{"title":"[A CASE OF ALLERGIC BRONCHOPULMONARY ASPERGILLOSIS THAT DEVELOPED FROM DE NOVO SENSITIZATION IN A STABLE NON-ATOPIC ASTHMA, DIAGNOSED BY COMPUTED TOMOGRAPHY].","authors":"Rina Harigane, Mami Rikimaru, Hiroaki Kume","doi":"10.15036/arerugi.75.153","DOIUrl":"https://doi.org/10.15036/arerugi.75.153","url":null,"abstract":"<p><p>A 76-year-old woman was diagnosed with asthma at 40 years of age because of chest tightness and wheezing. She was considered to have non-atopic asthma, because total and specific IgE values were normal. Combination therapy with inhaled corticosteroid and long-acting β<sub>2</sub>-adrenergic agonist was effective. Thereafter, the patient remained asymptomatic. Chest radiograph and pulmonary function test were normal, excluding mild elevation of peripheral blood eosinophil counts (maximum value of 670/μL) during this period. At 73 years of age, the chest computed tomography (CT) performed during this stable phase showed mucoid impaction, including a high-attenuation area in the left lower lobar bronchi (B<sup>8</sup>, B<sup>9</sup>) with central bronchiectasis. These high-attenuation mucus (HAM) plugs were not visible in chest radiograph. Re-examination of the blood test revealed elevated total IgE and specific IgE to Aspergillus. Moreover, Aspergillus IgG antibody titer was positive. Therefore, the patient was diagnosed with allergic bronchopulmonary aspergillosis (ABPA). Oral corticosteroids effectively removed HAM plugs. However, HAM plugs recurred in the same bronchi after 16 months. {"title":"[TWO CASES OF ASTHMA INDUCED BY SMOKE FROM BURNING RICE STRAWS AND RICE HUSKS].","authors":"Hiroaki Kume, Riko Sato, Ryuki Yamada, Xintao Wang","doi":"10.15036/arerugi.75.52","DOIUrl":"https://doi.org/10.15036/arerugi.75.52","url":null,"abstract":"<p><p>Two patients-a 40-years-old man and a 48-years-old woman- presented with dyspnea, a severe cough, and wheeze due to the inhalation of smoke from burning rice straws and rice husks. Systemic corticosteroids are often administered to relieve these symptoms. Since rice straws and rice husks are burned in some rice-producing districts of Japan during early autumn, both patients had previously been hospitalized twice due to the worsening of their symptoms in September-November. These symptoms are also caused by the inhalation of smoke from burning incense sticks, fragrances from lilies, and fragrances from air fresheners. These environmental substances include volatile organic compounds (VOCs), such as formaldehyde, acetaldehyde, benzene and benzyl alcohol. Multiple chemical sensitivity (MCS) may be associated with these symptoms. However, forced expiratory volume in one second (FEV<sub>1</sub>) fluctuated by more than 0.6l in both cases during the treatment period, indicating variability in airflow obstruction. The 40-years-old man causes wheezing. Although wheezing was not clear in the 48-years-old woman, the methacholine provocation test indicated airway hyperresponsiveness development. Based on these results, both patients were diagnosed with asthma due to VOCs. The peripheral eosinophil counts and fractional exhaled nitric oxide levels were not elevated, indicating no association with eosinophilic airway inflammation. {"title":"[SURVEY ON THE AWARENESS OF TEACHERS REGARDING BULLYING OF STUDENTS WITH FOOD ALLERGIES].","authors":"Chihiro Kunigami, Takanori Imai","doi":"10.15036/arerugi.75.44","DOIUrl":"https://doi.org/10.15036/arerugi.75.44","url":null,"abstract":"<p><strong>Purpose: </strong>To clarify Japanese school staff's perceptions of bullying among students with food allergies.</p><p><strong>Methods: </strong>School staff were recruited during an online training course on allergy management held in 2024, and a web-based questionnaire survey was conducted. Survey items included respondent characteristics, the perceived proportion of students who had ever experienced bullying, the perceived bullying risk for students with food allergies, and experience with bullying cases related to food allergy.</p><p><strong>Results: </strong>Responses from 198 school staff members (response rate 25%) were analyzed. Overall, 54% believed that students with food allergies are at higher risk of being bullied. Ten percent had experienced bullying targeting students with food allergies, and some reported incidents in which allergens were used as a means of bullying. As an effective measure to reduce this risk, 93% endorsed education about food allergies provided by teachers to all students.</p><p><strong>Conclusion: </strong>Many school staff members recognized the bullying risk faced by students with food allergies, and actual cases of potentially life-threatening bullying using allergens were reported. {"title":"[POST-TREATMENT SURVEY OF THE MAINTENANCE OF EFFECTS FOR CEDAR SUBLINGUAL IMMUNOTHERAPY].","authors":"Atsushi Yuta, Yukiko Ogawa, Hideaki Kozaki, Koji Matsumoto, Takeshi Shimizu, Mitsuhiro Okano","doi":"10.15036/arerugi.75.132","DOIUrl":"https://doi.org/10.15036/arerugi.75.132","url":null,"abstract":"<p><p>We studied post-treatment survey of the maintenance of effects for cedar sublingual immunotherapy (SLIT).</p><p><strong>Methods: </strong>Patients who started between 2018 and 2020 and had completed at least three seasons of SLIT with Cedarcure (post-SLIT patients) were surveyed by mail in 2025, regarding as 1) impressions compared to the time of SLIT completion, 2) the VAS (best 0-worst 100mm) of general symptoms in pollen season at 3 points (pre-SLIT, at the end of SLIT and in 2025), 3) medication score. Responses were received from 200 of 370 patients (age 15 [11-42] years, male 97).</p><p><strong>Results: </strong>The percentages of the maintenance of effects were 61%, 59%, 51% and 34% in order of 1- to 4-year follow-up, respectively (p<0.05 in 4-year vs 1- or 2-year, unadjusted and uncorrected). The VAS for all patients changed 90→10→20 in median at the three time points. On the 2025 VAS, the 4-year follow-up was significantly worse than the 1- and 2-year follow-ups, and the SLIT treated for 3 years was significantly worse than for 4 or 5 years (p<0.05). Compared with currently undergoing SLIT with the same treatment duration, the VAS of post-SLIT patients was significantly worsened in the group of 3 years of treatment, but not in the group of 4 or 5 years. The medication score was 0.3 in median, and there was no difference with currently SLIT patients.</p><p><strong>Conclusion: </strong>Although many patients progressed well, some cases worsened, and the four-year follow-up was significantly worse.
