Donoso-Hofer","doi":"10.20986/recom.2023.1448/2023","DOIUrl":"https://doi.org/10.20986/recom.2023.1448/2023","url":null,"abstract":"Malignant neoplasms of lymphoid origin correspond to a complex group of diseases whose diagnosis is difficult since it requires the application of immunohistochemical tests, an exhaustive analysis by the histopathological team as well as a representative sample of the tissue to be examined. We present the case of a patient previously diagnosed with non-Hodgkin's lymphoma type T, with clinical manifestation in the oral cavity, who presented a poor response to the initial chemotherapeutic scheme. Due to the therapeutic failure, new biopsies and immunohistochemical analysis were requested, being the oral cavity sample the one that confirmed the diagnosis, this time correctly, of T-cell lymphoblastic lymphoma, thus modifying the initial therapeutic scheme to one of greater intensity, achieving a favorable evolution of the disease. The present article corresponds to the first case of T-cell lymphoblastic lymphoma with maxillary location reported in the literature. Possibility of cardiovascular complications can be anticipated in these patients. This study compares blood pressure, heart rate, and peripheral capillary oxygen saturation, in normotensive and hypertensive patients prior to, during and after dental extractions. Patients and methods: A prospective comparative study was carried out in 100 (N = 100) patients between the age group of 40-70 years. 50 patients diagnosed with Stage I hypertension and 50 patients who were normotensives, requiring extraction of tooth were recruited. They were further divided into four groups namely, A, B, C and D, each consisting of 25 (n = 25). Group A included patients with controlled hypertension who were administered local anesthesia with epinephrine, while Group B had patients with controlled hypertension who were administered local anesthesia without epinephrine. Group C had normotensive patients who were administered local anesthesia with epinephrine and normotensive patients who were administered local anesthesia without epinephrine were under group D. The patients were evaluated at preoperative, intra operatively and post-operative interval for blood pressure, heart rate and peripheral oxygen saturation. Results: The sample consisted of 42 males and 58 females with a mean age of 42.6, 49.9, 40.6 and 31.6 years in group A, B, C and D respectively. Both systolic and diastolic blood pressure was increased after administration of anesthetic in all four group and decreased lower than baseline level after the surgical procedure. Heart rate was also found to be increased in all groups. A significant difference in peripheral oxygen saturation levels between group A and group B was observed post operatively. Se diagnostican inicialmente como lesiones metastásicas de un tumor estromal gastrointestinal, siendo el diagnóstico histopatológico final de un angiosarcoma agresivo y diseminado. La presentación clínica de este tipo de lesiones dificulta el diagnóstico definitivo y, en consecuencia, el manejo de este tipo de pacientes. Palabras ABSTRACT Oral angiosarcoma is a rare endothelial tumour that requires to be included as a differential diagnosis of several similar lesions due to its clinical presentation. We present a 66-year-old patient undergoing treatment for multiple digestive bleeding episodes, referred to the oral and maxillofacial surgery service of the Hospital de Carabineros de Santiago de Chile due to the presence of intraoral vascular lesions initially diagnosed as metastatic lesions of a gastrointestinal stromal tumour, with the final histopathological diagnosis being an aggressive and disseminated angiosarcoma. Clinical case: A 66-year-old female patient who had a history of multiple TMJ surgeries complained of functional worsening and pain in the left TMJ, with Walter-Lorenz prosthesis in the right TMJ. Intervention was performed to place another Walter-Lorenz prosthesis in the left TMJ with virtual 3D surgical planning of the osteotomies of the fossa and the ramus, and surgical guides fabricated with CAD/CAM technology. Surgery was carried out according to surgical planning. In the subsequent revisions, the mouth opening was stable and the pain was reduced. Histopathologically it preserves an ameloblastomatous differentiation pattern but shows cytological features of malignancy. Due to its rarity, there is limited information on its biological behavior, which is recognized in the literature for being nonspecific and variable, as it may present as a lesion with characteristics compatible with a benign lesion or as a highly malignant aggressive pathological entity. The latter has been a difficulty not only in its diagnosis but also in the therapeutical planning. They are located predominantly at the skull base and the sacrococcygeal region. The cervical spine is affected in only 6 % of cases. The treatment of choice includes resection, radiotherapy and sometimes chemotherapy. A case of a patient diagnosed with cervical chordoma without response to previous radiotherapy treatment is presented. The patient underwent surgical rescue including tumor excision, C2-C3 corpectomy and fixation with prosthetic material. He presented a surgical wound dehiscence in the oropharyngeal region in the postoperative period, with exposure of the prosthetic material.
