In the context of philosophical problems of death and dying, the ideas of perception of time, human fragility, the meaning of life, trauma, dignity of the individual, which form the modern understanding of the phenomena of death and dying in medical discourse, are analyzed. It is concluded that under the influence of the key ideas of philosophy, psychology, psychotherapy, palliative and hospice care should be transformed into a multidimensional activity that includes not only medical care, but also spiritual and psycho-corporeal, psychotherapeutic support, understanding of the uniqueness and spiritual dimensions of human life. Materials and methods. Regulatory and legal documents of Ukraine and authoritative international professional organizations were used; scientific literary sources using JAMA, Scholar and PubMed search engines; the Ministry of Health of Ukraine official statistical data; the following research methods were applied: bibliosemantic, system approach and system analysis. Results and Conclusions. The results of the scientific literature bibliosemantic research testify to the growing need for PHC both in Ukraine and in the world. The scientific base of the optimal concept of the PHC System is grounded on the following principles: availability, proper quality and maximum achievable efficiency. It should be noted that the current RLF, which regulates the PHC provision in Ukraine, needs further development and improvement in accordance with the current needs of the population and the requirements of international standards and norms. The Ukrainian Health Care System reform includes PHC System reform. Therefore, the accessible, high-quality and effective PHC System constructing and developing requires further development, improvement and optimization of the current RLF, which regulates the PHC provision in Ukraine. Adequate RLF is the basis and an important condition for the implementation of an optimal PHC System in Ukraine. An important task is the development of standards, quality indicators, clinical protocols and guidelines, improvement of the Procedure for PHC providing to adults and children, approval and improvement of indications and criteria for determining the status of PP in adults and children, etc. 3. The creation and development of a modern available, high-quality and effective PHC System in Ukraine meets the requirements of international standards and norms. The necessity of implementing a patient-family-oriented PHC System with multidisciplinary, interdepartmental and intersectoral approaches, due to the coordination and cooperation of organizations subordinate to various ministries and departments and privately owned institutions, the involvement of Non-Governmental Organizations (NGOs) and volunteers to provide general and specialized PHC. An urgent problem is also the need for appropriate funding of PHC by the state and regional and local authorities, the creation of new and modernization of existing PHC facilities, the training of a sufficient number of specialists involved in the PHC provision - doctors, nurses, social workers, psychologists, medical chaplains, support of NGOs and volunteers.","PeriodicalId":397576,"journal":{"name":"INTERNATIONAL JOURNAL REHABILITATION AND PALLIATIVE MEDICINE","volume":"58 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-02-25","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"123268823","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

In particular, the Resolution calls on governments and the World Health Organization to accelerate actions aimed at achieving the goals mentioned in the 2030 Agenda for Sustainable Development, adopted by UN General Assembly Resolution A/70/L.1 on 25 September 2015, in order to reduce premature deaths from cancer. One of the key points of this Resolution is to call on countries to develop standards and tools for management of planning and implementation of activities in the field of prevention, early diagnosis, screening, cancer treatment, as well as palliative care and assistance to people after treatment, including children with cancer. The difficult situation with the provision of palliative care to cancer patients in the terminal stage of the disease, the lack of sufficient material base and qualified staff requires additional involvement of the patient’s relatives and persons supervising them in the assistance. Improvement and clear legislative regulation of legal norms that in one way or another affect the effective construction of the relationships «doctor-patient-patient relative» should be the key to achievement of the maximum effect. A survey of 300 elderly people, medical and social specialists and statistical processing of these data was conducted. It is shown that the socio-psychological characteristics of temporarily displaced elderly people, the peculiarities of their social adaptation, insufficient funding and low level of medical and social care cause inadequate living conditions and require the establishment of a system of care and training of care staff. Curricula for training of medical, social specialists and volunteers on acquisition of theoretical knowledge and practical skills on care of elderly people are developed and implemented. Further prognosis of the disease depends on liver cirrhosis. Since in Ukraine liver transplantation is not available to most patients, the question of the economic feasibility of treating such patients remains open. According to the literature data, the survival rate in patients with cirrhosis and baseline MELD 10-15 is 96.0%, 93.6%, 89.7% within, respectively, 1, 2, 3 years after PPD treatment, and in patients with MELD 16 - 85.9%, 82.4%, 71.7%, respectively. The overall mean short-term change in MELD was less than one point, and a clinically significant improvement of 3 MELD points occurred in only 24% of patients. According to other studies, an improvement within 2 points of MELD occurs in 40% of patients with decompensated cirrhosis with SVR. In the remaining 60%, the condition does not change or worsens, i.e. antiviral therapy acquires the features of a palliative therapy that improves the quality of life of patients, but does not affect the outcome of the disease. The feasibility of prescribing antiviral therapy in patients with decompensated liver cirrhosis can be determined using the calculator, which assesses the likelihood of reaching CPT-A and the risk of death during the first year of follow-up. Purpose - to investigate that the grief reaction is one of the strongest and most painful human experiences. Materials and methods. There were 120 cancer patients observed. The reaction of grief consists of 4 stages: shock and protest - numbness, disbelief and acute dysphoria; absorption - acute longing, search and anger; disorganization - a sense of despair and acceptance of loss and decision. Results. The initial reaction of grief - shock, emotional numbness and disbelief. The excitement is most pronounced within about two weeks, followed by symptoms of depression, which reach its peak 4-6 weeks. Eventually, the former intense pain of severe loss begins to subside. In addition to the reaction of grief, there is a pathological, which is divided into suppressed (inhibited), delayed (delayed) and chronic. The role of the psychotherapist at this stage is to provide psychological support and assistance to both the patient and his environment to cope with this situation. Conclusions. The principle of the concept of palliative care is the need to ensure the psychological comfort of the patient. The research was carried out in accordance with the principles of the Helsinki Declaration. The informed consent of the patient was obtained for conducting the studies.
