Matsumura","doi":"10.26676/jevtm.v6i1.233","DOIUrl":"https://doi.org/10.26676/jevtm.v6i1.233","url":null,"abstract":"Abstract\u0000Arterial access is essential in resuscitation of trauma and hemorrhagic shock patients and can be effectively used for rapid endovascular treatment such as resuscitative endovascular balloon occlusion of the aorta (REBOA) and interventional radiology (IVR), continuous invasive hemodynamic monitoring, and frequent blood tests. In the REBOA procedure, obtaining an arterial access is the first and most critical step. Arterial access can be obtained in three ways: i) blind/landmark technique, ii) ultrasound-guided technique, or iii) surgical cutdown technique. Resuscitative endovascular balloon occlusion of the aorta (REBOA) is a resuscitation procedure for refractory hemorrhagic shock. \u0000The REBOA procedure itself is simple compared to other endovascular procedures, such as angioembolization or stent graft placement. The concept of damage control surgery in obstetrics has become widespread, and resuscitative endovascular balloon occlusion of the aorta (REBOA) can be a means of resuscitation in PPH. However, the potential benefits of endovascular strategies in obstetrics are not fully understood. This review discusses surgical and endovascular strategies for perinatal resuscitation and hemostasis. \u0000The endovascular strategies include REBOA, arterial embolization, and common or internal iliac artery balloon occlusion (CIABO or IIABO). Uterine artery embolization achieves rapid definitive hemostasis while leaving fertility with a less invasive procedure. N-butyl-2-cyanoacrylat (NBCA) should be chosen as needed for coagulopathy. The obstetrics and resuscitation team would have to develop each command system, and control and cooperate in parallel. REBOA operators must be knowledgeable about these possible complications and their management, along with its utility. While complete occlusion of the aorta could be effective for the control of blood flow, there is always a concern for ischemia-reperfusion injury of the distal organs from the balloon catheter. REBOA can control the degree of occlusion by changing the injection volume of the balloon catheter. This is often attributable to non-compressible bleeding in the apical thorax, hemodynamic instability, and the anatomically challenging location of these vessels making them difficult to access and control quickly. While the traditional management of ASI was with open surgical repair (OSR), recent years have seen an evolution towards less invasive endovascular repair (EVR). In patients with these injuries, EVR may be a safer alternative that achieves similar immediate results with significantly lower complication and mortality rates than the highly morbid open surgical option. In this article, we review and compare the two approaches, providing an overview of patient selection, anatomic considerations, techniques, postoperative management, and outcomes. Currently available conduits, such as autologous vein and synthetic grafts, are limited in one or more of these areas. The Human Acellular Vessel (HAV), a bioengineered, acellular blood vessel, can be obtained “off the shelf” and has shown promise in each of these properties. We describe a case in which the HAV was utilized for open bypass reconstruction in a patient with chronic limb-threatening ischemia who lacked alternative reconstructive options.
