The twin block appliance is widely considered as the most efficacious myofunctional appliance for treating skeletal class II malocclusion in growing patients. The success of treatment with any myofunctional appliance depends on a variety of factors, including the patient's age, growth status, growth pattern, the specific malocclusion being treated, patient compliance, the type of appliance, and other case selection criteria, such as clinical examination and certain cephalometric parameters that are not typically considered in routine practice. Among cephalometric parameters, the saddle angle is of utmost importance and should always be considered before commencing myofunctional therapy. Patients with a large saddle angle may have a class II skeletal relationship, characterized by a posteriorly positioned mandible. These cases can be more challenging to treat with functional appliances alone and may require additional orthodontic or surgical interventions. This paper presents a detailed description of three cases illustrating the importance of the saddle angle in the success of myofunctional therapy.","PeriodicalId":376004,"journal":{"name":"Journal of Contemporary Orthodontics","volume":"4 11","pages":""},"PeriodicalIF":0.0,"publicationDate":"2024-02-15","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"139774659","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Further, the study also aims to compare the post functional changes being observed between males and females. This is a prospective study on 15 Pre-adolescents (08 males and 07 females; mean age: 10.75 ± 0.91years) who underwent functional jaw orthopaedics with TBA. Lateral cephalogram at Pre treatment stage (T1) and post functional phase of TBA (T2) were obtained. The cephalometric parameters were measured and paired t test was done for comparison of pre and post functional changes. The mean period of functional phase of TBA therapy was 18 ± 02 months. The cephalometric changes post functional jaw orthopaedics were statistically insignificant when compared males Vs females (P > 0.05). However, mean increase of in the effective length of mandible and hypopharyngeal space (T2-T1) was 4.0± 1.16 mm and 1.47±0.57 mm respectively, which were statistically highly significant (P<0.5). 1. TBA therapy in skeletal Class II patients significantly increases the effective mandibular length and hypo-pharyngeal space post functional phase; 2. No significant differences were observed between males and females with respect to post functional cephalometric changes in mandibular length and hypopharyngeal space.","PeriodicalId":376004,"journal":{"name":"Journal of Contemporary Orthodontics","volume":"336 2","pages":""},"PeriodicalIF":0.0,"publicationDate":"2024-02-15","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"139834558","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The randomly selected patients are divided into 3 groups (20 each) on the basis of type of toothpaste given to them. The patients in groups were asked to brush twice daily with given toothpastes and to discontinue using their normal toothpaste. Plaque specimens were collected from the labial surfaces immediately surrounding the orthodontic brackets of the maxillary lateral incisors by four passes, each along the tooth at the bracket inter-face at the gingival, mesial, distal, and occlusal aspects. The isolated plaque is sent to Microbiology laboratory soon after collecting in a sterile container. The presence of was evaluated in colony forming unit through culture formation and statistical analysis was performed, and comparisons were made. After one month application of a probiotic containing toothpaste (group 3), a fluoridated containing toothpaste (group 2) and a non-fluoridated toothpaste(group 1) the level of significantly reduced in group 2 (non-fluoridated toothpaste) and group3 (Probiotic toothpaste) & minimal reduction is shown in gruop 1. Use of mini implants is one of the innovative approach for complex tooth movements such as mesialisation. Space closure from distal to mesial is one of the most difficult orthodontic tasks. Tooth-borne anchorage limits the scale of tooth movement. Extraoral or intraoral appliances are used especially for unilateral space closure. As an alternative to conventional mesialization appliances, osseointegrated implants may be used for maximum anchorage in orthodontic treatment. The use of direct anchorage in the form of installed mini-implant that is connected to the teeth requiring horizontal movement causes low levels of strains on anchor unit comparing to the results that were registered during indirect anchorage with one or two anchor teeth. This report presents the case of removal of mesiodens and blocked out and fractured premolar and the space closure by unilateral mesialisation with the help of mini implants and loops.","PeriodicalId":376004,"journal":{"name":"Journal of Contemporary Orthodontics","volume":"80 17","pages":""},"PeriodicalIF":0.0,"publicationDate":"2024-02-15","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"139774959","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Pre-treatment lateral cephalograms and orthopantomogram of 90 subjects (30 horizontal, 30 vertical and 30 average growth patterns with subgroup of male and female in horizontal, vertical and average growth pattern), with 8 – 16 years of age, were selected from the patients records who had reported to the Department of Orthodontics & Dentofacial Orthopaedics, Darshan Dental College and Hospital, Udaipur. Lateral Cephalograms were traced manually and orthopantomogram were used to get the overall dental maturity score by adding the value of left eight mandibular teeth. Student’s t test was used to compare the maturation of chronological age and dental age in horizontal, vertical and average growth patterns and Pearson’s correlation test was used to found correlation between the group. The results suggest a difference in dental age and chronological age in subject with vertical growth patterns. Vertical female growers showed early dental age maturation compared to chronological age. There was insignificant difference present in dental age and chronological age in horizontal growth patterns and average growth patterns. Insignificant intergroup difference was found in dental age between horizontal vs vertical, horizontal vs average and vertical vs average growth patterns. Subjects with vertical female growth patterns shows early dental age maturation than chronological age. Sn-Go-Gn, lower anterior facial height and Jara back’s ratio are highly significant in determining growth patterns. There also exists strong correlation between dental age and chronological age in horizontal, vertical and average growth patterns in male and female subgroup.","PeriodicalId":376004,"journal":{"name":"Journal of Contemporary Orthodontics","volume":"12 15","pages":""},"PeriodicalIF":0.0,"publicationDate":"2024-02-15","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"139776218","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
