Abdellatif","doi":"10.21608/scumj.2024.342143","DOIUrl":"https://doi.org/10.21608/scumj.2024.342143","url":null,"abstract":"Background: The \"100 Million Health\" survey performed in 2019 in Egypt, which examined 49.7 million adult individuals aged 18 years and above, revealed that 39.8 percent have obesity, with female predominance. Further research into the effect of obesity in RA is necessary due to the established link between obesity and inflammation as well as changes in the body composition of RA patients. Objectives: To determine obesity prevalence in RA cases and its effect on the functional outcome as well as the disease activity score (DAS). Patients and Methods: The current cross-sectional study included 104 RA cases who met the 2010 ACR/EULAR classification criteria. Subjects underwent laboratory investigations, clinical examinations, and anthropometric measures (Body mass index BMI and waist circumference/hip circumference WC/HC ratio). Patient functional status was assessed utilizing the health assessment questionnaire disability index (HAQ-DI). Results: It was found that 23.1% of RA patients cases have average weight, while 27.9% were overweight and 49% were obese, with significant female predominance. There was a significant increase in the scores of HAQ-DI and DAS-28 in overweight as well as obese subjects than those with normal BMI. It is involved in various cancer types and affects cancer cell physiology via involvement in complicated cell signaling and protein-protein interactions. im : Our work aimed to assess the expression patterns of SOX2 in prostatic adenocarcinoma in correlation with the histopathological findings to evaluate its role as a prognostic marker or possible therapeutic target. Materials and Methods: forty-eight specimens of prostatic adenocarcinoma were stained first H&E then immunohistochemically for SOX2 and scored by two histo-pathologists. Results: thirty-eight specimens showed positive nuclear expression of SOX2; with variable degrees. SOX2 expression showed a statistically significant association with different grade groups; with p-value = 0.025. There was no statistically significant association between its expression and the patient’s demographic data. Conclusion: Overexpression of SOX2 in Prostatic Adenocarcinoma could be an indicator of higher tumor grade and poor prognosis. It is usually added to local anesthetic in spinal anesthesia (SA). However, its use as an adjuvant in infant surgeries has not been well studied due to the scarcity of data. Aim: In this study, we aim to investigate the efficacy and safety of dexmedetomidine combined with bupivacaine in SA. Materials and Methods: in this randomized clinical trial, we included 46 infants who underwent infra-umbilical surgeries. We allocated patients into two groups: group 1 received bupivacaine only, and group 2 received 0.3μg/kg dexmedetomidine in addition to bupivacaine 0.5%. We assessed efficacy through evaluation of the onset and duration of sensory and motor block and time to discharge from the post-anesthesia care unit (PACU), while safety was assessed through monitoring of hemodynamics and rate of complications. Results: we found that using 0 .3μg/kg dexmedetomidine in addition to bupivacaine 0.5% (group 2) was associated with a significantly prolonged duration of sensory and motor block compared to group 1. Time to discharge from PACU was earlier with group 2, but not significantly different from group 1. Although Group 1 did not witness any complications; group 2 had limited side effects that were managed appropriately.
