This study aimed at investigating a number of issues relating to this form of assessment. The project piloted the Competency-based Assessment for Speech Pathology Students (CASPS), an observational assessment, adapted from the Competency Based Occupational Standards of the Australian Association of Speech and Hearing. The usefulness of the CASPS was surveyed, and the relationship between students' results on the CASPS and a traditional evaluation of the final clinical practicum was investigated. The relative contributions of assessment by direct observation and viva examination were determined. This study assessed the reliability of three groups of listeners in assigning a single rating on a 10-point scale to measure the severity of stuttered speech in 5-min speech samples from young children. The three rater groups were (1) clinicians experienced in stuttering management, (2) generalist clinicians, and (3) non-clinicians. Results showed promising intra-rater agreement, inter-rater agreement, and intraclass correlations for the three groups. The respiratory function of the CVA subjects was found to differ from the control group performance across a number of respiratory parameters. Specifically, both the spirometric assessments and the kinematic analysis of maximum-effort speech tasks identified reductions in lung volumes and chest wall movements during maximum-effort speech tasks in the CVA subject group that appeared to be the net result of reductions in both the abdominal and rib cage excursions. The perceptual assessments included an inventory of 32 perceptual speech dimensions, the Frenchay Dysarthria Assessment (FDA), and the Assessment of the Intelligibility of Dysarthric Speech (ASSIDS). The results revealed that the CVA subjects presented with features consistent with spastic dysarthria; however, the incidence of the dimensions identified was lower and the severity less than previously reported in the literature. Comparison between the CVA and the control groups revealed reduced speech intelligibility in the CVA group and deficits in all aspects of speech production including respiration, phonation, resonance, and prosody. Two recent studies (Kully & Boberg, 1988; Ingham & Cordes, 1992) found low levels of agreement in total stuttering counts across treatment centres. One possible reason for low agreement is the perceptual overlap between stuttering and normal disfluency. The present study investigated the intrajudge and interjudge agreement of seven experienced clinicians who worked closely together in the same treatment centre. In Part One of the study the judges were found to have high intrajudge reliability; however, there were considerable discrepancies in the judges' total stuttering counts. The difficulties for those who struggle to remain fluent following successful treatment for stuttering requires further analysis through investigation into relapse. The present study undertook a cross sectional questionnaire study of a large group of persons from an Australian self-help group who were dedicated to maintaining treatment gains in the long term. Relapse was defined as “stuttering to a degree which was not acceptable for at least a period of one week.” Altogether, 152 of 300 questionnaires were returned. Seventy percent of the participants believed they had relapsed, but the majority also believed they had regained fluency subsequent to that relapse. Relapse appeared to occur in cycles, with an average frequency of up to three times per year. The value of using proverbs for this purpose was examined with a random sample of 205 English-speaking residents of suburban Adelaide. Subjects interpreted three proverbs typical of those used for assessment. Only 15% of subjects correctly interpreted all three proverbs, and over a quarter of subjects did not correctly interpret any proverb, suggesting that proverb interpretation may lack specificity in identifying cognitive deficits. Performance was influenced by age, sex, educational achievement, and occupational classification. Normative data, which can allow for the effects of these factors, are not usually available for individual proverbs.
