Tsutsumi","doi":"10.3805/EANDS.7.45","DOIUrl":"https://doi.org/10.3805/EANDS.7.45","url":null,"abstract":"Objective: We used three-dimensional stereotactic surface projection (3D-SSP) technique to analyze I-iomazenil single-photon emission computed tomographic (IMZ-SPECT) images in temporal lobe epilepsy (mTLE) resulting from hippocampal sclerosis, to assess the usefulness of 3D-SPP in identifying epileptic focus and demonstrating the relationship between IMZ uptake distribution and clinical profile. Methods: We compared the 3D-SSP images with the original IMZ-SPECT visual images in 26 cases. We assessed IMZ uptake quantitatively in each brain region in all patients and investigated the influence of laterality (involved or non-involved side), a past history of secondary generalized seizure (SGS), and treatment with benzodiazepine on IMZ uptake in each region. Current conventional therapies including adrenocorticotropic hormone and vigabatrin often fail to control the spasms and improve the long-term outcome, especially in cases with structural/metabolic etiologies. To improve this situation, new treatments with a disease modifying potential must be identified. Recent translational studies have led to the development of several animal models of IS that reflect their multiple etiologies. However, scant attention has been paid to this issue lately. Adequate recognition of psychoses in epilepsy is essential for physicians caring for the affected patients, because the impact on their quality of life is too devastating to be overlooked. Investigations of the associations between epilepsy and psychoses were initially conducted by French and German neuropsychiatrists in the 19 century, while much of the literature came from England in the second half of the last century. It is widely recognized that the epoch-making reports by Slater, Beard and Glithero in 1963 raised important clinical questions regarding so-called “schizophrenialike” psychoses of epilepsy. Although some of those questions have been answered, many remain controversial.
