This article explores the conceptualisation of loneliness, emphasising its subjective nature and the perceived discrepancy between desired and actual social relationships. Drawing from psychological theories, loneliness is understood as an emotional response to perceived social isolation, encompassing feelings of emptiness, sadness and longing for companionship. In older adults, loneliness is exacerbated by life transitions, such as retirement and bereavement, as well as physical health limitations. The impact of loneliness on older adults' health and wellbeing is significant, with associations documented between chronic loneliness and poor mental health outcomes, including depression, anxiety and cognitive decline. Moreover, loneliness is linked to adverse physical health outcomes, such as cardiovascular disease and mortality, as well as functional decline and decreased quality of life. This case report discussed the case of a 66-year-old male patient who had adhered to a strict vegetarian diet for 5 years and presented with symptoms of fatigue, pallor and leg pain. Laboratory tests revealed severe pancytopenia (haemoglobin 4.8 g/dL, white cell count 2630/µL, platelets 112 000/mm<sup>3</sup>), macrocytic anaemia (mean cell volume 118 fL) and elevated lactate dehydrogenase (5480 U/L). Serum vitamin B12 was critically low at 61 pg/mL and bone marrow aspiration confirmed megaloblastic erythropoiesis. Intramuscular vitamin B12 supplementation resulted in rapid clinical and haematologic improvement, with complete resolution of pancytopenia and indirect hyperbilirubinaemia within 2 weeks. This case underscores the importance of early recognition of vitamin B12 deficiency in patients with unexplained cytopenia and haemolysis, particularly in populations with restrictive dietary habits. It also highlights ISC as a key intervention once high-pressure retention has resolved, ensuring bladder health is maintained, complications are minimised and quality of life is improved. The authors also provide an overview of how CUR can significantly impact the physical, social and psychological aspects of a man's life and the vital role of nurses in the holistic management of the condition. A case study is provided to demonstrate the impact of delayed management of the patient.
