{"title":"Echocardiographic Findings in Patients with Transient Ischemic Attacks","authors":"Nardine Abdelsayed","doi":"10.47363/jcrrr/2022(3)166","DOIUrl":"https://doi.org/10.47363/jcrrr/2022(3)166","url":null,"abstract":"Echocardiography has a well-established utility for the detection of intramural thrombi and left atrial enlargement during routine evaluation of cerebrovascular accidents (CVA), however the use of echocardiography in transient ischemic attack (TIA) is much less established. Current guidelines from the American Heart Association (AHA)/American Stroke Association (ASA) recommend echocardiogram in ischemic stroke when cardioembolic source is suspected, but there is less clarity with regards to how results should be interpreted for ongoing patient care. We aimed to examine common echocardiogram findings in patients presenting with TIA. From 2018-2019, we examined all TIA patients over the age of 18 who were discharged from our comprehensive stroke center. Patients were excluded if they were diagnosed with acute stroke, demyelinating disease or intracranial mass found on MRI of the brain, or carotid stenosis greater than 50%. A total of 588 patients were identified and of these, 449 (76.36%) patients who met these criteria underwent transthoracic echocardiography (TTE) as part of their TIA evaluation. 130 (28.95%) of the 449 patients underwent echocardiogram with agitated saline. 2D echocardiogram reports were reviewed for the presence of left atrial size greater than 4 cm, atrial fibrillation, left ventricular thrombus, left atrial thrombus, moderate to severe mitral stenosis, patent foramen ovale, atrial septal defect, vegetation or mass on any valve, new left ventricle function less than 50%, and atrial myxoma. The overall incidence of mitral stenosis, patent foramen ovale (PFO), atrial septal defect (ASD), thrombus, atrial myxoma and vegetation was low and not statistically significant. PFO was discovered in 11 of the 130 (8.46%) agitated saline studies. The analysis showed that 173 of the 449 (38.53%) patients were found to have left atrial size greater than 4 cm. Of the patients with left atrial enlargement, 59 (34.10%) also carried a diagnosis of atrial fibrillation (AF) while 114 of the 173 (65.9%) did not. Ultimately, of patients without known AF, only 6 of 114 (5.25%) were discharged with cardiac monitoring. Symptoms which persisted after a 30-day readmission following surgical myectomy included; angina-like chest pain owing to myocardial ischemia, shortness of breath/dyspnea, atrial fibrillation and diastolic dysfunction. HOCM is the most common inherited cardiac disorder. Our study honed-in on the predictability of readmission’s surging healthcare resources; for example, the cost and length of stay utilization after surgical myectomy. Recent theories about the cause of hypertrophy in HOCM patients have been linked to inefficient utilization of ATP. The purpose of this study is to access the incidence, predictors, as well as the causes for readmission in HOCM patients who undergo surgical myectomy. We wanted to investigate the incidence so as to better predict the causes for symptoms after a thirty-day readmission rate following surgical myectomy. Readmission nationwide rates from January 2010 to September 2015’s database was investigated to find out about the 30-day unplanned readmission after surgical myectomy. Current practices in new onset AF include obtaining laboratory values such as thyroid stimulating hormone (TSH) with reflex thyroxine (T4). We aim to prove that such practices rarely change management and should be limited to patients with additional indicators of hyperthyroidism. From January 2015 to August 2021, a total of 59,470 patients that were 18 years and older were identified with an admission diagnosis of atrial fibrillation using the ICD-10 code I48.92. Patients were excluded if their sex or age were not identified and if they had a previous admission for atrial fibrillation. Out of the 54,968 patients admitted with AF, TSH was ordered in 7,444 (13.54%), free T4 was ordered in 2,669 (4.85%), and both in 2,285 (4.16%). Only 29 (0.013%) of the 2,285 patients who had orders for both TSH and T4 were found to have both low TSH and high T4, indicating an overt hyperthyroid state. Overall, of the 7,444 patients with TSH labs drawn, only 404 (5.43%) were discharged on methimazole or Propylthiouracil (PTU). The purpose of this study is to increase awareness of supportive care in the form of treadmill exercise for African Americans of a specific age group with resistant hypertension. We hypothesized that a structured exercise training program along with reducing regular three or four medications after the study improves symptoms and quality of life for African American patients within a specific age group with resistant hypertension. Methods: Total number of recruited patients were one hundred and eighty (180) between ages eighteen-forty-eight (18-48). Ninety patients (90) were in the experimental group while ninety (90) were in the non-experimental/control group. Forty-five (45) males each were in both research groups while forty-five (45) females each were in both the experimental and control group. Weight of participants was between 200-230 pounds. Data was collected from National in Patient sample (NIS) HCUP and extracted using SAS ver. 9.4 from ICD-9 for 2016. Control group used the treadmill exercise one to two days within the week for about 45 minutes while the experimental group exercised at least 3-5 days on treadmill for 45 minutes and attained accumulated weekly mileage of ≥ 2. Results: The study showed that decreasing blood pressure from ≥130/80 to normal bp of <120/80 mmHg for African Americans of a specific age group was statistically significant (p-value for AA males and females respectively 0.0166 and 0.0125. That is; p<0.05). This case report also reviews the prevalence of right sided endocarditis, diagnosis, common microorganisms, predisposing risk factors and medical management. Eustachian valve endocarditis was first described in 1986 by Edwards and from 1986 to 2017, only 37 cases have been reported— 46% of which were caused by intravenous (IV) drug use and 24% by indwelling intravenous lines. Other risk factors for the development of eustachian valve endocarditis include rheumatic heart disease, pacemaker wires and immunologic compromise. Although COVID-19 is known to initially manifest as a pulmonary disease, we now understand it affects many major organ systems in the body, including the heart. Case reports have been published showing evidence of right ventricular (RV) strain as a result of contracting the COVID-19. However, the overall prevalence of RV strain secondary to the virus has not been well established. In this multicenter, observational analysis, we studied the prevalence of RV strain in hospitalized patients with COVID-19. We found the prevalence of RV strain to be small, ~1.5%. The average length of stay in patients with RV strain was 7 days compared to 8.63 days in non-RV strain patients. Left ventricular diverticulum is challenging to diagnose due to the asymptomatic nature and lack of consistent ways to describe the outpouching, however it is known that it contains all layers of the heart [3-5]. Previous authors have tried to classify based on using the size of diverticulum compared to the size of the left ventricle [4]. Diverticulum can be classified as either muscular or fibrous and they display contractile features consistent with the remaining heart [3,4]. As much as 30% of left diverticulum cases are not associated with congenital abnormalities and can present with chest pain [3].
