Excessive noise can reduce staff performance and disrupt patients' sleep, delaying their recovery and leading to a prolonged stay. Research has suggested that digital noise display devices can alert ward staff and prompt them to reduce noise levels, but the literature on this topic varies. This systematic review summarises the evidence regarding the use of digital noise display devices as the primary intervention to reduce noise levels in hospital wards. The authors searched CINAHL, MEDLINE and PubMed databases, as well as manually harvesting through the references. A total of 1110 articles were identified, of which eight were eligible for inclusion. Thematic analysis was used to synthesise the findings. The evidence demonstrated that digital noise display interventions could reduce noise levels in wards by 2–3 decibels and raise staff awareness of noise. However, there was a lack of evidence that this intervention changed staff behaviour regarding noise levels. This systematic review investigated the effects of efficient perioperative systems on the timeliness of upstream and downstream processes in surgical care pathways, looking at how these methods could reduce overall patient waiting times for elective surgery. The authors searched PubMed, EMBASE, SCOPUS, Web of Science and Cochrane databases for articles published after 1 January 2014. Both randomised and non-randomised studies were considered. A total of 7543 publications were screened, of which 20 were eligible for analysis. The studies varied widely in design, scope, reported outcomes and overall quality. Analysis demonstrated that a substantial amount of time could be saved through efficient scheduling and planning of perioperative processes, which could reduce overall patient waiting time for elective surgeries. Petri","doi":"10.12968/bjhc.2021.0025","DOIUrl":"https://doi.org/10.12968/bjhc.2021.0025","url":null,"abstract":"The authors' hospital implemented an intensity of care model in the surgical department, grouping patients by the level of care they required (low, high or intensive) and creating a short-stay unit for patients requiring a low level of care. This study evaluated care quality, along with staff and patient perceptions, in the department 3 years after this model was implemented. Data were collected from mandatory national databanks regarding organisation, economic, staff and patient factors relating to care quality and efficiency. The department's performance according to National Outcome Programme indicators was also assessed. Data from staff and patient satisfaction questionnaires used for routine audit were analysed retrospectively. Both questionnaires asked respondents to rate their satisfaction to items on a 5-point Likert scale so their scores could be analysed. Satisfaction scores for staff in the short-stay unit were compared with those working in general surgery using a t-test, with P<0.05 indicating a significant difference. Some improvements were found 3 years after the model was implemented, including increased elective activity and reduced length of stay. Patient satisfaction with the short-stay unit was generally high. However, staff satisfaction was generally low and turnover rates were high. This study analysed the moderating role of individual conscientiousness on the relationship between psychological contract breach and levels of job involvement among medical doctors working in Ghana. A psychological contract breach occurs when an organisation does not deliver on their expected obligations to an employee, whether these expectations are written or unwritten. All 251 doctors working in the upper east and upper west regions of Ghana were invited to participate, of which 214 doctors responded. Participants completed self-administered questionnaires, with 15 statements relating to psychological contract breach, 10 statements relating to job involvement and four statements relating to individual conscientiousness. Respondents rated their agreement to the statements on a 7-point Likert scale (1=least agreement; 7=strongest agreement). Participant demographics were also collected. Relationships between the data for each section were analysed using the Statistical Package for the Social Sciences (version 24) and the Smart PLS 2.0M.3. Psychological contract breach was found to have a significant negative relationship with job involvement (P<0.05). Conscientiousness had a moderating effect on this relationship, meaning that doctors who were more conscientious were less likely to reduce their job involvement when a psychological contract breach occurred. When a psychological contract breach occurs, doctors who have a conscientious personality trait are likely to continue showing high levels of job involvement. These hubs are now seen as a key resource for more robust and sustainable elective services, backed by bodies such as NHS England and the Royal College of Surgeons of England. The importance of a well-trained workforce to staff these hubs cannot be overstated. While evidence for the benefits of surgical hubs for patient care continues to come out, this article will focus instead on the potential benefits for staff.
