The main feature of the toolbox is its fully multivariate implementation, that is made possible by the design of an approach for the non-uniform embedding (NUE) of the observed time series. The toolbox is equipped with parametric (linear) and non-parametric (based on binning or nearest neighbors) entropy estimators. All these estimators, implemented using the NUE approach in comparison with the classical approach based on uniform embedding, are tested on RR interval, systolic pressure and respiration variability series measured from healthy subjects during head-up tilt. Approaches were applied to simulations and to HP variability series recorded during complete vagal blockade obtained via administration of a high dose of atropine (AT). Differences between the estimated complexity indexes derived from both simulations and real data indicated that permutation entropy has a larger sensitivity to noise than coarse-grained entropy. By decomposing the separate coupling components in a phase oscillator model, we infer the cardiorespiratory coupling functions with dynamical Bayesian inference for time-evolving and noisy coupled systems. The inferred coupling functions are themselves time-varying processes. On a longer timescale, application to ageing shows that the coupling functions and the direct influence of respiration decrease as age increases. One hundred healthy subjects were divided into five groups (n=20 for each), according to age: 21-30; 31-40; 41-50; 51-60; 61-70. The RR and SAP series were recorded for 15 minutes at REST and STAND. Short sequences of RR and SAP were analyzed by conditional entropy. At REST, RR complexity showed reduction with age in 61-70, and SAP complexity decreased, starting from 31-40. Polysomnographies were performed in 10 patients with severe obstructive sleep apnea syndrome. Both obstructive (OS) and mixed (MX) apneas were found in all patients. The FD of RRI interpolated series was estimated “locally” over a running window of 15 s, overlapped by 14 s, by an algorithm based on the Mandelbrot's definition of the FD of geometric curves. The FD estimator appeared able to follow rapid changes in RRI dynamics. Moreover, it was significantly higher in OS than in MX apneas for 9 out of 10 patients (p<;0.05). Despite significant progress in diabetes care, these abnormalities are still considered the result of anatomical neural damage. Recent research from our group showed that what had been called “neuropathy” is also in part a functional abnormality. This “dysfunction” can be induced by several factors, including tissue hypoxia, hence strictly connected to respiratory abnormalities, but potentially reversible. Standard procedures, in fact, rely only on subjective scores from interviews and questionnaires. Here we show that monovariate and multivariate multiscale entropy measures are able to characterize the dimension regulating the behavior towards something pleasant in subjects. Such a psychological dimension was quantified through the BIS/BAS scores and investigated with a complexity analysis of Heart Rate Variability and respiratory activity. Current approaches associate these measures to finite single values within an observation window, thus not being able to characterize instantaneous system dynamics. In this study, we introduce the definition of novel measures of entropy based on the inhomogeneous point-process theory and inspired by the approximate and sample entropy algorithms. The discrete heartbeat series are modeled through probability density functions defined at each moment in time, which characterize and predict the next beat occurrence as a function of the past history through Laguerre expansions of the Wiener-Volterra terms. In here, a new parameter called V-index, able to measure the standard deviation of myocites' repolarization times, was evaluated after moxifloxacin administration. 24-h Holter ECGs of 68 healthy subjects enrolled for a thorough QT study were retrospectively analyzed. The placebo and active-comparator (moxifloxacin) arms were considered. The standard QT interval (Fridericia's correction) was included as well, for a direct comparison. V-index and QT increased along with the drug's serum concentration and were statistically different from values in the placebo arm (p<;0.05). Peak values were reached 5 hours after administration (QTc: 428.77 ± 25.17 ms at 5h vs 418.08 ± 23.37 ms at 0h; V-index: 31.67 ± 11.01 ms at 5h vs 27.72 ± 7.59 at 0h) and both were statistically different from predose (p<;0.05). Heart rate variability (HRV) analysis allows identification of CAN but is sensitive to the length of recording and the presence of artifacts and ectopics, requiring preprocessing and consideration of length of recording. RR intervals from 10-second and 5-minute ECG recordings from patients with no CAN, early CAN and definite CAN were preprocessed using adaptive filtering with the controlling parameter c set at 0.2, 0.5 and 0.8, and time and frequency domain HRV analysis applied. Early CAN and definite CAN required different setting of c with respect to the length of recording. The 5-minute recording with c=0.2 provided the best results using RMSSD for normal versus eCAN (p=0.0007) and for eCAN versus dCAN (0.019).
