On the day of her visit, she took her daily pills without putting on her glasses. Immediately, she felt something get stuck in her throat. In the emergency room, an X-ray of the neck showed an ovoid soft tissue structure near the cervicothoracic junction. Subsequent endoscopy revealed a pill in its blister pack lodged in the upper esophagus. The pill was extracted using biopsy forceps. Thompson","doi":"10.17756/micr.2021-066","DOIUrl":"https://doi.org/10.17756/micr.2021-066","url":null,"abstract":"Undifferentiated pleomorphic sarcoma (UPS), previously known as malignant fibrous histiocytoma, is the most common type of soft tissue sarcoma. Commonly seen in the extremities, UPS is an uncommon primary tumor of the GI tract. We report a case of an elderly female with a known renal neoplasm who incidentally was found to have a malignant lesion in the epigastrium. Given exophytic character abutting the stomach and pancreas, the mass initially thought to represent a gastrointestinal stromal tumor arising from the stomach. Biopsy of the mass showed atypical and pleomorphic malignant cells of unknown differentiation and was nonreactive with various immunohistochemical markers including both stromal and epithelial cell lines. During the lifting injury he felt the shoulder ‘pop out’. The humeral head reportedly reduced soon after, however, he continued to experience residual symptoms. Upon physical exam, there was audible clicking with shoulder abduction and a positive O’Brien’s test. Otherwise, motor strength and tone of the patient was intact with full range of motion.
