Latimer","doi":"10.33235/wpr.28.4.154-162","DOIUrl":"https://doi.org/10.33235/wpr.28.4.154-162","url":null,"abstract":"Aim To measure the relationship between patient participation in pressure injury prevention (PIP) both before and after implementing a patient-centred pressure injury prevention care bundle (PPIPCB). Methods This before and after intervention study used surveys to measure adult medical patients' participation in their PIP care and satisfaction with the PPIPCB across three medical inpatient units in an Australian tertiary hospital. Descriptive statistics described the sample and patient intervention satisfaction. A paired samples t-test measured changes in the before and after survey scale scores. Results Eighty patients, with an average age of 67.2 years (SD=18.3), were recruited during November and December 2019. There were statistically significant increases in total mean scores for the patient participation in pressure injury prevention (PPPIP) scale after the intervention (before: mean=18.6, SD=2.8; after: mean=22.5, SD=1.9). Significant improvements in patients' self-reported knowledge of pressure injury (PI) risk and perceived levels of participation in their PI care were reported after receiving the intervention. A total of 98.8% of participants agree or strongly agree that they were satisfied with the intervention as delivered. Conclusion Engaging patients is one way to increase their participation in PIP. As we age, the body's physiological resilience is compromised and homeostasis becomes difficult to maintain. This scoping review examines the influence of biological ageing and the impact of age-related concerns on wound healing, including frailty, malnutrition, pre-existing medical conditions and clinician practices. Frailty rather than age was seen to have a greater physiological impact on outcome, resilience and healing. Clinician support, education and engagement were fundamental to achieve acute wound healing in the aged population. This focus has been facilitated by the generation of data on pressure injury point prevalence which began publication in Australia in 2003. Since then, many healthcare organisations have undertaken serial pressure injury point prevalence audits (PIPPA) as part of a comprehensive program to measure and address pressure injuries. In our local health district, there has been a commitment to an annual PIPPA, but recently we sought to determine if we had reached a tipping point where the relative benefits of collecting this snapshot data may no longer justify the costs, particularly given the economic environment in the Australian healthcare system. With the coronavirus (COVID-19) pandemic, healthcare staff are wearing personal protective equipment (PPE) for prolonged periods. Anecdotally, cases of healthcare workers' self report of injuries to their face, particularly the bridge of the nose, upper cheek, forehead and above the ears, as a result of extended use of eye protection and masks is increasing. Protecting the skin of frontline healthcare staff is as important as protecting patients' skin. {"title":"What is new in our understanding of pressure injuries: the inextricable association between sustained tissue deformations and pain and the role of the support surface","authors":"A. Soppi","doi":"10.33235/wpr.28.2.58-65","DOIUrl":"https://doi.org/10.33235/wpr.28.2.58-65","url":null,"abstract":"This paper first provides an overview of the contemporary research findings and latest aetiological discoveries concerning the prevention of pressure injuries (PIs) as reported in the 2019 version of the Prevention and treatment of pressure ulcers/injuries: clinical practice guideline (known as the International guideline), including, where relevant, through the new prism of the coronavirus disease 2019 (COVID-19) pandemic. Second, the biomechanical principles of PI prevention (PIP) through minimisation of tissue deformation levels are explained from a support surface design perspective. Third, and related to the second, the association between alleviation of sustained tissue deformations and mitigation of PI-risk-related or PI-related pain are reviewed with a focus on the role of the support surface.
