Abnormalities observed in sirenomelia constitute the most severe form of caudal regression syndrome (CRS). It can be diagnosed by antenatal ultrasonography. The present study aimed to discuss a case with sirenomelia born from a mother with a history of gestational diabetes mellitus (GDM) and hypothyroidism. For families and healthcare professionals, caring for COVID-19-infected patients require complex symptom management, specific care goals, and psychosocial support. There are studies in the literature showing many favorable aspects of palliative care enabling achievements such as improvements in the quality of life, a shortened length of hospital stay, and reduced costs of care. Alizadeh","doi":"10.30935/ejmets/11218","DOIUrl":"https://doi.org/10.30935/ejmets/11218","url":null,"abstract":"Introduction & Background: The current study aimed to compare the effect of vein finder device and conventional venipuncture technique on anxiety and physiological indices of children with 3-6 years old in pediatric medical care.\u0000Methods and Materials: This is a randomized clinical trial which was conducted on 62 children between 3-6 Years old who were eligible according to the study inclusion criteria. This study was patients hospitalized in pediatric ward of Namazi hospital, Shiraz-IRAN, 2015. Vein finder device was applied in the intervention group while the control group receives the conventional venipuncture technique. Anxiety responses such as heart rate, respiratory rate, blood pressure, and blood oxygen saturation were assessed at 5 minutes both before and after the intravenous catheter placement procedure in both groups. Yale Preoperative Anxiety Scale (YPAS) was used to measure children’s anxiety scale. The data were analyzed by using paired t-test, independent t-test, and ANOVA test in SPSS software.\u0000Results: Both groups were measured after the intervention. Parametric physiologic results reveal a significant difference in heart rate (p<0.001) and respiratory rate (p: 0.002) for both groups. No significant difference was reported for blood pressure and blood oxygen saturation. Konar","doi":"10.30935/ejmets/11129","DOIUrl":"https://doi.org/10.30935/ejmets/11129","url":null,"abstract":"Objectives: The aim of this study is to analyze biochemical parameters and develop risk prediction models in patients with schizophrenia and bipolar disorder.\u0000Methods: Biochemical values of 65 schizophrenia and 65 bipolar disorder patients were statistically compared by analyzing thyroid function tests (TSH, T3 and T4), 25-0H vitamin D3, calcium, ferritin, iron, folate, vitamin B12, and magnesium levels. Risk analysis of the control group was evaluated in line with the obtained findings.\u0000Results: Male patients diagnosed with schizophrenia were observed to have lower T3 level than that of control group (p<0.001). Vitamin B12 value and Ferritin levels of male patients diagnosed with bipolar disorder were higher than those of control group (p<0.05), while their T3 levels were lower comparing to control group (p<0.001).\u0000Female patients diagnosed with schizophrenia were observed to have higher Ferritin levels (p<0.05) and lower T3 levels comparing to control group (p<0.001). When female patients diagnosed with bipolar disorder were compared with control group, their Ferritin levels and T4 levels were observed to be higher (p<0.05).\u0000Conclusion: Unless an intervention is made by detecting vitamin deficiency, the treatment may fail with psychotropic medications. In psychotic symptoms, in addition to antipsychotic treatment, replacement of low serum vitamin level may have positive effects on recovery. Erkut","doi":"10.30935/EJMETS/11094","DOIUrl":"https://doi.org/10.30935/EJMETS/11094","url":null,"abstract":"Objective: To investigate the frequency of sarcopenia in cirrhotic patients, the correlation of sarcopenia with conventional prognostic scores and its relationship with mortality by measuring the psoas muscle area at the level of mid-3rd lumbar (L3) vertebra on computed tomography (CT) scan.\u0000Materials and Methods: We included 229 cirrhotic patients who had undergone abdominal CT scan for hepatocellular carcinoma screening. A control group was formed in 204 healthy individuals who had undergone abdominal computed tomography examination due to trauma. 1- and 3-year follow-up results were recorded. Cross-sectional areas of the right and left psoas muscles were calculated in axial sections passing through the level of the mid-L3 in CT images. The cases with two standard deviations under the mean of the total psoas muscle area in control group were evaluated as sarcopenic.\u0000Results: Sarcopenia was detected in 39.7% of 229 cirrhotic patients. While a statistically significant difference was detected in the Child-Pugh prognostic score between cirrhotic patients with and without sarcopenia, there was a modest difference in the MELD score. Sadly, the absence of symptoms in some SARS-CoV-2 carriers has impeded the success of these current interventions, and the number of COVID-19 cases continues to increase globally. Understanding the impact and pattern of transmission by presymptomatic and asymptomatic cases is crucial to the management of the disease. We aimed to review the transmission or spread of COVID-19, to provide insight into the impact of these silent spreaders, and inform public health policymaking. A systematic review of published articles was conducted. PubMed, Google Scholar, and Science Direct were searched for articles published in the English Language from January to July 2020. A total of 20 articles were found eligible for the review based on the predetermined inclusion and exclusion criteria. 17 articles, of which 13 were case reports, 1 serial interval report, 2 investigational reports, and 1 modeling study, provided evidence of asymptomatic and presymptomatic transmission. 3 articles reported on the quantitative contribution of asymptomatic and presymptomatic transmission. Evidence provided in the literature is not sufficient to elucidate the impact and relative contribution of asymptomatic and presymptomatic SARS-CoV-2 carriers on the spread of the disease. With the continuous rise in the number of emerging and re-emerging infectious diseases such as monkeypox, it is high time we drew lessons and insights from past outbreaks to guide and effectively prepare for possible future outbreaks. Monkeypox, popularly known as the most important orthopoxvirus infection, is a viral zoonotic disease prevalent in Central and West Africa. There are 2 known genetic clades of the monkeypox virus reported so far, the Western African and the Central African clades. The human monkeypox though originated from Africa has grown to be a global concern with cases reported in the United States, United Kingdom, Israel, and Singapore. It is distinctively characterized by lymphadenopathy and systemic rash symptoms. Monkeypox has a low fatality rate (0-11%) with the highest rates occurring in children and young persons who were not part of the smallpox vaccinated population. Report suggests that consumption of bush meats could be a potential risk factor in the transmission of the infection or caring for an infected patient. Several factors have been responsible for the weak surveillance system of monkeypox including the current COVID-19 pandemic that has put more pressure on the fragile healthcare systems and infrastructures of most countries. This paper aims to guide global health policymakers on the impending danger of neglecting this disease.
