Pharmacy","doi":"10.12680/balneo.2019.235","DOIUrl":"https://doi.org/10.12680/balneo.2019.235","url":null,"abstract":"Abstract Traumatic vertebral artery (VA) dissection is a severe consequence of a cervical injury, which usually involves the vertebrae and spinal cord. Traumatic VA dissection has been frequently underdiagnosed or misdiagnosed, mainly because many patients remain asymptomatic. The consequence of VA dissection is ischemia in the territory supplied by the affected artery. The VA dissection presents most often as a vertebrobasilary transient ischemic attack or ischemic stroke, usually preceded by local symptoms such as neck pain or headache. Headache is a common symptom in patients with cervical artery dissection, but its characters are not specific. Diagnosis of VA dissection usually requires a CT-angiography. Delayed onset of symptoms with a variable asymptomatic interval, ranging from several days to 3 months, has been reported. The risk of stroke remains high, especially in the first weeks after the confirmation of VA dissection. Treatment of traumatic VA dissections include anticoagulation or antiplatelet therapy and revascularization techniques. Given the prevalence of cardiovascular disease (CVD), preclinical detection may help modify atherosclerosis and prevent events. Although traditional cardiovascular risk factors relate to future CVD, the predictive value of these risk factors can be relatively limited. Contemporary scientific attention has focused on alternative direct measures of arterial function . Studies have provided unique insights into the pathophysiology of disease processes, the response to treatment and prognostic information that may be helpful in risk prediction. Peripheral measures of endothelial function are now established surrogate markers of vascular risk and have become important markers for clinical research. Cardiac rehabilitation (CR) is of critical importance in the secondary prevention of cardiovascular disease. It has been reported that CR was negatively related to reinfarction, cardiac morbidity and all-cause mortality. Spasticity is characterized by a velocity-dependent increase in resistance to passive movement, and is one of the “positive signs” of upper motor neuron syndrome. Spasticity induces pain, ankylosis, tendon retraction, increasing motor deficit, which may limit the efficacy of rehabilitation methods . Spasticity is also correlated with activity limitations, and reduces quality of life of patients and caregivers. Assessment of post-stroke spasticity requires first clinical examination; scales as Ashworth Modifyed scale and Tardieu Modifyed scale are useful quantitative tools. Treatment of spasticity is often challenging for the rehabilitation team, requiring a multidisciplinary approach. Therapeutic interventions include physical therapy, occupational therapy, use of assistive devices, pharmacological treatment and injectable treatment. In the interval 2014-2016, a series of 51 children (74 feet) suffering of clubfoot were treated according to the Ponseti method in our department. Of them, 36 children satisfied the inclusion criteria in the study. Compliance with orthoses was defined in the terms below: 23 hours/day in the first three months after the cast removal and minimum 8 hours/day up to the age of 3 years. Non-compliance was defined as the incapacity of observing the criterion mentioned above up to the age of 18 months. Out of the 36 patients, 25 (69%) patients (35 feet) complied with the requirement and 11 (31%) patients (15 feet) did not comply with the foot abduction orthoses. At the age of 18 months, it was found out that in the non-complying group, the Pirani score increased significantly as compared to the complying group which means a deterioration of the initial result. Another finding with reference to the foot abduction orthoses complying group is that the Pirani score was maintained or improved significantly (p<0.05), while with the non-complying group the initial results obtained by plaster casts were damaged due to lack of use of orthoses (p<0.05). Material and Methods: articles published in English between 2008 and 2018 from medical literature were selected. Results: when it comes to acute lesions three articles were found; one about the using of PRP in distal radius fractures and the two about the infiltration of PRP in the complete neurotmesis of radial nerve or in digital nerve incomplete section. In chronic conditions only one article about trapeziometacarpian (TMC) osteoarthritis (OA) was found, and three articles about a median nerve compression in carpal tunnel syndrome (CTS). Conclusions: 1.The medical literature contains few publications regarding the efficiency of PRP in treatment of traumatic lesions and chronic disease of forearm, wrist and hand. 2.
