{"title":"THE PROFILE OF NEONATAL SEPSIS IN DUHOK CITY AND PREDICTORS OF MORTALITY: A PROSPECTIVE CASE SERIES STUDY","authors":"A. Risk factors represent an interaction between maternal immunity and the defense mechanisms of the neonate. epidemiological, clinical and laboratory profiles of neonates with sepsis in relation to outcome and to determine the predictors of outcome. Subject and Methods: A prospective study included neonates with sepsis admitted to neonatal care unit. 126 neonates with features of sepsis were included with age ranged from (1-30) days. From each patient, neonatal and maternal data were collected and clinical features as well as laboratory test results of hemoglobin, platelets count. total white blood cell and absolute neutrophil count , C statistically analyzed. Results: of 126 neonates, 32 (25.39%) died while others survived. Age < 7 days was in 61.9% of all cases, 69.84% had respiratory distress syndr encephalopathy, 60.31% were preterm, 61.9% were born vaginally and male to female ratio was 1.73:1. There is a significant relation of mortality to respiratory distress syndrome and hypoxic ischemic encephalopathy, preterm d Vomiting, apnea, sclerema, cyanosis and tachypnea were significantly related to the mortality. Eschericia coli were mortality is with Acenatobacterbaumani relation. The C reactive protein was>10 mg/dl was in higher number of neonates with sepsis who died by comparison to those who survived, with a significant relation. Conclusions: Neonatal sepsis is still a c the pattern of causative organisms and this requires more monitoring and periodic surveillance. {"title":"EFFECT OF ADDING DIFFERENT DOSES OF DEXAMETHASONE TO BUPIVACAINE ON INTRATHECAL ANESTHESIA IN CESAREAN SECTION","authors":"A. However, the short-term duration of local anesthetic drugs’ action has created a matter of concern for anesthesiologists. The purpose of this investigation was to evaluate the effect of different doses of dexamethasone conjugated with bupivacaine on the duration of spinal anesthesia as the primary outcome, and their adverse effects were examined among patients underwent cesarean section as well. Subject and Methods: In the current double-blind clinical trial, 90 pregnant women scheduled for cesarean section were randomly assigned into three groups after taking written consent. Patients assigned to the group A (n=30) received intrathecal bupivacaine 12.5mg (2.5 ml) of 0.5% hyperbaric bupivacaine diluted in preservative free normal saline (1 ml), group B (n=30): received 12.5 mg (2.5 ml) of 0.5% intrathecal hyperbaric bupivacaine and 2 mg (0.5 ml) preservative free dexamethasone, diluted in preservative free normal saline (0.5 ml), overall 3.5 ml volume intrathecally, and group C (n=30): received 12.5 mg (2.5 ml) of 0.5% hyperbaric bupivacaine and 4 mg (1 ml) preservative free dexamethasone, overall 3.5 ml volume intrathecally. Results: In the present study the crude mean age of the patients was 26.49±5.49 (range: 1840 years) and the patients’ age, weight, and body mass index were comparable among three groups (p>0.05). The study showed the sensory block (min) in patients was significantly higher in groups 2 and 4 mg dexamethasone; 284.63±64.85 and 313.37±70.53, respectively compared to the control or group control, 165.33±44.95 minutes. Similarly, the motor block (min) in patients assigned groups 2 and 4 mg dexamethasone was substantially higher; 223.43±52.67 and 227.20±47.17, respectively in comparison with it’s in group control, 126.33±34.62 minutes. However, the difference between groups 2 and 4 mg dexamethasone not substantial for both sensory and motor blocks (p>0.05). The patients of three groups had not substantial difference in frequency of post-operative adverse effects, including postopetaive nausea & vomiting, hypotension, bradycardia, shivering, post-dural-puncture headache. Conclusions: administration of 2 or 4 mg dexamethasone intrathecally prolongs the duration of sensory and motor block of bubivacaine substantially in patients underwent cesarean section under spinal anesthesia and the effect of adding 2 mg on the duration of sensory and motor blocks is same like adding 4 mg. There is no national oral survey has been carried out to determine the prevalence of oral health problems in this group people (Singar people). Subject and Methods: The aim of this study was to estimate the prevalence and severity of dental caries in 6-12 year old children in Sharia camp of refugees from Singar. Material and methods: Thecross section study was carried out in elementary school children, which were randomly selected fromSharia camp of IDP Singar people in Kurdistan of Iraq. Results: Results showed the mean dmft and DMFT scores were (2.45 – 2.606) and (0.927±1.583), respectively. Also, 23.4% of the students were caries-free. Conclusions: It was concluded that the present study findings for dmft and DMFT scores in 6-12 year old elementary school children are higher than global standards according to the World Health Organization (WHO). Subject and Methods: This is a case-control study carried out on 55 patients with established RA attending the Rheumatology ward of Ibn-Sina Teaching Hospital in Nineveh Governorate during the period from 1 November 2010 to the 1 June 2011 and 35 apparently healthy individuals as a control. Anti-CCP2 antibodies were measured using enzyme linked immunosorbent assay (ELISA).The RF was tested by latex agglutination and ELISA. C-reactive protein (CRP) was measured by Latex agglutination test. Patients' demographic data, disease activity and duration and erythrocyte sedimentation rate (ESR) were also recorded Results: Anti-CCP2 antibodies were found significantly associated with RF, total swollen joints (TSJ) and total tender joints (TTJ) counts (p <0.05) while the correlation with ESR, CRP, disease activity score 28 (DAS28) and disease duration was not significant (p>0.05). In RA patients with smoking history and joints deformity, anti-CCP2antibodies was more often detected. The same results were obtained for RF seropositivity. It was also found that the differences between RF and RF groups were comparable to those between anti-CCP2 and anti-CCP groups except for ESR and CRP. The causes are unclear, but dysregulation of the immune response has been proposed to be implicated in the development of the disease. In this study, we hypothesized that RAS is provoked by the dysregulation of the complement system, through the impairment of the function of complement regulatory proteins, the present study has aimed to investigate the impact of CFH gene SNPs that encodes the production of complement factor H in RAS development. Subject and Methods: Blood samples from 46 patientswith recurrent aphthus stomatitis were collected including 35 males and 11 females and 46 age and sex matched apparently healthy (with healthy oral cavity) volunteersincluding 23 males and 23 females were involved as a control group. Gnomic DNA was extractedfrom each blood sampleusing the isopropanol/ethanol method. Specific primers were used to amplify the CFH gene fragment that harbors the rs1061170 site encoding theTyr402 aminoacid. The PCR products were digested with NlaIII restriction enzyme. Results: A significant difference was found between the age groups among the RAS patients in regard to the severity of and recurrence of the RAS episodes, it was found to be higher significantly among the age group (20-30 years) compared to other age groups among RAS patients. Out of the 46 RAS patients, the CFH single nucleotide polymorphism (SNP), Tyr402His polymorphism variant was detected in 18 (39.1%); 11 (23.9%) females and 7 (15.2%) males, represented as 8 (17.4%) Tyr/His heterozygous variants and 10 (21.7%) were His/His homozygous variant. Among the 46 healthy control group, the CFH single nucleotide polymorphism (SNP), (Tyr402His polymorphism variant) was detected in 6 (13%) all of them were Tyr402His variant, 2 (4.3%) males and 4 (8.7%) females. There was a significant difference in the CFH (Tyr/His, His/His) variants rates between the RAS group and the healthy control group (p <0.05), but there was no significant difference of CFH (Tyr/His, His/His) variants rates between the males and females in the RAS group. Conclusions: We suggest that Tyr402Hispolymorphism can be considered as a risk factor for the RAS development, and His204His variant is more associated with the disease, however, more studies are recommended to be conducted on a larger sample size to confirm these evidences.
