Her only complaint was infertility. She was not suffering from chronic pelvic pain or increased vaginal discharge neither before nor after marriage. Diagnosis of cystic cervicitis was made by both transvaginal ultrasound examination and MRI. As her main problem was infertility, she refrained from doing cone biopsy and opted for an ICSI trial and aspiration of the cervical cysts at the same time of ovum pick up (OPU). Ovum pick up resulted in the recovery of two oocytes. On the same setting and using the same needle, aspiration of all of the cervical cysts was performed successfully. Aspirated fluid was sent for cytology as well as culture and sensitivity. The available healthcare infrastructure also determines that surgical approach considered for the patients and needs some customisation. In this case report, the case of Non-Descent Vaginal Hysterectomy along with the right sided & left sided salpingo-oophorectomy in rural tribal belt tertiary healthcare center in Dahod, Gujrat, India is reported. Total weight of the tumor removed measured 6.5Kg out of that 6.1Kg was fluid and 400 Gram solid component. On histopathology report confirmed serous cystadenoma. Sánchez-Díaz, Andrés Aguilar-Silva","doi":"10.31487/j.crogr.2022.01.03","DOIUrl":"https://doi.org/10.31487/j.crogr.2022.01.03","url":null,"abstract":"Introduction: Currently, maternal-fetal vertical transmission has not been reliably demonstrated; however, it has been shown that when there is a maternal cytokine storm, it can severely affect the fetus, leading to risks of fetal brain development abnormalities and risks of autism spectrum disorders.\u0000Materials and Methods: In a descriptive and retrospective study carried out from June 2021 to June 2022, admitting pregnant patients over 18 years of age with a diagnosis of COVID-19 by positive antigen test or PCR-RT, previous symptoms and comorbidities are analysed, as well as the presence of obstetric complications. associated and the use of vaccines during pregnancy.\u0000Results: 58 pregnant women with an established diagnosis of COVID-19 are entered into the study; the average age is 28 years; regarding comorbidities; only 2 reported suffering from chronic hypertension and 10 are overweight/obese. 40 patients were reported during the 3rd wave of COVID (June to December 2021) dominated by the Delta variant and 18 patients during the 4th wave dominated by Omicron (January to June 2022). According to the gestation trimesters, the majority were in the first trimester with 32 patients (55%), in the second trimester with 18 patients (31%), and only 8 patients in the third trimester (14%). Only 10 patients (17%) presented obstetric events associated with acute COVID-19 infection, highlighting threatened abortion and threatened preterm labour.\u0000Conclusion: This cohort of pregnant patients with COVID-19 infection, remained free of serious disease thanks to vaccination, and being a young and healthy population, the importance of preventive vaccination in the gestation stage is of utmost relevance to avoid both serious maternal illness as well as poor fetal outcomes. It is mostly associated with history of uterine surgery, especially previous cesarean section but can rarely occur in unscarred uterus. Diagnosing this condition in the absence of uterine scar requires a high degree of suspicion and fetal heart tracing abnormalities remain the most common symptom.\u0000Case Report: 21-year-old G2P1 was admitted in latent labour. Pitocin was used for augmentation. Fetal heart tracing was in category 1 except an episode of bradycardia accompanied by uterine tachysystole that was resolved by resuscitative measures and turning off Pitocin. Later in the labour course, the FHT showed recurrent late and variable decelerations, cervix was found to be fully dilated at the time. The unicornuate uterus accounts for 2.4-13% of all Mullerian anomalies and has a prevalence of 1:1000. A unicornuate uterus may be present alone or with a rudimentary horn and 75-90% of rudimentary horns are non-communicating. A pregnancy in the rudimentary horn may lead to uterine rupture, haemoperitoneum with a high risk of maternal mortality. The association of a unicornuate uterus with a non-communicating uterine horn to the VACTERL association of defects (vertebral anomalies, anorectal malformations, cardiovascular anomalies, tracheoesophageal fistula, esophageal atresia, renal anomalies and limb defects) has been described only twice in the literature. We report a patient who was born with an extra thumb and had a trachea-bronchial remnant with oesophageal stenosis that were both operated on earlier, and they were associated with a non-communicating rudimentary uterine horn. To the best of our knowledge, this is the third case reported in the literature of a VACTERL association to a unicornuate uterus with a non-communicating functional rudimentary horn. Given the high risk for the patient if pregnant an early diagnosis is paramount. A case is \u0000illustrated from Northern Ireland in which an ectopic pregnancy was discovered in the tubal stump \u0000previously undergone salpingectomy.\u0000Case Facts: A 34-year-old woman (G2 E1) with a tubal stump ectopic of a previous cornual excision two \u0000years prior, presented to EPAU complaining of mild abdominal pain and 6 weeks amenorrhoea. She was \u0000clinically stable (β-hCG 1407 mIU/mL), while TVUS revealed no evidence of IUP or adnexal mass but \u0000fluid in the Pouch of Douglas. Laparoscopic salpingectomy was performed after a diagnosis of tubal stump \u0000ectopic.\u0000Inference: Women who has had a previously ectopic pregnancy are at a significantly greater probability of \u0000relapse. In obstetrics, the focus is on \u0000the effects on pregnancy and fetuses. Here we present two cases of parturient women that demonstrate the \u0000high risk of postpartum SARS-CoV-2 co-infections.\u0000Materials and Methods: Patients` data were obtained from their medical records after confirmation of \u0000written informed consent. Literature review was conducted across PubMed.\u0000Case Report: i) A 33-year-old Caucasian Gravida II, Para I was transferred to our tertiary hospital at 23+1 \u0000weeks of pregnancy with previous preterm premature rupture of membranes (PPROM) at 17+5 weeks of \u0000gestation. During antenatal corticosteroid therapy for fetal lung maturation with 24+0 weeks, intrauterine \u0000fetal death was diagnosed in the course of amniotic infection. Due to progressive hemodynamic instability \u0000and confirmed SARS-CoV-2 co-infection, our patient had to be treated with catecholamines for 24 hours \u0000and stayed at the intensive care unit (ICU) for 72 hours. An acute myocardial injury occurred during septic \u0000shock. Immediate intensive care prevented permanent damage. ii) A 27-year-old Caucasian Gravida III, \u0000Para III was admitted to our tertiary hospital, 11 days after her third caesarean section. After circulatory \u0000collapse at home, the patient arrived with signs of hemorrhagic shock. Emergency curettage was performed, \u0000and because of cardiopulmonary worsening pulmonary embolism was suspected. Computed tomography \u0000revealed typical signs of COVID-19 pneumonia. Our patient was treated by nasal oxygen in the ICU for 24 \u0000hours. Similar to the first patient, acute myocardial injury occurred without any lasting harm. \u0000Conclusion: Our two cases show rapid and worse clinical courses in parturient women with SARS-CoV-2 \u0000co-infection. Rapid diagnosis and availability of intensive care were crucial for the prevention of long-term \u0000harm. A KRAS-mutation and c-MYC-amplification were detected, and\u0000we initiated a therapy with the CDK4/6- inhibitor Palbociclib in combination with the steroidal aromatase\u0000inhibitor exemestane. We postulated that the combination therapy leads to a blocking of the c-MYCamplification-activated cell cycle, proliferation, and inhibition of apoptosis, further enhanced by other\u0000oncogenes like mutated RAS. Their\u0000average age was 34.33 years and the diagnosis was based on clinical-biological clues: epigastric pain with\u0000strong elevation of lipasemia. Biliary etiology was confirmed in all patients initially by ultrasounds. Two\u0000patients had benign pancreatitis of favorable evolution. The third patient presented with a clinically severe\u0000pancreatitis and systemic inflammatory response syndrome, multiple organ failure syndrome with maternal\u0000and fetal deaths.
