Empirical administration of fluid preload is time consuming, costly and might lead to volume overload, especially in elderly patients. Therefore, predicting post spinal hypotension is of great importance to reserve fluid infusion for susceptible patients. Ultra-sonographic evaluation of the internal jugular vein (IJV) diameter has been used to estimate intravascular volume in the ICU. Therefore, we investigated the efficacy of sonographic measurements of the internal jugular vein parameters for predicting post spinal hypotension in patients undergoing orthopedic surgeries. Methodology: The study was conducted on 62 patients scheduled for orthopedic surgeries. Sonographic measurements of IJV diameter and cross-sectional area were recorded at supine and Trendelenburg position before spinal anaesthesia. Blood pressure was monitored for 15 minutes for all patients and hypotension was documented. Results: Twenty-eight patients (45.2%) developed hypotension. The rate of increase in the diameter and cross-sectional area of the IJV with the change in patient’s posture was significantly higher in hypotensive patients. According to ROC curves analysis, a rate of increase of 12.36% in IJV diameter, and a rate of increase of 25.55% in IJV cross sectional area showed moderate specificity and sensitivity in the prediction of spinal hypotension. Conclusion: Preoperative sonographic measurements of the internal jugular vein diameter and cross-sectional area at supine and Trendelenburg position can predict the occurrence of post-spinal hypotension.","PeriodicalId":41531,"journal":{"name":"Sri Lankan Journal of Anaesthesiology","volume":" ","pages":""},"PeriodicalIF":0.2,"publicationDate":"2023-05-02","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"41882777","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

We present a 33-year-old pregnant woman who was diagnosed with a large thymoma causing marked compression of the tracheobronchial tree and right atrium. After various multidisciplinary meetings she presented for elective caesarean section delivery at 31 weeks of gestation. A combined spinal-epidural anaesthesia was performed, along with colloid pre-and co-loading, and vasopressor support. The delivery was uneventful. The possibility of catastrophic complications was foreseen. Therefore, all requirements for the possibility of airway or haemodynamic collapse were planned carefully, including the possibility of emergent cardiopulmonary bypass.Copyright © 2023, College of Anaesthesiologists of Sri Lanka. It is not available in some places and thereby forcing the operator to rely on conventional laryngoscope. Intubations in critically ill COVID-19 patients are challenging and may lead to adverse complications which may be affected by the laryngoscope used. The aim of the study was to analyse predictors of difficult intubation, complexity of intubation and complications of using conventional laryngoscopy in intubations of COVID-19 patients in ICU. Method(s): A prospective observational study was done in COVID Intensive Care Unit of the Base Hospital-Teldeniya, Sri Lanka from 1st of January 2021 to August 2021. The data was collected using a Performa filled by operator at the end of the intubation. Apart from demographic data and clinical parameters, data related to intubation including MACOCHA score which is a validated score predicting difficult intubation in ICU(3) and Intubation difficulty scale (IDS) which measures the actual complexity of intubation(4) were collected. The complications related to intubation were also analysed. Result(s): Approximately 84 out of 91 emergency intubations were done using conventional laryngoscope. Out of them 50.72 % were male and 49.27% Were female. Mean age was 50.75 years. First pass success rate was 83.3%. 25% had MACOCHA score more than 3. Most patients scored on the desaturation component of the score. Despite this only 50% of the predicted difficult intubations actually had major difficulty in intubation. The commonest complication was hypotension. 12 had suffered cardiac arrest, and 4 of them were fatal. Conclusion(s): Intubation of Critically ill COVID-19 patients are challenging but can be done successfully with conventional laryngoscope. However, one must prepare for complications including cardiac arrest.Copyright © 2023, College of Anaesthesiologists of Sri Lanka.
