Barry","doi":"10.1308/rcsfdj.2023.22","DOIUrl":"https://doi.org/10.1308/rcsfdj.2023.22","url":null,"abstract":"There are limited paediatric specific data on antibiotic prescribing practices, which have been cited as one of the causes of general overuse and misuse of antibiotics in young patients. The aim of this study was to investigate prescribing practices among dentists with an interest in paediatric dentistry in the UK. A prospective cross-sectional survey was distributed electronically to members of the British Society of Paediatric Dentistry between February and May 2021. The questions and scenarios were based on antibiotic prescribing guidelines in the UK. Two hundred and six dentists completed the survey (45% response rate). Most participants (93%) conformed to guidelines and prescribed amoxicillin as a first-line antibiotic for an odontogenic infection. For patients with a penicillin allergy, 16% of dentists reported clindamycin as their first-choice antibiotic. Seventeen per cent of those surveyed stated that they would prescribe antibiotics for local swelling without any additional factors indicating a persistent or spreading bacterial infection. Further insights into prescribing practices for prophylaxis, paediatric doses and non-clinical factors are described. Antibiotics are part of the armamentarium that dentists can use to treat patients. Updated guidance emphasised antimicrobial prescribing for children. This study assessed whether the correct dose of antibiotics had been prescribed for children who were triaged by telephone at a dental emergency clinic in a London teaching hospital during the COVID-19 lockdown. Data were collected for a retrospective service evaluation from the case notes of children whose parents had contacted the triage telephone line between May and September 2020. A proforma was used to extract information including the type of antibiotics that had been prescribed, the prescriber and the dose. Of the 815 children who were triaged via the telephone emergency clinic, 229 (28%) had received antibiotics. Three-quarters (n = 175, 76%) had been prescribed amoxicillin. Half of these (n = 88, 50%) had been prescribed a lower than recommended dose of amoxicillin; all incorrect dosages were prescribed by dentists in primary care. Patients aged 5–11 years were most affected by this (65%). Incorrect antibiotic dose prescribing is a likely contributor to antimicrobial resistance as these patients often required further antimicrobial treatment. Written consent must be obtained where treatment involves conscious sedation or general anaesthesia. For children, consent may be provided by a person with parental responsibility (PR). A retrospective evaluation was completed of 160 children over 2 UK hospital sites with paediatric services. Cases involving conscious sedation or general anaesthesia for dental treatment were selected. Data were obtained to establish whether it was documented that the correct person had provided consent for a child and whether all possible individuals with PR for the child were identified at the initial visit. UK national legislation and guidance was reviewed, from which a PR form (to determine PR status for a child) was created and implemented. A second evaluation was subsequently completed, again with 160 children. Combined data from both sites confirmed documentation of an appropriate person providing consent in 127 cases (79%) in the first evaluation. This improved to 155 cases (97%) following implementation of the PR form. All possible individuals who had PR for the child were identified at the initial visit in 35 cases (22%). This improved to 139 cases (87%) following the introduction of the PR form.
