We demonstrate a novel augmented reality tele-proctoring platform to enable a remote attending to guide a fellow through neuro-interventions.MethodsA consecutive series of neuro-endovascular cases were performed by a first-year neuro-endovascular fellow with remote guidance from an attending surgeon. The fellow and attending were connected using the Proximie platform, a cloud-based solution that enables users to capture and stream up to four live video feeds from a clinical environment to a remote user. In this setting, two video streams were obtained directly from the AP and lateral cameras on the biplane. Videos were streamed live to the remote attending surgeon providing them situational awareness. The attending could provide immediate vocal feedback to the fellow over an intercom. The attending could also deploy Proximie’s augmented reality tools which were visible to the fellow in real-time on a laptop in the angiography suite.ResultsA total of ten cases were performed on eight patients during the study period. 30% of participants were female and the median patient age was 63±18.6 yrs. Six interventions and four diagnostics were performed in the series. All cases were a technical success. No complications or deaths occurred.ConclusionsThis study demonstrates successful use of an augmented reality tele-proctoring platform in a fellowship training paradigm.ReferencesBechstein M, Elsheik S, Wodarg F, et al. Seven cases of in-stent-stenosis (12,3%; morbidity n=0) were detected on long-term follow-up with 6 of them when using the kissing-Y stenting technique. Conclusions Endovascular treatment of various complex intracranial aneurysms using the Acandis Acclino stent systems is safe and efficient with high aneurysm occlusion rates combined with low complication rates at long-term follow-up. Circulatory failure and severe intracranial hypertension prior to ECI, or within 3 days from bleeding if no ECI, were retrospectively determined. The association between ECI, prior circulatory failure, severe intracranial hypertension and patient outcomes was tested using uniand multivariate analyses. Results Seven-hundred-and-fifty-three patients with aSAH were included. ECI were observed in 40 patients with a prevalence of 5.3% (95% CI; 3.7–6.9%). New ECI lesions developed in-hospital in 70% of cases. Circulatory failure or severe intracranial hypertension was more common in patients with ECI compared to those without ECI (90% vs.11% respectively <0.001). In ECI patients, in-hospital occurrence of circulatory failure or severe intracranial hypertension was observed in 60% of cases, and was significantly associated new in-hospital ECI lesions (71%vs.33% in patients without new in-hospital ECI lesions, P=0.036).
