{"title":"Is the Endometrium in Women with PCOS Compromised?","authors":"T. Piltonen","doi":"10.1017/9781108236263.022","DOIUrl":"https://doi.org/10.1017/9781108236263.022","url":null,"abstract":"21.1 PCOS Endometrium and ART Even though women with polycystic ovary syndrome (PCOS) are as likely to get at least one child as the general population [1], the diagnosis remains as the most common reason for anovulatory infertility; thus, these women are overrepresented among women undergoing assisted reproductive technologies (ART). Although anovulation can be solved in many cases with ovulation induction medication, the metabolic component remains often unsolved in ART contributing to subfertility in these women. Besides ovarian dysfunction there are several studies showing that the endometrial function is also compromised in women with PCOS, mostly related to anovulation. However, studies have also implied that even with ovulation, the endometrial function in women with PCOS remains unoptimal [2,3]. Indeed, there are several clinical manifestations that imply endometrial dysfunction in PCOS. The first reported endometrium-related clinically adverse outcome was miscarriage risk that was suggested to be increased in women with PCOS [4], even independently of body mass index (BMI) and ART [5], although not all studies agree [1]. Furthermore, affected women have been shown to present with a threeto fourfold increased risk for pregnancy-induced hypertension and preeclampsia, and a twofold higher chance of premature delivery [6,31]. The most crucial health burden for PCOS endometrium is the abnormal and unfavorable metabolic state and obesity in these women that also increases the need for ART. In addition to the impaired oocyte quality and the need for higher doses of gonadotrophins in in vitro fertilization (IVF), it is likely that adverse metabolic phenotype (women presenting with amenorrhea, hyperandrogenism, obesity, diabetes, hypertension) also affects the preconceptional endometrial milieu in women with PCOS [7]. In fact, some adverse outcomes may be derived from abnormal trophoblast invasion and placentation in cases of PCOS shown as altered placental histology with increased inflammation, thrombosis, and poor placental development even in uncomplicated PCOS pregnancies [8]. As for clinical practice, several studies have been evaluating the optimal treatment protocol for women with PCOS for ART. Recently, aromatase inhibitors were shown to yield higher live birth rates (LBRs) in women with PCOS compared with clomiphene citrate, possibly partly due to more beneficial implantation profile [9]. Indeed, a recent study reported secretory-phase endometrial gene expression profile more favorable for implantation in women using letrozol compared with clomiphene citrate [10]. Also, IVF protocol plays a role. For women with PCOS being in high risk for ovarian hyperstimulation syndrome (OHSS), antagonist protocol is recommended.
