Ortiz-López","doi":"10.1016/j.rmu.2016.05.007","DOIUrl":"10.1016/j.rmu.2016.05.007","url":null,"abstract":"<div><p>Triple negative breast cancer (TNBC) is a subtype of breast cancer (BC) with a heterogeneous nature that stains negatively for estrogen receptor (ER), progesterone receptor (PR) and human epidermal growth factor 2 (HER2) during immunohistochemistry. Approximately 15–20% of all cases of breast cancer are triple negative phenotypes. Compared to patients with hormone receptor-positive cancer, TNBC patients are typically younger (<50 years), African American, and have a high incidence of mutations in BRCA1/2 genes. To date, not a single targeted therapy has been approved for TNBC treatment, and cytotoxic chemotherapy remains as the standard systemic treatment, meaning that TNBC is an aggressive subtype of breast cancer with a poor prognosis. In this review, the literature search was done up to date on which gene expression profile of TNBC has been analyzed in order to identify the consensus on molecular mechanisms involved in carcinogenesis and/or the prognostic markers of the disease. In conclusion, these studies have reported that TNBC is composed of several clusters or genomic signatures as basal keratins. They have also reported on their proliferation, luminal/basal apocrine, regulatory interferon, immune cells/immunoglobulin related to stem cells, epithelial-mesenchymal, androgen receptor and angiogenesis. However, not all research groups have reported reproducible results. This confirms the heterogeneous nature of TNBC and the need for research on uniform selection criteria. However, these discoveries have led to the proposal of new treatments, such as the addition of platinum salts, new combinations of therapeutic agents, some targeted therapies such as PARP inhibitors, and PI3K and androgen antagonists. It is an entity with a poor prognosis. Several conditions can cause this, the most common being colon, rectum, ovary, stomach or appendix cancers, including peritoneal pseudomyxoma, among others. The abdominal cavity invasion is considered a clinical stage IV. For a long time life expectancy of this entity was very short. With the advent of meticulous techniques in cytoreductive surgery (CRS) and hyperthermic intraperitoneal chemotherapy (HIPEC) the prognosis of patients has changed. In some conditions, these procedures are standard treatments. CRS is a very important prognostic factor; leaving a less residual disease in the patient, the evolution will be better. The HIPEC starts immediately after the surgical event. The hyperthermia increases the cytotoxic effect of antineoplastic drugs. Numerous studies have appeared in medical literature wherein the clear improvement in survival of the affected population is demonstrated. They may also cause an important effect in the social and psychological development of the patient because of the aesthetic problems that they produce. This article presents a case of a 20 years-old female patient who was referred to our institution because of facial trauma secondary to a gunshot wound, whose treatment consisted of multiple reconstructive surgeries in order to obtain an adequate, functional and esthetic result. Flores-Treviño","doi":"10.1016/j.rmu.2016.01.001","DOIUrl":"10.1016/j.rmu.2016.01.001","url":null,"abstract":"<div><h3>Objectives</h3><p>Set the sensitivity of the histopathological diagnosis of chorioamnionitis (CAMH) for early diagnosis of neonatal sepsis and the relationship between histological chorioamnionitis and premature rupture of membranes and neonatal sepsis.</p></div><div><h3>Materials and methods</h3><p>Prospective, observational study and diagnostic test performed in the Neonatology Service of the “Dr. José Eleuterio González” University Hospital. Epidemiological variables were collected from mothers and newborns. The relationship between histological chorioamnionitis with premature rupture of membranes and early neonatal sepsis was established.</p></div><div><h3>Results</h3><p>We recorded 3694 births. Of these, 122 patients were studied as potentially infected, of whom 37 patients were excluded (2 by transfer to another hospital and 35 by not finding a histopathological study of the placenta). The study included 85 newborns. Of these, 43 (50.5%) developed clinical and laboratory data of early neonatal sepsis, the rest (<em>n</em> <!-->=<!--> <!-->42, 49.5%) were healthy newborns. The sensitivity of histological chorioamnionitis with premature rupture of membranes (PRM) of more than 24<!--> <!-->h was 81% for neonatal sepsis and 51% without. The risk factors for neonatal sepsis were: Mother with infection (<em>p</em> <!--><<!--> <!-->0.001), weight <1500<!--> <!-->g (<0.001), gestational age <28 weeks (<0.05), APGAR score <6 in 5<!--> <!-->min (<em>p</em> <!--><<!--> <!-->0.05).</p></div><div><h3>Conclusions</h3><p>Placental chorioamnionitis with premature rupture of membranes > 24<!--> <!-->h has an 81% sensitivity for neonatal sepsis. It is estimated that more than 90% of mortality happens before the patient reaches the hospital and only 15–50% of those will receive appropriate medical treatment.</p></div><div><h3>Case report</h3><p>A 42-year-old hemodynamically stable male is brought to the ED with a protruding nail in his thorax. He is taken to the operating room (OR) where a medial sternotomy is performed and an injury is found in the left ventricle.
