Its presence in a pregnant woman with a complication such as peritonitis by ileal perforation is an extremely rare case. We report a clinical case concerning a 32-year-old woman, amenorrhoeic for 24 weeks, referred to the general and visceral surgery department of the Ignace Deen National Hospital for acute intestinal obstruction in pregnancy in whom, after performing an X-ray of the abdomen without preparation which revealed mixed hydro-aeric levels and a biological assessment, she was taken to the block, at the celiotomy purulent liquid flowed out and the exploration revealed a pudding of ileo-coeco-colic intussusception whose disinvagination revealed three ileal perforations with respective diameters of 1.5cm, 2cm and 3cm located respectively 10cm, 30cm and 50cm from the ileocecal junction with pre-perforation zones. We performed a resection removing the perforations followed by an end-to-side ascending ileocecal anastomosis. {"title":"Obstetric Fistulas: Epidemioclinical Aspects and Management at the Maroua Regional Hospital Summary","authors":"Ngaroua Ngaroua","doi":"10.47363/jsar/2023(4)161","DOIUrl":"https://doi.org/10.47363/jsar/2023(4)161","url":null,"abstract":"Obstetrical fistula is an abnormal communication between the vagina and the urinary tract or between the vagina and the rectum, or both, leading to urinary incontinence. The cameroonian government with its developmental partners have set up centers to deal with these fistula among which, the one of the Maroua Regional Hospital. After two years of active operation, it was judicious for us to evaluate the functioning of the said center. Hence, the interest of this study led to the general objective of the evaluation of the management of obstetric fistula at the Maroua Regional Hospital Center. We conducted a cross-sectional descriptive study with retrospective data collection based on 118 records of patients managed during the study period. Results obtained showed that, the age group of [20-25] years were mainly represented in the sample. The vesico-vaginal form was the most common fistula with 72.90%. The surgical approach by the lower route with fistulorraphy was practiced in 84.70% of the overall cases. The success rate was 96.62% against 3.38% failure at hospital discharge. Long-term follow-up gave us a cure rate of 77.23% against 22.77% cases of recurrence. Methods: A triple-blind clinical trial was conducted with 20 patients who underwent elective operations under general anesthesia, randomly allocated into two groups, CG (control: 0.9% saline) and CGS (s-ketamine: 0.2 mg.kg-1). Each group was assessed at M0 (immediately before pre-oxygenation), M1 (3 minutes after SR stabilization) and M2 (3 minutes after infusion of s-ketamine or 0.9% saline). Bispectral index (BIS), electromyography, and SR were recorded at each moment. Results: There were no statistically significant changes regarding BIS and EMG from M1 to M2. There was also no indication of a significant difference between the average SR values concerning the factors group and moment, jointly (p=0.499), and separately (G1 and G2, p=0.812 and p=0.923). Conclusions: Low doses of s-ketamine were insufficient to determine changes in SR values, validating this important electroencephalographic parameter. The external tissue is initially unaffected, therefore we rely on secondary signs of toxicity, which often represent an already established systemic infection. It typically effects immunocompromised patients, who are slow to manifest these secondary signs, complicating the diagnosis further. We present a case of an immunocompromised patient, who did not manifest typical signs of systemic infection. Atrial Fibrillation was the first clue to an underlying infection, and initially the only localising infective source was a hydrocele. Cutaneous features in keeping with a Fournier’s Gangrene occurred forty-eight hours later. {"title":"Influence of (S)-Ketamine on the Bispectral Index and Electroencephalogram during Total Intravenous Anesthesia: A Randomized Clinical Trial","authors":"R. Method: Forty participants, submitted to controlled venous effector site anesthesia, were randomly distributed into 5 groups. In the k0 control group, participants did not receive (S)-ketamine; k1, k2, k3, and k4 received (S)-ketamine 200 µg.kg-1 iv followed by a continuous infusion of 100, 200, 300, and 400 µg.kg-1 h-1, respectively. In each group, 11 moments were evaluated: M0, before anesthetic induction, M1, 10 minutes after anesthesia stabilization with BIS between 45 and 55, and M2 to M10, 3, 6, 9, 12, 15, 18, 21, 24, and 27 minutes after (S)-ketamine. At all moments, BIS and pEMG values were recorded. An ANOVA was used for repeated measures, and the p-value was adjusted for multiple comparisons by Tukey’s test, considering p< 0.05 as significant. Results: No significant changes (p> 0.05) were observed at each moment after M0 by comparing the mean BIS and pEMG values between all groups. In order to share our experience with complex traumatic popliteal vascular injuries at Al-Thawra Modern General Hospital in Sana’a City- Yemen, we focus on the incidence, initial presentation, surgical management, and outcomes of popliteal artery injuries. Patients and Methods: Over the 3-year period from 1 January 2020 to 31 December 2022, all patients with popliteal artery injuries regardless of their other synchronized injuries were included. By contrast, patients who are managed as popliteal artery injuries by a military care giver at battle field were excluded from our analysis. In the diagnosis of popliteal artery injury, we relied on the physical examination. The main outcome was either patients get complete recovery, partial function loss of the limb, or end with amputation. Results: During study period, an overall incidence of popliteal artery injuries was 0.9%. Of 26 patients with popliteal artery injuries, 25 (96.2%) patients were men, and more than half of our patients (53.8%) were in the age group ≤ 25 years, with an overall mean of age 24.5 ± 5.6 years. The distribution of patients in groups of time delays in treatment of ≤ 6 hours, 6-10 hours and more than 10 hours was 9 (34.6%), 12 (46.2%) and 5 (19.2%) respectively. The mechanisms of injury were penetrating and blunt in 24 (92.3%) and 2 (7.7%) patients respectively. Patients who received a fasciotomy was 25 (96.2%). In our cohort of patients, incidence of amputation was 11.5%, while partial function loss rate was 27%. Conclusion and Recommendations: In our cohort of patients, incidence of popliteal artery injuries was high. Despite technical improvements in management of popliteal artery injury, a high amputation rate is observed in our study. The need for early diagnosis and early referral to definitive care must be emphasized. The aim of this study is to contribute to the understanding of this disease, particularly in terms of diagnosis and optimization of surgical management. Although serious complications have been described, Amyand hernia generally has a good prognosis. However, practitioners must master the clinical and surgical peculiarities of Amyand hernia in order to ensure optimal and adequate treatment of the hernia while avoiding recurrence. Questions remain regarding its pathophysiology, clinical peculiarity, and treatment.
