It has been shown to improve prognosis and survival rate in patients of colorectal, ovarian and gastric cancers with peritoneal metastasis. A 57-year-old man diagnosed with primary colorectal carcinoma and pseudomyxoma peritonei was posted for HIPEC surgery. General anaesthesia with epidural analgesia (pain management) and invasive lines (arterial, central) for managing fluid shifts were used. Stroke volume variation was measured throughout the 19-hour procedure for goal directed fluid therapy (GDT) and to maintain urine output. Aim of this study is to determine the association of hyperchloremia with rise in C-reactive protein (CRP), serum ferritin, lactate dehydrogenase (LDH), serum creatinine and anaemia in COVID-19 patients in intensive care unit (ICU). It was a single-centre study, 62 COVID-19 patients participated. Change in CRP, serum ferritin, LDH, serum creatinine, haemoglobin between day one and day three of ICU admission were noted. Outcome in terms of in-hospital mortality was noted. Hyperchloremia (>106mmol/L) or rise in chloride by > 5mmol/L was seen in 13/62 (20.96%) patients. It was not associated with a rise in CRP, ferritin, LDH, creatinine, drop in hemoglobin within 72 hours or in-hospital mortality. 44/62 (71%) patients survived and were discharged from hospital. Multivariate logistic regression showed that if age and gender are also considered, there is a 25 times higher chance of having had a significant ferritin rise (> 95 ng/mL) in 72 hours of ICU admission among those who died. (p=0.01). Hyperchloremia (>106mmol/L) or a rise in chloride by > 5mmol/L in 72 hours of ICU admission was not associated with a significant increase in CRP, LDH, ferritin, serum creatinine within 72 hours of ICU admission or mortality in moderate-severe COVID-19 patients. After noting down the baseline vitals, intravenous ketamine 1mg was given. The child was carefully positioned, and caudal anaesthesia was performed using 3ml of 2% lignocaine with adrenaline and 7 ml of 0.25% bupivacaine. The child was positioned supine carefully with adequate padding and was sedated using dexmedetomidine infusion with a loading dose of 10 microgram over 15 minutes and maintenance dose of 5 microgram per hour. The surgery lasted for two hours. Anaesthesia for such cases could be demanding which would require one lung ventilation and intubation in unfamiliar positions. Depending on the extent of underlying injuries, sudden haemodynamic collapse should be anticipated, and precautions should be taken to manage such occurrences. We present a unique case of a safely conducted anaesthesia and surgery for a thoracoscopic removal of a long-bladed knife. One lung ventilation with a double lumen endotracheal tube placed at right lateral position in the absence of a fiber–optic bronchoscopy with improvisation using available equipment is described. The size and depth of the veins relative to the size of the cannula, in-situ thrombi, length of the catheter inside the lumen of the vein and the use of hyperosmolar solutions are implicated. Smaller the size of the vein, higher the risk of infiltration. It is recommended to choose veins with an antero-posterior diameter of at least 4 mm in some studies, although much smaller veins could be cannulated with appropriately sized catheter. The ratio between the external diameter of cannula to antero-posterior diameter of vein comes to play in minimizing infiltration, as essentially a ratio of < 0.3 or catheter occupying less than one-third of the area of the venous lumen is associated with reduced incidence of infiltration. Similarly, increased length of the catheter inside the lumen, lessen the risk of infiltration. Ultra-long or midline catheters and use of superficial veins have been proposed in view of increasing the in-situ length. The plane of ultrasonic view, either long-axis or short-axis, was not found to be superior to one another in averting infiltration.
