{"title":"A comparative study between Roux-en-Y reconstruction with isolated biliary limb and single loop after pancreaticoduodenectomy: a solution for delayed gastric emptying","authors":"Mostafa Abdo, Gad M. Behairy, S. Albalkiny, Ehab Abdel-Wahab","doi":"10.4103/ejs.ejs_303_22","DOIUrl":"https://doi.org/10.4103/ejs.ejs_303_22","url":null,"abstract":"Background Whipple’s operation is the primary treatment for periampullary and pancreatic tumors. The rates of morbidity and mortality have gone down to approach 50 and 1%, respectively. The incidence of delayed gastric emptying (DGE) is reported in 25–70% of patients, making it a significant morbidity. In this study, we described the use of an isolated biliary limb in Roux-en-Y hepaticojejunostomy (HJ) for biliary reconstruction and analyzed its effect on the incidence and severity of the DGE as a primary end point as well as postoperative morbidity and mortality and length of hospital stay as secondary end points. Patients and methods A before-and-after prospective study was conducted between January 2019 and December 2021 in the hepatopancreaticobiliary and liver transplantation unit of Air Force Specialized Hospital. We included 39 patients who underwent pancreaticoduodenectomy with resectable or borderline resectable preampullary tumors and were divided into two groups: group A included 17 patients who underwent standard pancreaticoduodenectomy, and group B included 22 patients with Roux-en-Y HJ with an isolated biliary limb. Results The overall incidence of DGE in the study group was 33% (13/39 patients). It was lower in group B and showed significant statistical values in all grades A, B, and C, with P values of 0.015, 0.000, and 0.023, respectively. After the use of isolated biliary limb in group B, the incidence had decreased significantly to 13% (3/22 patients), with significant prolongation of the mean hospital stay by a mean of 6.34 days longer in group A (P=0.016). The prolonged need for nasogastric tube was recorded in three (17.64%) of 17 cases in group A, two-thirds of which were grade A and one-third were grade B, with P values of 0.022 and 0.041, respectively, when compared with only 4.55% of patients in group B. The mean time needed until the removal of the nasogastric tube was significantly lower in group B (2.1±0.43 vs. 3.9±1.65 days), with P value of 0.037, with no incidence of reinsertion on both groups. When considering the solid oral intake tolerance, group B patients developed at a faster rate, with a mean of 3.33±2.88 days, whereas group A needed 8.46±2.81 days to reach tolerance (P=0.055). Conclusion The technique of reconstruction with isolated biliary loop away from both HJ and gastrojejunostomy in pancreaticoduodenal resection markedly reduced the postoperative incidence and severity of DGE reflected in a lesser duration of hospital stay.","PeriodicalId":43393,"journal":{"name":"Egyptian Journal of Surgery","volume":"41 1","pages":"1675 - 1684"},"PeriodicalIF":0.1,"publicationDate":"2022-10-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"47638133","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Therefore, attention is no longer given to the donor site only but also to the recipient area. Thus, internal mammary perforators (IMPs) are adopted to be used as recipient vessels in autologous free flap breast reconstruction as it carries many advantages in comparison with the internal mammary, or thoracodorsal vessels. Our aim in this study was to evaluate the clinical reliability and to test the efficacy of using these perforators as recipient vessels in Egyptian population especially fatty patients coming after radiotherapy for delayed breast reconstruction. Patients and methods A prospective analysis was performed on 16 Egyptian patients who visited Ain Shams University hospitals over the course of 2 years, from September 2020 to September 2022.Deep inferior epigastric perforator flap breast reconstructions were performed by a single surgeon on all patients. The use of IMP vessels was attempted, and intraoperative measurements of the vessels and flaps were collected. Results All of the included patients underwent delayed breast reconstruction surgery. In four (25.0%) patients, IMP vessels were used. The subcutaneous plane (57.1%) is where the majority of the perforator vessels are situated. The third intercostal space held 64.3% of the perforator vessels, whereas the second held 35.7% of them. The diameter of IMPs was considerably less than that of internal mammary arteries. With the exception of one patient who needed debridement due to fat necrosis, all recipients were appropriate and trustworthy as recipient vessels without experiencing any serious morbidities such flap loss, fat necrosis, or mastectomy skin flap necrosis. Conclusion Despite their relatively small size, IMP vessels are trustworthy and safe to employ as recipient vessels in free flap breast reconstruction because they achieve a less-invasive procedure than using internal mammary vessels.","PeriodicalId":43393,"journal":{"name":"Egyptian Journal of Surgery","volume":"41 1","pages":"1834 - 1842"},"PeriodicalIF":0.1,"publicationDate":"2022-10-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"47412020","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Patients A total of 316 patients who had RLQ pain were evaluated clinically and by abdominal ultrasonography, and their blood samples were taken for estimation of total and differential leukocytic count and estimation of serum levels of C-reactive protein. Then, patients received the appropriate surgical procedure. The predictability of the Alvarado score, appendicitis inflammatory score (AIS), Lintula score, the calculated neutrophil/lymphocyte ratio, the systemic inflammatory response index (SIRI), and the systemic inflammation index for operative findings was statistically evaluated. Results The Alvarado score showed significantly higher diagnostic performance for appendicitis necessitating appendectomy in comparison with AIS (P=0.049) and Lintula score (P=0.044). For gynecological pathologies requiring surgical intervention, the diagnostic performance of the Alvarado score was significantly higher than that of AIS. The diagnostic performance of the three leukocyte ratios showed nonsignificant differences. Multivariate regression analysis defined a high SIRI ratio and Alvarado clinical score as the variate that can discriminate cases that required appendectomy and high SIRI and neutrophil/lymphocyte ratios as the significant predictors for gynecological cases requiring surgical intervention. Regression analysis confirmed the ability of low SIRI and low AIS scores to indicate canceling of laparotomy. Conclusion Surgical decision making for patients who had RLQ pain could be improved by combining the Alvarado score with the determined differential leukocytic indices.","PeriodicalId":43393,"journal":{"name":"Egyptian Journal of Surgery","volume":"41 1","pages":"1666 - 1674"},"PeriodicalIF":0.1,"publicationDate":"2022-10-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"47471345","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
