The individuals were beneficiaries of the Spanish social thermalism programme. A total of 10 sessions were prescribed, individually, considering the clinical situation and preferences of each participant. In all cases, 10 bath sessions were administered, along with other techniques. The clinical assessments were conducted before initiating the balneotherapy and 1, 3, 6 and 9 months after. The main variable was HRQoL (Euroqol 5D-5L; WOMAC). The statistical analysis was performed by intention to treat.\u0000Results: The study included 71 users. At 6 months, improvements were found in the following dimensions of the WOMAC questionnaire: pain (24.47%), stiffness (30.75%) and functional capacity (26.66%) (p<0.05). Pain and anxiety/depression showed statistically significant differences in the EQ 5D-5L throughout the study.\u0000Conclusions: HRQoL in KOA would improve with the Balneotherapy, prevailing the effects for up to 6 months. {"title":"Systematization of Degrees of Complexity and Objectification of Clinical Assessment of Dysphagia of Oral and Oropharyngeal Stages of Swallowing","authors":"A. Kushta, S. Treatment of such patients, regardless of its type (surgical, radiation, chemotherapy) is quite traumatic, which leads to damage to surrounding tissues, disruption of their functions and before swallowing. Objectification of the severity of functional disorders is difficult in both the first and second phases of swallowing. There is also no systematization of severity, which could be used in the clinic in such patients. The aim of the work was to select and evaluate methods for diagnosing swallowing disorders and systematization of oral and oropharyngeal dysphagia.\u0000Materials and Methods: The study included 36 patients aged 38-55 years (men) who were treated in the Department of Head and Neck Tumors „Podolsk Regional Oncology Center” with malignant tumors of the tongue, bottom of the mouth and oropharynx stage I-III. All patients had problems with chewing and swallowing. A comprehensive method of assessing the effectiveness of chewing and ultrasound examination of the act of swallowing, the presence of the pain component on the visual-analog scale (VAS) at the time of hospitalization and in the postoperative period for 10 days.\u0000Results: Clinical and ultrasound studies have shown a reduction in muscle contraction in all patients, regardless of the location of the process. Changes were observed depending on the clinical course of the disease, the prevalence of the tumor and the volume of the muscles being incised. There was a tendency that patients who could not mix and form the food bolus were also unable to take a normal sip. These manipulations were accompanied by varying intensity of pain. Based on the obtained data, groups of patients by severity and their systematization were formed.\u0000Conclusions: The systematization of oral and oropharyngeal dysphagia on the basis of objective indicators is proposed (masticatory efficiency – masticatory test, ultrasound examination). To assess the first phase of swallowing, a set of studies with three methods is recommended: 1 – chewing test; 2 – visual-analog scale of pain; 3 – ultrasound examination. To assess the second phase of swallowing – two methods: 1 – visual-analog scale of pain; 2 – ultrasound examination. {"title":"Improving the Quality of Medical Care and Prevention in Patients with Type 2 Diabetes on the Basis of Remote Medical Service","authors":"V. Smiianov, Nataliia O. Dryha, L. Rudenko","doi":"10.36740/abal202201105","DOIUrl":"https://doi.org/10.36740/abal202201105","url":null,"abstract":"Aim: To analyze the effectiveness of implementation of remote medical service, involving patients with type 2 diabetes mellitus in dynamic monitoring of their health and treatment, designed to optimize the quality of medical care and prevention at the primary health care level. Materials and Methods: The study included adults aged 18 to 70 years, residents of Sumy who suffer from diabetes. They took part in a medical and sociological survey before introduction of the ICS and gave consent for processing and using of their personal data. A total of 96 patients were involved. We conducted a survey form of the study, using a closed questionnaire for medical and sociological research, conducted during December 2019. The study included such methods as systematic approach, bibliosemantic, comparative and statistical analyses, logical generalization. Results: During the following medical and sociological survey, 96 questionnaires from Sumy residents aged 18 to 69 years, 58 women (60,42±3,53%) and 38 men (39,58±3,53%)) were studied. Analysis of re-monitoring results showed that during the period of implementation of the ICS at the level of primary health care, the indicators of MCQ and health status of respondents have improved. The proportion of patients, who regularly control the level of blood pressure, increased by 32.29%, and those, who control the level of blood glucose – by 31.24%. The incidence of hypoglycemia and/or ketoacidosis states has decreased by 4.17%. During the survey, the part of respondents with targeted blood pressure level increased by 25.00% and the number of respondents with satisfactory level of diabetes compensation increased by 29.18%. The survey showed that more respondents (12.5%) began to regularly take prescribed medications; the number of those, who began to follow diet and dietary regimen has increased by 28.12%; 5.19% of respondents abandoned bad habits (smoking). The number of respondents who have a body mass index ≥ 30.0 has decreased by 4.69%. Materials and Methods: The experimental part of the research involved the use of the valid psychodiagnostic techniques (Hall’s method, the Mayer-Salovey-Caruzo Emotional Intelligence Test, ”Understanding Emotional States”, ”Human Drawing”), as well as methods of questioning and observation. We used the rank correlation coefficient according to the two-sided Student’s t-test in order to compare the results of the research. In experimental research involved 76 high schoolers in the age of 14-17 who studied in 7-9 grades. There were 45 children with intellectual disabilities and 31 high schoolers with normative development. Results: The conducted diagnostic work made it possible to establish both the integrative indicator of emotional intelligence and the levels of expression of its individual components. Low and medium levels of emotional intelligence development in high schoolers with intellectual disabilities was found: a decrease in the level of perception of emotions by expression; superficial perception of expressive features, vagueness of ideas about them; insufficient differentiation of the constituent elements of the emotional standard. Materials and Methods: 256 cadets took part in the retrospective study. The study was conducted in a survey form. The specially prepared questionnaire included questions about injuries and pain syndromes with an emphasis in the manifestation of symptoms of medial tibial stress syndrome. Results: 59.5% of respondents reported on the occurrence of injuries and pain syndromes during training in higher military educational institutions. The first year was pointed out as the most traumatic by the cadets of all year of studying. Out of all respondents 62% of the first year cadets, 37.8% of the second years, and 32.2% of the third years pointed the first year as the most traumatic. 83.2% of respondents indicated having an experience of injuries and pain syndromes in the lower limbs during training. It was found that 13.6% of injuries and pain syndromes in cadets occur in the lower limbs; among them 62.1% have the localization of pain on the medial surface. Conclusions: The highest number of injuries and pain syndromes in cadets occurs in the first year of studying. The most common are injuries and pain syndromes of the lower limbs, a third part of which are the injuries of the tibiae area. More than half of all injuries and pain syndromes of the lower limbs are localized on the medial surface, which means the risk of development of MTSS. Materials and methods: We studied 199 families. One member of each family contracted pulmonary tuberculosis and later became a source of the disease for contactees. The examination was performed with standard clinical techniques using radiological, bacteriological, and immunological examination techniques. Results: We found that in 12-24 months, 207 people were diseased in these families. We also revealed the shift toward a younger age among the diseased contactees. Special emphasis should be paid to the disease of children under 18–52 people, while there was only one patient among the sources of tuberculosis. There were more cases of familial tuberculosis in urban than in rural areas. Females were more likely to suffer from these forms of tuberculosis. Materials and Methods: The analysis of scientific literature based on the Web of Science database has been carried out. A computer search for the title “sports shoulder injury” revealed 2428 articles. Later, the search was specified. At the final stage, eight articles were analyzed. The analysis of selected articles suggests that the following exercises are recommended for the prevention and rehabilitation of shoulder injuries: proprioceptive, resistance exercises (e.g., kinesiology tape), passive exercises, exercises for the upper extremities with closed and open kinematic chains, isokinetic, plyometric and specialized sports exercises (depending on the sport). Besides therapeutic exercises, for the prevention of sports injuries of the shoulder, we admit it is appropriate to consider the method of kinesiology taping as a promising means of physical therapy. Conclusions: Among the therapeutic exercises to prevent of sports injuries of the shoulder, we consider promising proprioceptive, isokinetic, exercises with open and closed kinematic chain, exercises with resistance. It is necessary to study the feasibility of using these exercises and develop appropriate preventive measures and recommendations in the training process. Conclusions: The work of the multidisciplinary rehabilitation team plays an important role in the restoration of athletes, so their choice should be more individualized with the aim of more effective restoration of functional capabilities of athletes’ performance. The influence of training and competitive loads on the athlete’s body is quite diverse and can cause various appropriate reactions, which are largely determined by the functional state of his body. The effectiveness of the restorative effects is closely related to the initial functional state, and the same procedures, can both accelerate recovery processes and be an additional burden on the body. {"title":"Assessment of Home-based Physical Therapy in Stroke Patients","authors":"W. Aim: The aim of the study was to assess the effects of 6-month home-based rehabilitation in stroke patients. Material and Methods: The study included 37 patients (35% were women and 65% were men) after ischaemic stroke. Study patients received 6-month home-based physical therapy in Kielce County (Gmina Chęciny and Gmina Łopuszno). The study used the Barthel Index, the Timed Up and Go Test and the Modified Rankin Scale. Physical therapy included kinesiotherapy methods (such as active and assisted exercises), physiotherapy methods (electrotherapy, laser therapy) and special methods (PNF and kinesiotaping). Results: The study showed significant differences in the results between the pre- and post-rehabilitation assessments. Before rehabilitation, the majority of study patients (22 people, 60%) scored between 21 and 85 points according to the Barthel Index, which suggests moderately severe disability. After rehabilitation, this number was reduced to 20 patients (54%). With respect to the Timed Up and Go Test, most study patients (25 people, 68%) both before and after rehabilitation needed 10 to 19 seconds to perform the test, indicating limited functional mobility. The degree of disability according to the Modified Rankin Scale was 4 in 23 study patients (62%) before rehabilitation and in 27 study patients (73%) after rehabilitation. A total of 8 study patients (22%) before rehabilitation had very severe disability with a result of 5 according to the Modified Rankin Scale; only 3 study patients (0.1%) did not show any improvement after rehabilitation. Conclusions: 1. Home-based physiotherapy performed over a period of 6 months had a very beneficial influence on increasing independence in stroke patients. 2. A significant gait quality improvement was seen in stroke patients. 3.
