All participants in the therapeutic and diagnostic process must have a high level of motivation to achieve the maximum level of effective interaction deserving specific attention. In this aspect, it is necessary to study the factors that influence on the motivation in workers in the Healthcare system. \u0000The aim of the work: based on the data of Russian and foreign literature on motivation in the Healthcare system to evaluate this indicator as the index for increasing the effectiveness of interaction between participants in the therapeutic and diagnostic process. \u0000The presented article includes the sources of literature (Russian and foreign) presented in the Scopus, Web of Science, MedLine, The Cochrane Library, EMBASE, Global Health, CyberLeninka, RSCI, etc. databases, which demonstrate the main issues concerning motivation in the healthcare system. \u0000There are considered issues in the healthcare system, related to the motivation and some ways of solving them are proposed. \u0000Conclusion. Patient satisfaction is one of the most important indicators of the quality of the health care noted in the priority objectives of state policy this area. \u0000The aim of the study was to assess satisfaction with the quality of medical care provided by a general practitioner (GP), using the example of the activities of the Stupino Regional Clinical Hospital of the Moscow Region. \u0000Materials and methods. The international EUROPEP questionnaire was used. Statistical processing of the study materials was performed using Statistica 10 and SAS JMP 11. \u0000Results. Respondents highly evaluated the provision of medical care 17,87 ± 3,05 (17,04; 18,70, p = 0,05). High scores were obtained for the block «accessibility» of medical services, amounting to 21.76 ± 2.53 \u0000(21.07; 22.45, p = 0.05). Strong positive relationships «patient-GP» interactions (r = 0.9; p < 0.0001), joint decision making, and establishing trusting relationships (r = 0.73; p < 0.0001) were found. The level of trust relationship between patients and GPs was 18.94 ± 4.37 (17.75; 20.14, p = 0.05) and rated high. Problem areas were identified for individual issues in the management of medical care and recommendations for their adjustment were developed. \u0000Limitations. The research is limited due to a small sample due to the testing of the EUROPEP questionnaire and its adaptation for the Russian health care system. \u0000Conclusion. On the example of EUROPEP questionnaire on the base of the the Stupino regional clinical hospital of Moscow region there were obtained high estimates of satisfaction with the quality of medical care, provided by GPs/FDs. The revealed problems in achieving the expected medical results refer to the making of joint decisions and are limited by the interpretation of confidentiality in GP-patient interaction. Thermal lesions represent a significant medical and social problem, they are characterized by the duration and high cost of treatment, a significant proportion of the limitation or disability. In Siberian region besides burn injuries, cold injury is common. \u0000The purpose of the work was to identify factors influencing the indicators of specialized medical care for patients with thermal injuries in the Siberian region. \u0000Materials and methods. The analysis of the reports of the main specialists of the subjects of the Siberian Federal District on the provision of specialized medical care to victims of thermal injuries in 2017 was carried out. Statistical processing was performed using the Microsoft Excel for Windows 10.0 program. \u0000Results. The average provision for victims of thermal trauma is 2.8 beds per 100,000 of the district’s population, the turnover rate per 100,000 of the population is 134, the mortality rate is 4.6%. The hospitalization rate for patients with burns per 100,000 populations is 39, with cold injury — 6.0. In the structure of adult patients, non-core patients hospitalized for a “burn” bed occupy 12%. The mortality rate in adults is 10 times higher than in children. For each specialized bed for a patient with thermal injuries, 6.4 beds intensive critical care are required. Over 12% of victims in intensive care units require mechanical ventilation. The rate of anesthetic management per patient varies in subjects by 6 times. \u0000Limitations. The study was conducted on the basis of available official statistical data, primary data were not used. \u0000Conclusion. Cold trauma has a significant impact on the performance of the medical care system in the Siberian Federal District for patients with the “combustiology” profile. The variability of a number of key parameters in the provision of specialized care for thermal trauma is due to different numbers and densities of the population, the degree of industrialization and the financial capabilities of the region. We examined associations between registered, counted and uncounted disability and integral measures of children with mental disorders in 0–14, 15–17 and 0–17years groups. In addition, health resource and activity measures, mental disorder morbidity rates, demographic indicators and their association with child disability rates were examined. Period of analysis was from 2010 to 2020. The study is analytic, non-personalized, retrospective, not randomised. \u0000Results. In the Arkhangelsk region, in the 0–14 years group the rate of calculated total disability was on average 55.9% higher than the rate of registered disability. The underreporting of total disability was higher than in the 15–17 years group. We found no significant links between demographic, socio-economic, health resource indicators and the level of official childhood disability. \u0000Limitations of the study. To avoid errors of extrapolation, caution should be exercised when attempting to scale the results obtained in the study to the population of other regions or the country as a whole, other types of disability, separate age and gender groups, and individual cases of disability. \u0000Conclusions. In the Arkhangelsk region there is underreporting of childhood disability caused by mental disorders. The levels of children’s disability due to mental disorders recorded by official statistics have no significant relationship with indicators reflecting the demographic, socio-economic situation, resources and health care activities. The proportion of citizens leading a healthy lifestyle has been declining since 2019, despite the measures currently implemented. To improve the situation, it is necessary to develop specific effective measures to motivate and encourage citizens including children to maintain a healthy lifestyle. \u0000The purpose of the study was to investigate the components of the health-related aspect of the schoolchildren’s lifestyle during the study at school to develop proposals for improving the management of the formation of a healthy lifestyle for schoolchildren, in particular, motivation for a healthy lifestyle. \u0000Material and methods. The study was conducted on the base of a municipal educational institution — secondary school No. 8 with in-depth study of individual subjects of the urban district of Zhukovsky, Moscow Region. A total of six hundred fifty completed depersonalized (anonymous) questionnaires were collected. \u0000Results. Smoking and using nicotine-containing products by the majority of the family has been confirmed to have a negative impact on smoking and using such products by schoolchildren, which proves the obligation to take into account the family aspect in the development and implementation of health measures aimed at combating smoking and usage of nicotine-containing products. The most common violations of the principles of a healthy lifestyle among the respondents, the most popular channels for obtaining information about a healthy lifestyle and the methods preferred by respondents to encourage healthy lifestyles in the three study groups were identified. \u0000Limitations. When studying the components of the health-related aspect of the lifestyle of students of secondary school No. 8 with in-depth study of individual subjects of the urban district of Zhukovsky, Moscow Region, 650 questionnaires were evaluated, which is a sufficient reference sample. \u0000Conclusion. Rykov","doi":"10.47470/0044-197x-2023-67-3-259-266","DOIUrl":"https://doi.org/10.47470/0044-197x-2023-67-3-259-266","url":null,"abstract":"“Personalized” medicine is based on the belief that each person has unique molecular, physiological, environmental and behavioral characteristics, and in case of development of the disease, each patient should be treated taking into account these unique characteristics. This belief was to some extent confirmed by the use of the latest technologies, such as DNA sequencing, proteomics, imaging protocols and the use of wireless devices for health monitoring, which revealed large inter-individual differences. \u0000The purpose: to substantiate a personalized approach will be used in the medicine of the future. \u0000Literary sources (scientific articles) were searched, including those published in peer-reviewed journals indexed in Pubmed, WoS, Scopus, Global Health, CyberLeninka, and the Russian Science Citation Index. Seventy two articles devoted to personalized medicine were analyzed, of which 51 are included in this review. The review included articles on personalized medicine, published mainly over the past 10 years on the topics of oncology, genetics, and articles considering the legal, demographic and social aspects of this problem. From the analysis were excluded articles devoted to the philosophical aspects of personalized medicine, the use of computer and wireless technologies for data processing and communication. Historical precedents, some legislative and policy aspects of its implementation in practice, new technologies that make it possible, some new experiences including successes and failures, ways of testing and using individualized drugs, and future directions, including potential ways of treating people with problems concerning fertility and infertility are considered. The existing limitations of “personalized” medicine are presented. Since aspects of “personalized” medicine are rooted in biological realities, individualization of medical practice in certain cases is probably inevitable. \u0000Conclusion. With the development of technology and the accumulation of experience, an individual approach to the patient becomes more effective and cost-effective. Acquaintance with the existing foreign and domestic practice of developing long-term forecasts can be useful not only for those involved in their development, but also for decision makers in the healthcare sector. \u0000The purpose is generalization and systematization of published long-term forecasts in healthcare. Systematic search for publications according to the developed protocol. The protocol included databases of peer-reviewed publications, search terms, and search restrictions. The search for publications was carried out in the databases including Embase, Medline, Scopus, RSCI, etc. The search query was carried out using a combination of terms “free text” and “medical subject headings” related to the definition of long-term forecasts in healthcare. The main search restrictions were the language of publications, the date of publication and the forecast period covered. Conducting a comprehensive analysis, generalization and systematization of the information received on published foreign and domestic long-term forecasts in the field of healthcare, covering forecasts for the world as a whole, its regions and the Russian Federation. The long-term forecasts are most widespread in certain areas of healthcare development. Comprehensive forecasts covering several aspects of system development are less common. Quantitative forecasting methods are used more often, qualitative methods are mainly used in forecasting the technological development of the sector. \u0000Conclusion. Data were obtained on the difference in the quantitative and specific composition of intestinal microbiota bacteria in practically healthy individuals with subclinical invasions by pathogenic intestinal protozoa Lamblia intestinalis, Blastocystis hominis. Сhanges in bacterial composition were shown to have characterological features and species-specific «targets» of intestinal microbiota destruction. \u0000Material and methods. Bacteriological and parasitological research methods were used. For a comparative analysis of quantitative and specific changes in the symbiotic microflora, there were formed two groups: “Yes” and “No” for each type of pathogenic protozoa, bacteriological indicators were ranked by quantitative content — CFU “0–103”, CFU “103–10max”, CFU “10max”. Statistical differences in research results were determined by the Chi-square calculation method. \u0000Results. Antagonistic activity of unicellular protozoa in relation to the symbiotic bacterial intestinal microflora in the examined population groups has a different degree of severity depending on the type of invasion. Parasitic pathogens were shown to inhibit up to 40% of the quantitative and 50% of the species composition of the intestinal microflora and are an aggressive biogenic factor in the destruction of biota. There are no limitations of the study. \u0000Limitations. The studies had no methodological or administrative restrictions. \u0000Conclusion. For the first time, significant statistical material has confirmed that asymptomatic parasitosis is a strong biogenic determinant of the development of destructive changes in the integral structure of the symbiotic microflora and the formation of a subclinical form of dysbiosis in an “almost healthy contingent” of the population. Currently, the medical and social significance of the metabolic syndrome is related o the high specific weight among the causes of disability and mortality, which, along with this, is the leading risk factor for cardiovascular, cerebrovascular, diseases, diabetes mellitus and their complications. The high prevalence of metabolic syndrome in the world ranges from 44.9% in Japan to 50.9% in Spain. Due to the continued increase in the proportion of the elderly and senile population, a further increase in the incidence of metabolic syndrome is predicted, and a pronounced gain in health care costs for outpatient and inpatient medical care. \u0000The purpose of the study is to analyze the prevalence of the metabolic syndrome and its components according to the data of domestic and foreign reports. \u0000Russian and foreign scientific reports, monographs, and collections of articles for 2003–2019 were retrospectively studied using computer search engines and peer-reviewed journals in international databases (Scopus, Web of Sciences). Tregubov","doi":"10.47470/0044-197x-2023-67-2-156-162","DOIUrl":"https://doi.org/10.47470/0044-197x-2023-67-2-156-162","url":null,"abstract":"Following the special attention of foreign colleagues to appoint doctors as hospital managers, the issue \u0000of appointing specialists without medical education to the management of medical institutions in Russia is increasingly being looked at and discussed. \u0000The purpose of the study is to analyze historical and modern literary sources that highlight the experience of domestic and foreign healthcare by appointing specialists without medical education to the head positions of treatment and preventive medical institutions. \u0000The analysis of literary sources from 1940 to 2021 was carried out in electronic scientific libraries: Scopus, Web of Science, MedLine, eLibrary, CyberLeninka, Central scientific medical library and the Russian State Library by keywords: manager, physician, physician leadership, hospital CEO \u0000The article presents historical data on the formation in Russia since the beginning of the 18th century of the first medical institutions for the needs of the army and navy and the appointment of doctors to senior positions in them. From 1755 to 1908, the management of military hospitals was temporarily transferred to specialists without medical education, which led to numerous conflicts and a decrease in the efficiency of hospitals. Currently, in a number of foreign countries, in order to improve the quality of medical care, the issue of transferring managerial functions to doctors in hospitals is being actively discussed. \u0000The main reasons for the discussion about the expediency of appointing specialists to senior positions in medical institutions without medical education are: shorter terms of their training; health care activities in market conditions; the need for modern managers to possess leadership, managerial, economic and legal competencies. \u0000Conclusion. The work experience of domestic and foreign health care indicates the expediency of appointing doctors with leadership qualities and special training in the field of healthcare institutions and public health to senior positions in medical institutions.
