High-quality colonoscopy is important for optimal detection of adenomas and serrated polyps and mitigating the risk of postcolonoscopy colorectal cancer (PCCRC). Optimal bowel preparation quality is also important in reducing PCCRC incidence. Key performance metrics described in this article include bowel preparation adequacy rate, cecal intubation rate, adenoma detection rate, adenomas per colonoscopy, sessile serrated lesion detection rate, and withdrawal time. To date, 3 blood-based CRC screening tests have completed large validation studies (Epi proColon, Shield, and the Freenome test). Multi-cancer early detection tests are also actively being investigated for their role in CRC screening. Stool-based tests are becoming more essential in CRC screening due to their accessibility, affordability, and noninvasive nature. While screening performance varies among modalities, adherence to testing and timely follow-up remains the main barrier to effectiveness. Fecal immunochemical test (FIT) offers a favorable balance of sensitivity, cost, and patient uptake. Newer DNA-based and RNA-based tests improve early-stage cancer detection but are costlier and marginally less specific. High-quality colonoscopy is also essential. Tracking of screening activities has the potential to enhance program performance, support equitable screening delivery, and strengthen preventive impact at the population level.
