The outcomes of COVID-19 epidemic crisis on the elderly: A review study 新冠肺炎疫情危机对老年人的影响综述

Mortality rates are significantly higher in the elderly than in others during the epidemic of COVID-19; age is significant vulnerabilities to COVID-19. Analysis of clinical features according to the latest published data allows better understanding and better clinical judgment. Methods: In this review study, using the keywords, COVID-19, severe acute respiratory syndrome coronavirus (SARS-CoV-2), coronavirus, viral and elderly pneumonia, pandemic, epidemic, quarantine, mental health, and social isolation in articles indexed in Embase, Proquest, Pub med, Google Scholar, Science direct, Scopus, Medline, Cochrane databases from December 2019 to February 2020, were searched. Findings: The elderly of more than 65-year-old are classified in high-risk groups, are more vulnerable than other age groups, and require more attention. Social isolation, social distance, and lockdown has raised concerns about their mental health to prevent the spread of COVID-19. Conclusion: Due to the increasing population of the elderly, such biological disasters, if do not take care of and pay enough attention to the various dimensions and problems caused by its consequences, would affect acute and long-term consequences on community health and general health of the elderly. © 2021 Isfahan University of Medical Sciences(IUMS). Clear understanding of the nature of the diagnostic tests for severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2) and their challenges, collecting the most diagnostically valuable specimen at the right time from the right anatomic site, and interpretation of their findings is important. This review scrutinizes current challenges and interpretation of reverse transcriptase polymerase chain reaction (RT-PCR), as the reference method, loop-mediated isothermal amplification (LAMP), antibody and antigen detection, typical lung imaging characteristics and prominent abnormal changes in laboratory findings of patients with proven COVID-19, and describes how the results may vary over time. Bronchoalveolar lavage fluid and sputum specimens demonstrate the highest positive rates (93% and 72%, respectively) in molecular diagnosis of COVID-19. Alternatively, repeated RT-PCR assays can be performed;as over time, it is an increase in the likelihood of the SARS-CoV-2 being present in the nasopharynx. Combining clinical evidence with results of chest computed tomography (CT) and RT-PCR can minimize the risk of diagnostic errors. Elevated levels of interleukin 6 (IL-6) and D-dimer are thought to be closely associated with the occurrence of severe COVID-19 in adults, and their combined detection can serve as early factors predicting the severity of COVID-19. Moreover, elevated acute phase proteins are associated with a poor outcome in COVID-19. Serological diagnosis also is an important tool to understand the extent of COVID-19 in the community, and to identify individuals, who are immune. Clinical manifestations in patients are very diverse, and can range from asymptomatic disease to severe illness with death. The main purpose of this study was to compare the outcomes of patients with Covid-19 with early pulmonary, gastrointestinal, and neurological symptoms in patients admitted to hospitals in Qazvin City, Iran. Methods: This was a retrospective study with 300 samples, performed after obtaining ethics license and obtaining patient consent. The samples were selected using purposive method. Confirmation of COVID-19 was done due to having a lung scan confirmed by an internal medicine or infectious disease specialist, as well as having a positive polymerase chain reaction (PCR) test. Exclusion criteria also included pregnancy. Findings: The group of patients presented with early pulmonary symptoms were significantly different from the other two groups in terms of mortality, intensive care unit, intubation and hospitalization (P < 0.050). Conclusion: Patients presenting with early pulmonary symptoms should be given priority in hospitalization. © 2021 Isfahan University of Medical Sciences(IUMS). Methods: A total of 228 patients with COVID-19 were included in this cross-sectional study. Of these, 114 patients were alive and 114 patients were dead, who were selected using the available sampling method. The logistic regression analysis method (Inter model) was used to predict the effective factors of severity of disease and mortality of patients and control of confounders. Findings: Multivariate logistic regression analysis showed that increase in C-reactive protein (CRP), blood urea nitrogen (BUN), and potassium (K), and decrease in the mean time from onset of symptoms to hospitalization could be predictors of death in patients with COVID-19. Moreover, the deceased patients were older than the surviving group. There was also a significant difference between the two groups of deceased and survivors in terms of the presence of underlying diseases of hypothyroidism and chronic kidney insufficiency (P < 0.001). Conclusion: According to the results of this study, measuring electrolytes at the beginning of hospitalization, and then serially, is recommended to take corrective measures in the field of disease control. © 2021 Isfahan University of Medical Sciences(IUMS). One of the threats in this process is cheating;cheating is a major concern for professors and institutions, especially when universities conduct exams online due to the Covid-19 pandemic. It means to consider the academic work of others in its place. Cheating can have a devastating effect on the performance, reputation, and longevity of an institution. When academic institutions do not have control over cheating, the result will be employers' dissatisfaction with the graduates' job performance. The first line of defense to prevent cheating is the adoption of ethical codes. In this regard, it is necessary to hold empowerment courses for teachers to train them to design appropriate questions. Other factors include setting a time limit for each question, designing questions with different arrangements, random distribution of questions, holding several midterm exams and reducing the volume of end-of-semester test resources, changing the scoring system to qualitative, matching the taught content with the exams, facilitate vague and complex lessons, project assessments, and reduce exams. The use of web-based cameras (webcams) as a technology-based solution to prevent cheating has also been suggested. Artificial intelligence and image processing, including special software, can also be used to monitor the student's body and face movements, and report suspicious behavior to the teacher. The use of Lockdown Browser also restricts the student's ability to search for resources during the test. © 2021 Isfahan University of Medical Sciences(IUMS). Afsahi","doi":"10.22122/jims.v39i621.13894","DOIUrl":"https://doi.org/10.22122/jims.v39i621.13894","url":null,"abstract":"Background: Fear and stress caused by COVID-19 increases the feeling of insecurity in children and adolescents, and if not taken timely action, will cause serious mental health problems in them. The aim of this study was to determine the mental health status of girls aged 12-17 years in Aran-Bidgol City, Iran, during the COVID-19 pandemic. Methods: This descriptive cross-sectional study was performed on 390 girl students of Aran-Bidgol in 2020-21. Data collection tools were demographic/family information checklist and of child health questionnaire (CHQ), spiritual health, social support, and measuring the long-term effects of the accident on children's mental health, which were completed electronically. Data were analyzed using SPSS software with independent t, ANOVA, Pearson correlation, and multiple linear regression tests with a significance level of 0.05. Finding: 65.10% of girls received a score of 25-50 from the Mental Health Questionnaire, which indicated a mild mental health problem. 34.40% got a score of 50-75 and none of them got a score above 75. The regression model showed that two patterns of family communication (dialogue and compliance) and girls' existential health predicted the girls' mental health score (P < 0.05). Conclusion: The results showed that the patterns of family communication and existential health in adolescents had effect on adolescents' mental health;so it is suggested to identify the best regulatory model and family function with an emphasis on existential health, to improve their mental health by gaining the ability to positive valuation. © 2021 Isfahan University of Medical Sciences(IUMS). Today, the goal of health providers and policymakers around the world is to prevent and control the severity of disease outcomes by making and using vaccines. The present study is a letter to the editor based on the authors' experiences and other studies around the world. It is possible to increase the acceptance of the COVID-19 vaccine in the community with solutions such as sensitization through education and information about vaccines, encouraging the production of COVID-19 vaccines for factories producing, creating exponential demand for vaccines, secure access to vaccines, approving preconditions for receiving vaccines to register and enter Government centers and the employment of manpower, and the provision of educational and health facilities.
