It primarily affects young women. The clinical characteristic is nonspecific, but the pathological characteristic is distinct. The tumor consists of both cystic and solid component. The typical histopathological appearance of SPT is the pseudopapillary structure with a fibrovascular surrounded by several layers of epithelial cells.Only a few SPTs present invasive behaviors. The tumor cells express a wide range of immunohistochemical markers. Usually the neonates are born with no clinical symptoms. Nevertheless, in the post natal period, a non-bilious, vigorous, “projectile” vomiting ensues. Emaciation result from the gastric outlet obstruction terminating in death in untreated cases. Palpation of the thickened pylorus or “olive” is the mainstay of clinical evaluation. Precise abdominal palpation is contingent to determinants such as the investigator’s judgement, gastric distension and a sedated infant. Intestinal epithelial barrier function disturbances or defect in intrinsic immune, can lead to increased antigens uptake that stimulate the immune system. So inflammatory response occurred [1-4]. Intestinal microbes may cause widespread diseases so their interactions with the host are the subject of recently researches. Planned manipulation of the intestinal microflora with a therapeutic intention are of the main parts of these researches. Dyspepsia is a frequent syndrome our country, while there are restrictions to endoscopy and high prevalence of Helicobacter pylori (H. pylori) infection. This study was conducted to evaluate the endoscopic findings of the syndrome, in an outpatient screening clinic of the Baqiyatallah Hospital in Tehran, Iran. \u0000Material and methods: Between 20 March and 20 November 2015 and according to Rome III criteria, outpatients with uninvestigated dyspepsia, answered Dyspepsia Hong Kong Index Questionnaire and underwent esophagogastroduodenoscopy. The Rapid Urease Test was applied to fragments of the antral mucosa, and epidemiological data were collected from the studied population. Normal endoscopic findings were analyzed with different variables to verify statistically significant associations. \u0000Results: Eventually 400 patients (57.5% males and 42.5% females) with the mean age of 51.32±17.98 years were evaluated. About 55% of them were smokers and 45% used NSAID. H. pylori (80%) were the most endoscopic findings between Endoscopic findings. Among positive and negative H. pylori patients, 56.25% and 50% were smokers respectively. All patients with gastric adenocarcinoma (100%) and 13.2% patients without gastric adenocarcinoma were with fundus, body or antrum ulcer. \u0000Conclusions: Predominance of functional disease was showed in our setting by the endoscopic diagnosis of uninvestigated dyspepsia. Recent studies have disclosed the relationship between Helicobacter pylori (H. pylori) infection and IR. This study aimed to evaluate the differences in IR between NAFLD patients with and those without H. pylori infection. \u0000Patients and method: From January 2011 to December 2012, patients with pathologic proof of NAFLD were included. All patients received a C14 urea breath test for H. pylori infection evaluation. Serum samples were taken for homeostasis model assessment of insulin resistance [HOMA-IR], lipid profile, inflammatory cytokine and adipokine studies. Patients with H. pylori infection received standard triple eradication. Serum samples were then subjected to HOMA-IR three and six months after eradication therapy. \u0000Results: Eleven patients with both NAFLD and H. pylori infection and 18 patients with only NAFLD were included into this study. There were no significant differences between the two groups in demographic, laboratory or histological data. The only difference was mean HDL value. Patients with H. pylori infection had higher mean HDL values than patients without H. pylori infection [50.1 ± 8.8 vs. 38.8 ± 9.7 mg/dL, p=0.004]. The serial values of HOMA-IR from patients with H. pylori infection following H. pylori eradication 0, 3 or 6 months later did not reveal an increasing or decreasing trend following H. pylori eradication treatment. Conclusion: There were no significant differences in HOMA-IR values between NAFLD patients regardless of whether they had H. pylori infection.
