The issues of change, quality of care, morale and opportunities involved in being a nurse today in a system undergoing this magnitude of change is presented within the framework of the quality of care initiatives that have been launched by VA. Fifty-seven spouses participated in the study, 47 were female and 10 were male. Data were collected by structured questionnaires, which were distributed to the spouses from 2 weeks–4 months after the patient’s AMI. The researchers found that age, negative life event during the last 12 months, time since infarction, and the spouse’s fears influenced the choice of strategies. In the early stage of the crisis, positive reappraisal seems to be a strategy that increased physical strain. Spouses’ need for social support in the early stage of the crisis is a factor for health care providers to bear in mind. This model tends to focus on structure components (e.g., presence of sprinkler systems, number of nursing staff) and process components (e.g., proper charting of medications), rather than outcomes (e.g., residents who remain continent; absence of pressure ulcers). A common strategy to evaluate quality was gathering data through retrospective chart reviews or observations. Often, these data did not translate into useful information, which otherwise could have been used to improve the quality of care and life for nursing home residents. In the more recent past, nursing facilities have been moving away from the QA model to a quality improvement model. This latter model has been a focus of Minnesota nursing facilities due, in part, to a demonstration project initiated in Minnesota by the Department of Human Services (DHS). Its clinical presentation is variable and can mimic any number of medical and surgical conditions. The most prevalent symptom is excruciating chest pain. Essential treatment includes rapid initiation of pharmacologic agents to control hypertension. Aortic dissection involving the ascending aorta also necessitates immediate surgical repair. Surgical treatment of aortic dissection involving only the descending aorta generally is reserved for patients with persistent pain, intractable hypertension, or evidence of propagation of the dissection.
