Irga-Jaworska","doi":"10.5603/ahp.a2023.0040","DOIUrl":"https://doi.org/10.5603/ahp.a2023.0040","url":null,"abstract":"Introduction: Hypertriglyceridemia (HTG) is one of the common complications of the regimens based on steroids and asparaginase used in the treatment of acute lymphoblastic leukemia (ALL) and lymphoblastic lymphoma (LBL) in children. The aim of this cross-sectional retrospective study was the analysis of the prevalence, clinical course and management of hypertriglyceridemia following the administration of asparaginase and steroids according to the binding protocols. Material and methods: A cohort of 75 children with ALL or LBL was analyzed with reference to anthropometric and laboratory parameters, clinical symptoms, implemented treatment, and complications. Results: The prevalence of HTG in the analyzed cohort was 29.3%. Risk factors for HTG development were older age, cachexia and lower body mass index, but there was no correlation with high risk group. It worsens their quality of life especially if it leads to chronic kidney disease (CKD). Collagen turnover is known to be disturbed in CKD. Urinary type IV collagen (U-Col4) is recognized as an exponent of extracellular matrix synthesis and degradation in glomeruli and is thus involved in this turnover. The purpose of this study was to evaluate the usefulness of U-Col4 in assessing early renal impairment in CCSs. Material and methods: Seventy-eight CCSs, without CKD, bilateral kidney tumors, congenital kidney defects, urinary tract infections and diabetes, at least one year after ending antineoplastic treatment, and aged 1–18 years, were divided into three groups: 1) patients after nephroblastoma treatment (n = 18); 2) patients after other solid tumors treatment (n = 42); and 3) patients after anti-hamatopoietic and lymphatic system proliferative treatment (n = 18). Although in the past ALL was incurable, today’s survival rate is as high as 80–90%. The most important drugs in its treatment are glucocorticosteroids (GCS). This study focuses on exogenous GCS — dexamethasone and prednisolone, which suppress the hypothalamic–pituitary–adrenal (HPA) axis. The main objective of this study was to analyze articles about steroid side effects in ALL therapy, factors contributing to the frequency of HPA axis suppression, and possible methods for its assessment. According to research, abnormal cortisol values have been observed in almost every patient treated for ALL. This means it is important to perform HPA axis stimulation tests. In the reviewed studies, adrenal crisis, hyperglycemia, obesity, iatrogenic Cushing’s disease, growth failure, increased risk of infection, and a high mortality rate have been reported to be the most important complications of this therapy. It is important to note that some authors classify patients in terms of cortisol level disorder predispositions.
