Not only do patients with dementia exhibit symptoms associated with cognitive impairment, but those with some mental disorders such as psychotic and mood disorders as well. However, differences in cognitive impairment between these disorders remain unclear. In this study, we used the Korean Mini-Mental State Examination (K-MMSE), a tool that can be easily administered to patients, to compare cognitive impairment profiles among patients with dementia, psychotic disorders, and mood disorders. \u0000Methods: We collected demographic and clinical characteristics of 59 patients who were over 50 years old. Cognitive function was assessed using the K-MMSE. Patients were divided into three groups based on International Classification of Diseases 10th revision diagnosis codes: 1) F00-F01 Dementia, 2) F20-F29 Psychotic disorders, and 3) F30-F39 Mood disorders. We compared K-MMSE subscale scores between the three groups using one-way analysis of variance. \u0000Results: The three groups did not differ in demographic data. The dementia group showed the lowest scores in orientation to time (standard deviation [SD]=1.45, F=3.233, p<0.05) and place (SD=1.25, F=3.388, p<0.05), as well as registration (SD=1.00, F=4.425, p<0.05) and recall (SD=0.91, F=3.364, p<0.05) of memory compared to the groups with psychotic and mood disorders. The psychotic disorder group showed significant impairment in language (SD=1.34, F=3.348, p<0.05) compared to the other groups. No significant differences were observed in calculation and drawing. \u0000Conclusion: This study suggests that certain K-MMSE subscale scores could indicate an illness that causes cognitive impairment, especially in dementia, psychotic disorders, and mood disorders. The purpose of this study was to determine the difference in WMH depending on the presence of Post-traumatic stress disorder (PTSD). \u0000Methods: The patients who underwent brain magnetic resonance imaging and clinician-administered PTSD scale (CAPS) at the Veteran Health Service Medical Center were reviewed retrospectively. WMH were evaluated using the Fazekas scale. The patients with PTSD (n=51; age 66.25±10.43 years) were compared with patients exposed to trauma without PTSD (n=67; age 69.45±10.44 years). \u0000Results: The scores of Beck Depression Scale, Beck Anxiety scale, and CAPS were significantly higher in the PTSD group. The PTSD group showed a significant difference on the Fazekas scale compared to the trauma-exposed group without PTSD (periventricular white matter, p=0.001; deep white matter, p=0.008). {"title":"The Effect of Depressive Symptoms in Elderly on General and Cancer Health Screening Participation","authors":"Seung-Run Cho, S. Kim","doi":"10.47825/jkgp.2023.27.1.8","DOIUrl":"https://doi.org/10.47825/jkgp.2023.27.1.8","url":null,"abstract":"Objective: In this study, we investigate the effect of depressive symptoms in elderly on general and cancer health screening participation using the large epidemiological study data in South Korea. \u0000Methods: This study was conducted on 1,977 people who responded to a health examination questionnaire among 2,360 adults aged 60 or older who participated in the Korea National Health and Nutrition Examination Survey (KNHANES) in 2020. Depressive symptoms were measured by the Patient Health Questionnaire-9 and classified as minimal (0-4), mild (5-9), and moderate and severe (10-27). To investigate the relationship between depressive symptoms and health screening participation, a multiple logistic regression model controlling covariates was used. \u0000Results: Compared to the group with minimal depressive symptoms, the mild group did not show a significant difference (odd ratio [OR]=0.71, 95% confidence interval [95% CI]=0.45-1.13), but the moderate to severe group showed a significant negative relationship with general health screening participation (OR=0.59, 95% CI=0.35-0.99). There was a significant negative relationship between depressive symptoms and cancer health screening participation (mild group OR=0.77, 95% CI=0.48-1.23; moderate to severe group OR=0.50, 95% CI=0.28-0.87). \u0000Conclusion: This study showed that people with depressive symptoms are less likely to get a general and cancer health screening. Choo","doi":"10.47825/jkgp.2023.27.1.23","DOIUrl":"https://doi.org/10.47825/jkgp.2023.27.1.23","url":null,"abstract":"Objective: The aim of this study was to investigate the correlation between subjective and objective depression rating scale for the elderly with depressive symptoms. \u0000Methods: Thirty three outpatients aged more than 55 with depressive symptoms were assessed with the Patient Health Questionnare-9 (PHQ-9) and the Montgomery-Asberg Depression Rating Scale (MADRS). Similar items from each scale were paired to investigate their correlations. The level of agreement between the PHQ-9 and the MADRS was also assessed for subjects by using discrepancy-score. \u0000Results: Correlation between total scores of the PHQ-9 and the MADRS was significant (r=0.85, p<0.001). There were significant correlations between the paired items except for the item about volition. These results were the same in the group whose total education years are 7 or more. All subjects were divided into carriers and non-carriers of the e4 allele. Forty APOE e4 carriers and 29 APOE e4 non-carriers underwent neuropsychological, structural magnetic resonance imaging, [18F]fluorodeoxyglucose positron emission tomography scans (PET) and [18F]florbetaben amyloid PET. Analysis of covariance was conducted to compare the differences on cognition, brain volume, glucose metabolism and amyloid deposition between APOE e4 carriers and non-carriers after controlling demographics. \u0000Results: APOE e4 carriers had 50% lower scores of Seoul Verbal Learning Test (delayed recall) compared to non-carriers (0.88±1.65 vs. 1.76±1.75, p<0.05). However, APOE e4 carriers performed better on other cognitive tests than non-carriers (Korean version of Boston Naming Test [11.04±2.55 vs. 9.66±2.82, p<0.05], Rey Complex Figure Test [25.73±8.56 vs. 20.15±10.82, p<0.05], and Stroop test [color response] [48.28±26.33 vs. 31.56±27.03, p<0.05]). APOE e4 carriers had slightly smaller hippocampal volume than non-carriers (3.09±0.38 vs. 3.32±0.38, p<0.05), but greater total brain cortical thickness (1.45±1.55 vs. 1.37±1.24, p<0.05). The physical health, mental health, living habit, environment factor, and cognitive function with differences were balance, physical activity, frail, sarcopenia, and area. The factors affecting cognitive function was education, economic activities, monthly living expense, instrumental activities of daily living, and hope. \u0000Conclusion: This study identified the factors affecting the cognitive function of the elderly living alone, and the most influential factor was Instrumental Activities of Daily Living. This study aims to develop a cognitive intervention treatment program (CITP) as a non-pharmacological therapy, apply this program into MCI patients, and examine patients’ changes in cognitive function. \u0000Methods: Among 16 patients with MCI, 10 patients were randomly assigned to the experimental group and 6 patients were assigned into the control group. The patients assigned to the experimental group participated in the CITP once a week for a period of 15 weeks. The control group were suggested to live a normal daily life without CITP given. After 15 weeks (3 months), pre- and post-investigations, such as cognitive function test, emotional test, brain oxygen saturation test, were conducted and compared for each group. \u0000Results: The cognitive function scores and the brain oxygen saturation levels taken during the Verbal Fluency Test showed a statistically significant difference between those of experimental and the control groups. To be specific, while the cognitive function score improved in the experimental group, there was a decline in the control group. There was no statistically significant difference in emotional changes between two groups. Looking at the changes within each group, the overall cognitive function score of the experimental group was significantly increased, but no pre- and post-significantly changes were observed in brain oxygen saturation activation. On the other hand, the control group showed a statistically significant decline in the attention criteria of the cognitive functional areas, and no statistically significant changed in brain activation. \u0000Conclusion: The result from this study has given some promising views on maintaining and improving the deteriorating cognitive function in patients with MCI. Conducting CITP on patients is expected to strengthen the neural network. This study aimed to investigate factors related to institutionalization in patients with dementia in South Korea using nationwide population-based data. \u0000Methods: National Health Insurance Service and Long-Term Care Insurance (LTCI) database were used for analysis. The study population consisted of patients with dementia aged 65 years or older who qualified for LTCI institutional care benefits from 2008 to 2017. Multiple logistic regression was used to analyze factors related to institutionalization in patients with dementia. \u0000Results: We identified 141,552 patients, of which 90,110 were institutionalized and 51,442 were not. Older age, female sex, low income, non-family caregiver, less comorbidities, no disability and diagnosis of Alzheimer’s disease (AD) were related to institutionalization. \u0000Conclusion: Consistent with previous studies, older age and diagnosis of AD were related to institutionalization.
