Introduction Hydatidosis is a severe zoonosis, human contamination occurring by the ingestion of foods contaminated by dog faeces containing Echinococcus granulosus (E. granulosus) eggs. Peritoneal hydatidosis is one of the serious complications of the disease, often associated with another visceral location, it affects all ages and all segments of the peritoneal cavity. Materials and methods: We present the case of a 34 years old male, admitted in Saint Spiridon Emergengy Hospital, Surgery Clinic I, Iași, România with severe disseminated hydatidosis. Result: Our patient presented diffuse pain in the superior abdomen, without any other complains. The biochemical tests were normal, except the eosinophils. The ultrasound and the computer tomography revealed the presence of multiple cystic tumors in the liver, peritoneum and in the spleen, with imagistic appearance specific for hydatid cyst. A surgery team was formed to extract the multiple cysts, by laparotomy. The histopathological examination confirmed the diagnosis of multiple hydatid cysts. Conclusion: Peritoneal hydatidosis is a rare but serious complication of hydatid disease. The positive diagnosis is based on epidemiological, clinical, and paraclinical arguments represented essentially by CT scan. Early diagnosis and treatment of primary sites as well as optimal surgical management of peritoneal hydatidosis determine the prognosis.","PeriodicalId":385372,"journal":{"name":"Jurnalul de Chirurgie","volume":"77 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-03-31","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"124419082","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Introduction Molluscum pendulum is a benign fibrous tumor of the skin, also known as 'acrochordons’ usually small in size. Clinically it presents as a small, soft, pedunculated growth flesh-colored but the giant molluscum pendulum is a rare finding. Materials and methods: We present you a case of a 48-year-old men, with no significant personal pathological history, that addresses the County Emergency Clinical Hospital „Saint Spiridon’’ Iasi, for acute hemorrhage from a tumor at the level of the left thigh. The clinical examination revealed a pedunculated tumor, with macroscopic aspect of Molluscum pendulum, with subcutaneous hematoma. Result: Our patient presented a pedunculated tumor on the left thigh, with recent signs of hemorrhage and a subcutaneous hematoma, without any other complains. The biochemical tests revealed inflammatory syndrome and anemia. Surgical intervention was performed with de resection of formation. The histopathological examination confirmed the diagnosis of cutaneous fibroepithelial polyp. Conclusion: Early diagnosis and treatment of primary sites of cutaneous lesions avoids complications and the need of ample surgery.","PeriodicalId":385372,"journal":{"name":"Jurnalul de Chirurgie","volume":"7 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-03-31","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"128867574","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

BACKGROUND: Gossypiboma (retained surgical sponge) is a rare but a severe surgical case. It can cause variable and often serious complications requiring repeated and risky surgical interventions in order to remove them, and may lead to high morbidity and mortality. CASE PRESENTATION: We present a case of a gauze pad unintentionally left in the abdominal cavity of a 61-year-old patient after a sigmoidectomy with a Hartmann’s procedure. Although the textile sponge was recognized and removed during the same hospitalization, the patient was subsequently diagnosed with a large abdominal fat-containing mass, requiring repeated surgery. Microscopic examination revealed fibrous-adipose and granulation tissue, and thread granulomas of a foreign body type. CONCLUSIONS: Even relatively short-term retention of a gauze object in the abdominal cavity associated by unusual host response may result in the growth of a large abdominal granuloma, which has all radiological signs of a tumor. To prevent the incidence of gossypiboma, it is necessary to strictly follow the rules of the operating room and apply all the available measures","PeriodicalId":385372,"journal":{"name":"Jurnalul de Chirurgie","volume":"16 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-03-31","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"116825565","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

{"title":"Transvaginal intestinal evisceration: clinical case","authors":"Valentin Bernic","doi":"10.7438/jsurg.2023.01.11","DOIUrl":"https://doi.org/10.7438/jsurg.2023.01.11","url":null,"abstract":": Small bowel evisceration through the vagina is a rare surgical emergency that requires urgent surgical intervention because of the risk of developing acute small bowel ischemia. The clinical case is represented by an 81-year-old patient with vaginal intestinal evisceration, known in the past with 5 natural births, uterine cerclage in the last pregnancy, persistence of vaginal prolapse for 20 years. Gut evisceration was caused by the postmenopausal period, atrophic vagina caused by the enterocele susceptible to rupture from increased intra-abdominal pressure or trauma. From the pathogenic point of view, the lesion of an anatomical structure, as in the case reported by us of the posterior vaginal recess with the evisceration of the small intestine (ileum) in its evolution, initially achieves an inflammatory contact from the first stages. Objectives: to present the surgical emergency, the etiology, the possible complications and the method of choice for the treatment of the given pathology. Material and methods: were composed from the specialty literature, the case was elucidated by surgical intervention through a double approach: transabdomial and transvaginal. The multidisciplinary team was composed of general surgeons, gynecologist and anesthetist. Results: The results of the given case show the importance of rapid resolution of intestinal evisceration to avoid bowel ischemia and minimize the risk of subsequent recurrence. Conclusion: Early detection and surgical management of this rare surgical emergency is imperative to prevent small bowel ischemia that may require resection and the development of sepsis and systemic inflammatory response syndrome due to intestinal necrosis that may ultimately lead to death.","PeriodicalId":385372,"journal":{"name":"Jurnalul de Chirurgie","volume":"46 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-03-31","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"114328926","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

{"title":"A rare case of entire small bowel diverticulosis- a case report","authors":"Andreea Ludusanu","doi":"10.7438/jsurg.2023.01.13","DOIUrl":"https://doi.org/10.7438/jsurg.2023.01.13","url":null,"abstract":". Small bowel diverticulosis is a rare condition, with diverticula that can be classified as congenital or acquired, mostly of them being acquired pseudo diverticula, excepting the Meckel diverticula that is congenital. Their exact etiology has not been definitively identified and most of the patients with this condition are asymptomatic, but when they get complicated with occlusion or perforations, they become an emergency. The clinical case is represented by an 87- year-old patient with intestinal oclusion, caused by an intestinal volvulus on adherential brida, with a personal history of hysterectomy 20 years ago, left brest mastectomy 15 years ago and a laparoscopic cholecystectomy 5 years ago. Although the patient had an abdomino-pelvic computer tomograph at admission in the emergency room, the small bowel diverticulosis was discovered intraoperatively. Objectives: to present the case particularity, the etiology, the possible complications and the method of choices for the treatment of this given pathology. Material and methods: the specialty literature was researched to see what is the standard procedure in this type of condition and what to do when we find these so-called incidentalomas. Results: The results of the given case shows that diverticulosis of the small intestine, a rare condition, can always appear in clinical practice concomitantly or not with other diseases so it’s important to recognize it and know what to do in this case . Conclusion: More knowledge of this rare condition would be very useful in clinical practice, from putting the diagnosis through radiological investigation methods to knowing the treatment options.","PeriodicalId":385372,"journal":{"name":"Jurnalul de Chirurgie","volume":"11 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-03-31","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"125251005","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
