Shim","doi":"10.15279/KPBA.2019.24.1.35","DOIUrl":"https://doi.org/10.15279/KPBA.2019.24.1.35","url":null,"abstract":"Bronchobiliary fistula (BBF) is a rare complication after transcatheter arterial chemoembolization (TACE) for hepatocellular carcinoma. We present a case of BBF that developed 5 months after 11 TACE procedures in a 78-year-old male patient with a 3-month history of a persistent cough and yellowish sputum. BBF was found between the bronchus and the right intrahepatic bile duct (B7) by magnetic resonance cholangiopancreatography. On the initial endoscopic retrograde cholangiopancreatography (ERCP), we failed to approach to the BBF due to severe hilar obstruction. Percutaneous transhepatic biliary drainage (PTBD) was performed, and 30-50 mL of bile was drained daily. But the BBF was not resolved until 2 months after PTBD. The patient was treated by placement of a plastic stent to the BBF site during the second ERCP. BBF was resolved 7 days after ERCP on the PTBD tubogram. It is associated with high rates of morbidity and mortality compared to interstitial edematous pancreatitis. A collection of fluid and necrotic tissue is called acute necrotic collections (ANC) and may form an enhancing wall consisting of reactive tissue after 4 weeks, which is called walled-off necrosis (WON). ANC and WON could be either sterile or infected. WON is often complex and septated, and when it gets infected or causes other serious complications, drainage or resection is indicated. The traditional approach is to surgically remove all the infected necrotic tissue, but this invasive approach carries high rates of complications and death. The recent advance of percutaneous and/or endoscopic approaches has enabled a stepup method for the management of necrotizing pancreatitis. Serious complications such as perforation, pancreatitis, hemorrhage, and sepsis are rarely reported. To our knowledge, delayed hemorrhage after EUS-FNA is very rare and hemorrhage from iatrogenic pseudoaneurysm has yet to be reported. We report a case of delayed hemorrhage from gastroduodenal artery pseudoaneurysm, which developed after EUS-FNA of a solid pancreatic lesion. A 68-year-old man presented with tarry stool 10 days after EUS-FNA of a 1.5 cm-sized pancreatic head mass. Abdominal computed tomography showed a 2-cm-sized intensely enhancing round lesion near pancreatic head. EUS-FNA was negative for malignancy. The patient refused admission for further evaluation. Twelve days later, he reported to the emergency room with persistent tarry stool. Angiography showed a gastroduodenal artery pseudoaneurysm. Subsequent coil embolization resulted in successful hemostasis. The patient underwent pylorus-preserving pancreaticoduodenectomy and was diagnosed with stage IIB pancreatic cancer. The suggested mechanism of acute pancreatitis with diaphragmatic hernia is that herniation of pancreas itself. The other possible mechanism is pancreatic ischemia due to traction after aggravation of hernia or acute distention of stomach. We report a case of acute pancreatitis due to diaphragmatic hernia with no evidence of herniation of pancreas. A 78-year-old male was administered for epigastric pain and dyspnea. The radiologic and laboratory result demonstrated an acute pancreatitis with diaphragmatic hernia without any evidence of herniation of pancreas. The patient was managed conservatively to reduce the hernia and to treat pancreatitis. One is functional dyspepsia in the gastrointestinal tract and the other is pancreatobiliary dyspepsia. Biliary dyspepsia is caused by biliary tract disease and can even cause biliary pain. Acalculous biliary pain (ABP) is biliary colic without gallstones, it is caused by functional biliary disorder or structural disorders such as microlithiasis, sludges or parasitic infestation like Clonorchiasis. The endoscopic ultrasonography is helpful tool for differential diagnosis of ABP. Although sphincter of Oddi manometry (SOM) is performed for the confirmative diagnosis of sphincter of Oddi dysfunction (SOD), several non-invasive tests have been studied because of some practical limitations and invasiveness of SOM itself. In fact, the most clinically used easy test to diagnose functional biliary disorder is quantitative hepatobiliary scintigraphy and it can distinguish gallbladder dyskinesia, SOD, or combined type. Initial treatment of functional biliary disorder is adequate dietary control and medication, but if the symptoms worsened or recurred frequently, laparoscopic cholecystectomy could be performed with gallbladder dyskinesia. If SOD is suspected, additional SOM should be considered and endoscopic sphincterotomy (EST) can be done according to the outcome. If the SOM is not available, the patient could be diagnosed by stimulated ultrasound.
