanova","doi":"10.37219/2528-8253-2021-6-26","DOIUrl":"https://doi.org/10.37219/2528-8253-2021-6-26","url":null,"abstract":"Aim: study of the characteristics of the acoustic stapedius-muscle reflex in conjunction with the data of threshold tone audiometry in combatants with different dynamics of the course of sensorineural hearing loss.\u0000Materials and methods: The characteristics acoustic stapedius-muscle reflex and the indicators of threshold tonal audiometry in combatants with different dynamics of sensorineural hearing loss were conducted. 60 military personnel were examined: group 1, with positive dynamics after treatment, group 2, in which progression sensorineural hearing loss took place, 30 people per group.\u0000Results and discussion: The study showed that patients who received combat acutrauma showed significant changes in the qualitative and quantitative characteristics of the acoustic stapedius-muscle reflex, which may indicate that they have disorders in the brainstem of the auditory analyzer. In patients with progressive sensorineural hearing loss, these disorders are more pronounced. Thus, they observed a significant (p<0.01) decrease in amplitude not only compared to the control, but also with the indicators of the group with positive dynamics. In addition, in the group with progressive course (group 2) there is a greater \"dissociation\" of acoustic stapediusmuscle reflex – a larger number of patients with no acoustic stapedius reflex, \"asymmetric\" acoustic stapediusmuscle reflex. When examining patients with combat acutrauma, we recommend examining both the peripheral (according to threshold tonal audiometry) and brainstem (according to acoustic stapedius-muscle reflex) department of the auditory system, which will not only identify disorders and prescribe the necessary treatment, but also assess the severity and predict the course of disorders in such patients. Therefore, acoustic stapedius-muscle reflex and its amplitude indicators should be used as diagnostic and prognostic criteria when conducting expert examinations of this category of patients.\u0000Conclusions:\u00001. In combat acutrauma there are violations both in the peripheral department of the auditory analyzer according to the threshold tonal audiometry, and in the central (brainstem) according to acoustic stapedius-muscle reflex.\u00002. In combat acutrauma there is a significant (p<0,01) decrease in the amplitude of acoustic stapediusmuscle reflex, more pronounced in fighters with progressive SNP.\u00003. \"Asymmetry\" of acoustic stapedius-muscle reflex, pronounced decrease in the amplitude of the acoustic reflex, \"dissociation\" of acoustic reflex indicators with the results of audiometric examination is not only a typical sign of hearing loss in fighters with acute trauma, but can be a prognostic marker and one of the criteria disorders in this category of patients.\u00004. The transition of inflammation to a chronic form is associated with the suppression of factors of colonization resistance of this biotope, the emergence of pathogenic microorganisms such as St. рyogenes and S. aureus. Аgainst the background of developing dysbiosis and immune imbalance, a number of complications from organs and systems may develop. Therefore, the search for means for effective topical etiotropic treatment of chronic tonsillitis and, at the same time, gentle on the indigenous microflora, is relevant.\u0000Materials and methods: The study involved 52 patients with chronic tonsillitis. All patients were divided into 2 groups of 26 each. Subjects of the 1st group in local therapy used the bacterial component \"Bioplasmix throat spray\", and patients in the 2nd group – used the traditional course of treatment: washing the lacunae of the tonsils with antiseptic agent – decamethoxine at a concentration of 0.2 mg/ml.\u0000Results and discussion: Studies have shown the selective antimicrobial activity of \"Bioplasmix Throat Spray\". Thus, in relation to the main pathogens \"Bioplasmix Throat Spray\" showed a pronounced antibacterial effect and, at the same time, helped to normalize the population level of indigenous microflora, which indicates the restoration of colonization resistance of the biotope. The positive dynamics of the main complaints of patients and pharyngoscopic signs testified to the clinical effectiveness of \"Bioplasmiх Throat Spray\". The analysis of clinical indicators in patients of the 2nd group showed a decrease in the severity of patient complaints and objective signs of the disease during treatment with decamethoxine. The sense of smell plays an important role in regulating human behavior. For example, the olfactory impulse reaches the brain faster than the impulse of pain, and therefore very effectively regulates us, regardless of awareness.\u0000Odors can change the work of various body systems: they affect the rhythm of our breathing, the excitability of muscles, the work of the brain and the entire nervous system. The sense of smell affects the limbic system, the part of the brain that controls hunger, feelings of aggression, sexual emotions and other \"animal\" feelings of a person.\u0000OD violates the informational and controlling roles of odorous substances (attractants, repellents, pheromones), therefore disorganizing the performance of nutritional, sexual, protective, orientation functions. For people of some specialties (taster, perfumer, cook), the decrease in the sense of smell can mean job loss and social maladaptation.\u0000Complaints about problems with taste perception more often reflect violations of the sense of smell, since the taste of a number of foods (chocolate, coffee, vanilla, strawberries, etc.) in reality depends on the stimulation of the sense of smell by volatile components that enter the nasopharynx when swallowing.\u0000Aim: To offer an otorhinolaryngologist guidance in the information stream regarding the problem of olfactory dysfunction, its prevalence, diagnosis and treatment.\u0000Materials and methods: The Sniffin’ Sticks test (CST) has been certified in the EU since 1995, and is used in Germany, Austria, Switzerland, and Italy. Odors are offered in the form of markers, in which the odorous substance takes the place of the coloring one. There is a version with 16 odors from the extended test [extendid] or with 12 odors [screening]. The test is well validated, can, unlike CCSIT, be used repeatedly, the shelf life is 0.5-1 year, depending on the intensity of application. The test can be used by patients independently. Factors of inflammation (interleukin-1β, γ-interferon), anti-inflammation and regeneration (interleukin-34 and transforming growth factor – TGF-1β) were determined in the blood serum by ELISA, under the influence of medications that remodulate bone matrix and regeneration – osteogenon and soluble vitamin D3.\u0000Results and conclusions: It was found that the osteogenon and aquadetrim (vit. D3) have a positive influence on the production of cytokines by blood mononuclears of the patients with FBT: they both decrease the level of production of inflammatory cytokines in vitro and increase the level of production of anti-inflammatory cytokines especially when used together.
