Albayrak","doi":"10.32677/EJMS.2019.V04.I02.010","DOIUrl":"https://doi.org/10.32677/EJMS.2019.V04.I02.010","url":null,"abstract":"Wilkie’s syndrome is a rare acquired disease that occurs when acute dilatation occurs between the superior mesenteric artery and the aorta, compressing the duodenum between these two arteries. A loss of fat pads around the SMA due to reasons such as anorexia nervosa, weight loss, burns, trauma or prolonged bed rest is thought to be involved in the etiology of the disease. While the disease has an acute and chronic form, the chronic form is more common. Initially, conservative medical therapy is recommended for the treatment of Wilkie syndrome. In cases where conservative treatment fails, surgical treatment is indicated. In cases of extreme dilation of the stomach and duodenum being observed in the CT of patients presenting with complaints such as abdominal pain, nausea, and vomiting, a differential diagnosis of Wilkie’s syndrome must be considered. A 22-year-old female patient was presented to our hospital with the complaints of epigastric pain, abdominal distention and nausea continuing for three days. On general examination, it was determined that her abdomen was distended and her epigastric region was sensitive. {"title":"COMPARATIVE STUDY BETWEEN INTRAVENOUS RAMOSETRON AND DEXAMETHASONE AS PRE-TREATMENT TO ATTENUATE PAIN DURING INTRAVENOUS PROPOFOL INJECTION","authors":"S. Objective: The aim of this study was to compare the efficacy of ramosetron and dexamethasone as a pretreatment drug for attenuation of pain due to propofol injection. Methods: This randomized comparative study was conducted at a tertiary care hospital in Eastern India, over a period of one year (March 2016-February 2017). Total 90 American Society of Anesthesiologists (ASA) grade I and II patients were randomly assigned into three groups (30 in each). Group R received 2 ml (0.3mg) of ramosetron, Group D received 2 ml (0.1mg/kg) of dexamethasone, and Group N received 2 ml of 0.9% normal saline. Venous drainage was occluded by using a tourniquet in mid-forearm before injecting the pre-treatment solution and released after 1 min and then 2 ml of propofol was injected over 5 sec. Patients were observed and questioned 15 sec later, if they had pain in the arm or discomfort during drug injection. The pain was scored on a four-point scale: 0 = no pain, 1 = mild pain, 2 = moderate pain, and 3 = severe pain. Pearson’s Chi Square test/Fisher's Exact Test and Mann-Whitney U test/Kruskal Wallis Test were used to analyze the results. Results: The overall incidence of pain was 90% in the saline group, 17% in the ramosetron group and 10% in the dexamethasone group. The intensity of pain was significantly less in patients receiving ramosetron and dexamethasone than those receiving saline (P < 0.05). Haemodynamic parameters like heart rate (HR) and mean blood pressure (MBP) were recorded at different points of time. No significant inter group differences in MBP and HR were noted at any point of time. To provide postoperative analgesia after arthroscopic knee surgery, various medications have been administered intra-articularly. Aim: To compare the postoperative analgesic effects of intra-articular clonidine and dexmedetomidine administered as adjuvants with local anesthetic levobupivacaine in patients undergoing arthroscopic knee surgery. Methods: Sixty patients undergoing elective knee arthroscopy were randomly assigned to one of the following groups containing 20 patients each. Group L patients received 19 ml of 0.25% levobupivacaine and 1 ml of isotonic saline (20 ml in total) intra-articularly. Group C patients received 150µg [1ml] of clonidine added to 19 ml of 0.25% levobupivacaine intra-articularly (total volume 20 ml). Group D patients received 100 µg (1 ml) of dexmedetomidine added to 19 ml of 0.25% levobupivacaine intra-articularly (total volume 20 ml). Analgesic effect was evaluated by measuring pain intensity (VAS score) and duration of analgesia. Results: A longer delay was observed between intra-articular injection of study medication and first requirement of supplementary analgesic in group C (10.44 ± 4.6 hours) and in group D (10.72 ± 5.6 hours) compared to group L (6.18 ± 1.8 hours). Total consumption of diclofenac sodium in first 24 hours in postoperative period was significantly less in group C and D. No significant side effects were noted. Collagen synthesis is an essential feature of anastomotic healing. Objective: Our study aimed to investigate individual and combined effects of high protein diet and stanozolol on the intestinal anastomosis in rats. Methods: Total of 48 Wistar-albino rats were divided into four groups. Group C: Control group; Group P: high protein diet (4mg/day) administered by gavage; Group A: group to which stanozolol was administered intramuscularly (i.m) (2mg/kg/day); Group A+P: group to which both stanozolol (2mg/kg/day) i.m and high protein diet (4mg/day by gavage) were administered. In each group, half of the animals underwent surgery on the 3 rd postoperative (PO) day and the other half on the 7 th PO day for analysis procedures. Bursting pressures of each anastomotic segment, tissue hydroxyproline levels, and histologic tissue repair process were evaluated. Results: On 7 th PO day, group A+P had statistically and significantly higher bursting pressures than the control group. Hydroxyproline levels at the anastomotic segment on both 3 rd and 7 th PO days were also significantly higher in group A+P. Conclusion: A high protein diet and stanozolol do not significantly impact the anastomotic healing when used separately. Discolorations and opacities in the maxillary incisors provide the grounds for varied treatment modalities, for managing MIH. Microabrasion and composite resin-based restoration can be considered as the treatment options in moderate to severely affected teeth. Here we present a case report of 12-year-old female patient, who was presented with moderately affected hypomineralized incisor teeth. About 80–85% of lung cancers are non-small cell lung cancer (NSCLC). The development of therapeutic agents targeting products of epidermal growth factor receptor (EGFR) gene mutation and anaplastic lymphoma kinase (ALK) rearrangements has significantly improved survival in patients with NSCLC. Thus, the patients eligible for the treatment should be selected through appropriate molecular tests. Objective: The main objective was to study the distribution of NSCLC and its genetic mutations, in the patients of Eastern India. Methodology: A prospective study was carried out among 228 patients, in a tertiary care hospital of Eastern India during a period of 2 years from January 2016 to January 2018. We have included all the patients who were screened and found to have lug carcinoma. The detailed clinical history of the patients was recorded. We have used techniques such as radiology, histopathology, immunohistochemistry (IHC) and molecular study by FISH technique. Results: Out of 138 cases of NSCLC on histopathology 78 cases (56.7%) were reported as squamous cell carcinoma, 46 cases (33.3%) as adenocarcinoma and 14 cases (10.1%) as NSCLC. IHC was used for categorization of NSCLC cases which showed adenocarcinoma in 4 patients and squamous cell carcinoma in 10 patients. EGFR exon 19 deletion mutation was the predominant mutation in adenocarcinoma.
