Expertos de varias sociedades científicas internacionales publicaron en 2018 la Guía del manejo de patología herniaria inguinal para consensuar líneas de trabajo y mejorar resultados. La adherencia a sus recomendaciones es muy variable. Objetivo: Conocer la realidad nacional española del manejo de esta patología evaluando la adherencia a las recomendaciones de la guía. Método: Encuesta redactada ad hoc: 29 preguntas de respuesta múltiple siguiendo los capítulos de la guía que fueron respondidas entre junio y diciembre de 2019 mediante formato correo electrónico en cadena y de forma presencial en cuatro reuniones de cirujanos generales. Resultados: Se recopilaron 173 encuestas que representaban a la geografía española. El 76 % de los cirujanos eran adjuntos, aunque menos de la mitad realizan usualmente cirugía de pared abdominal La ambulatorización es la norma, aunque solo en el 10 % de casos se intervienen con anestesia local. El abordaje laparoscópico es poco frecuente y suele indicarse en hernias bilaterales y recidivadas. El 94 % de los encuestados piensa que la vía laparoscópica no se convertirá en el estándar para la cirugía de la hernia inguinal. Contrariamente a las recomendaciones de la guía, sigue utilizándose tapón (38 %) y las mallas autoadhesivas fueron bien valoradas (59 %). La mayoría cree subjetivamente tener buenos resultados, pero el 78 % desconoce sus cifras reales por carencia de estudios. La guía Hernia Surge tiene poca difusión en nuestro medio: no la conocen el 47 % y solo el 10 % la considera como referencia. Conclusión: Esta guía es poco conocida entre los cirujanos españoles, aunque muestran interés en actualizarse. Destaca la reticencia al abordaje laparoscópico. La mayoría se ajusta a las recomendaciones en cirugía abierta, pero son minoría los que auditan sus propios resultados. Objective: To assess the possible over-indication of imaging tests performed to diagnose inguinal hernia and identify the services that order most improper tests. Methods: 202 patients that underwent elective inguinal hernia intervention were included in this retrospective study. The image tests requested were analyzed, as well as their suitability and the requesting services. Results: In 55.4 % of the operated patients, an imaging test was performed (50.5 % ultrasound). Only 27.9 % were correctly indicated. Primary Care and Emergency services requested a greater number of unjustified tests (8.9 % and 5 % respectively).
