At the same time, it is the most well- known malignant tumor. The word \"melanoma\" is derived from the Greek melas, melanos means “dark, black.” However, there is also an amelanotic type of melanoma. Melanoma is associated with skin disorders, but can also occur in multiple organs and tissues: the choroid and iris, urogenital tract (vulva, vagina, uterine cervix, penis), mucous membrane of the gastrointestinal tract, meninges, lungs, etc. This is related to the fact that all these organs have melanocytes (pigment-producing cells) from which melanoma develops. Vaginal melanoma or \"mucosal melanoma\" is a very rare and aggressive neoplasm accounting for less than 10% of malignant tumors of female genital organs and 0.3% of all melanoma cases. Clinical and pathological characteristics of this tumor are understudied, diagnostic tools are scarce, and an optimal method of treatment has not been developed. Therefore, it is important to share experience and clinical reports on this disease. The authors present a rare and complicated case of the disease — achromatic vaginal melanoma. Patients and Methods: this prospective study was conducted from January 2016 to September 2022 and involved 178 women with monochorionic diamniotic twins in the first trimester (11+0–14+0 weeks) who underwent ultrasound once every 2 weeks. Group 1 included women who were later diagnosed with TTTS (n=68). Group 2 included women without TTTS (n=110). Results: in women who later developed TTTS, a significant increase in the discordance of the nuchal translucency (NT) thickness (p=0.036), increase in the pulsatile index (PI) of blood flow in the ductus venosus in one fetus higher than normative (p=0.036), a significant difference in the difference in the crown-rump length (CRL) (p=0.046), large arterial anastomosis in the placenta with predominant signal on one side of the isoline, and chorion previa (CP) (p=0.012) were reported. The difference in isovolumic relaxation time (IVRT) in the first trimester was more than 9 (6–11) ms in the TTTS group and 5 (4–9) ms in the non-TTTS group. However, this difference was insignificant (p=0.06). A prognostic model of TTTS was built based on ultrasound signs (NT thickness, CRL, IVRT, ductus venosus PI) using logistic regression. Conclusion: our study revealed risk factors for TTTS at the end of the first trimester in monochorionic twins. However, these risk factors require further evaluation in a large prospective case series of monochorionic twins. A method for predicting severe complications of monochorionic twins during early pregnancy provides an additional option for stratifying patients. {"title":"Gastrointestinal tract in children with novel coronavirus infection and post-COVID-19 syndrome. Many observations demonstrate that SARS- CoV-2 affects the gastrointestinal tract, gut microbiota composition, and intestinal permeability. Aim: to assess whether Maxilac® Baby synbiotic effects qualitative and quantitative composition of gut microbiota and intestinal permeability in children aged 3–14 with mild-to-moderate COVID-19. Patients and Methods: an open, observational, prospective, single-center study with minimal intervention was conducted. Group 1 included 16 children who received Maxilac® Baby 4 weeks after recovery. Group 2 children did not receive synbiotics after recovery. History and complaints were assessed, gut microbiome composition was determined, and fecal zonulin was measured at disease onset, time of recovery (day 14), and 4 weeks after recovery. Results: at disease onset, abdominal pain and diarrhea were reported in 16 children (50%), nausea in 8 children (25%), and vomiting in 1 child (3.1%). At the time of recovery, only abdominal pain was reported in both groups (4 patients each). Thirty days after recovery, re-appearance of intestinal symptoms was reported in group 2. No significant differences in the microbiome diversity profile at birth level were revealed between children with the COVID-19 at onset and healthy age-matched children. Taxa (species) whose predominance was typical for each group were identified. Conclusions: post-COVID-19 syndrome in children aged 3–14 diagnosed with mild-to-moderate disease who received Maxilac® Baby for 1 month after recovery is characterized by the lack of nausea, vomiting, and diarrhea, less abdominal pain, no changes in fecal zonulin levels, and biodiversity of gut microbiome, including those assessed by Shannon diversity index. Patients and Methods: the study included 42 female patients of the reproductive age who were planning pregnancy in the nearest future. The main group consisted of 24 female patients who took a drug containing omega-3 PUFA 585 mg for the pregravidar preparation. The control group included 18 female patients who took drugs containing folic acid 400 μg in a daily dose, iodine 200 μg, vitamin D 2000 U. The follow-up period of female patients and results control of the laboratory parameters during the treatment was 3 months. Then, the follow-up was based on the results of the reproductive performance implementation in a year. Results: during the therapy, omega-3 PUFA intake leaded to a slight increase of the fatty acids (FA) content, i.e., to the normalization of this indicator, which didn't not require constant control of the FA level and did not limit the time of use. Besides, when omega-3 PUFA was prescribed, the frequency of inevitable miscarriage might decrease (OR 2.6, 95% CI 1.2–14.7), however, this statement requires further study and confirmation in a larger number of female patients. Conclusion: the additional prescription of omega-3 PUFA for the pregravidar preparation has a positive effect. At the same time, routine determination of the fatty acids level and its control during the intake is not required before the prescription. The intake of omega-3 PUFA during pregnancy and in the early neonatal period is important for the subsequent normal functioning of the child's body. The article summarizes data on the biological role and functions of calcium in children's body, elucidates the physiology of calcium metabolism, and highlights the major exogenous and endogenous causes of calcipenia development and the clinical symptoms of Ca deficiency at the early stages. In view of a high prevalence of the calcium deficiency worldwide, the authors review potential outcomes of this condition, depending on the children's age. The article discusses the opportunities for preventive measures with an emphasis on primary specific and nonspecific prevention of calcium deficiency to be achieved by making changes in diet and lifestyle and using vitamin and mineral complexes (VMC) and dietary supplements. Thus, it would be possible to meet the physiological needs for calcium in children and to manage the cases of developed calcium deficiency. The conference brought together specialists from multiple Russian regions. The development of effective treatment algorithms for infection-caused inflammatory diseases in the practices of obstetricians and gynecologists remains relevant in view of the continuous growth of antimicrobial resistance underpinned both by the irrational use of antibiotics and the biofilm-forming ability of infectious agents. The speakers shared their experience of using systemic enzyme therapy (SET) in the combination treatment of inflammatory diseases in obstetric and gynecological practice, as well as in the prevention of fibrous and adhesive complications of pelvic inflammatory diseases (PID) and gynecologic surgeries. It was pointed out that systemic enzyme therapy should be recognized as a justified component of the combination PID treatment which helps to reduce the duration and severity of symptoms due to its proven immunomodulatory, fibrinolytic and antiplatelet effects. The use of SET drugs reduces the duration of all stages of reparative regeneration along with tamping down the inflammatory response. The Expert Council Resolution provides recommendations on using SET in gynecology. Patients and Methods: the study included 510 children (2-6 years old) attending kindergartens in Khanty-Mansiysk city. The children were divided into 2 groups: vaccinated (n=50) and non-vaccinated against pneumococcal infection (n=460). The data on vaccination status were received from the prophylactic immunization records (Form 063/u) and child's medical records (Form 112/u). In addition, the following data were analysed: age, gender, gestation age at birth, attitudes of child's legal representatives towards vaccinations, and the presence of chronic health problems. Pneumococcal polysaccharide conjugate adsorbed 13-valent vaccine (PCV-13) was used for the prevention of pneumococcal disease. Nasopharyngeal swabs were collected from children as a source of biological material. The pneumococcus was detected using a culture-based method and PCR assay. Results: the average age of organized children examined in Khanty-Mansiysk for Streptococcus pneumoniae (S. pneumoniae, pneumococcus) was 54.31 [53.46–55.16] months. In 56% of the examined children, pneumococcal DNA was found in nasopharyngeal swabs by polymerase chain reaction assay. The identified serotypes in 71.1% of cases were the same as in the PCV13 vaccine formulation. The most common serotypes detected were 4 (27%), 19 F (13.3%), 23 F (3.6%) and serogroup 6 (11.6%). In children immunized against S. рneumoniae, the pneumococcal strain DNAs, same as the vaccine strain DNAs, were found in 53.7% of cases, while in non-immunized children — in 73% of cases (OR 0.54, 95% CI 0.29–0.97). Conclusion: the increasing prevalence of S. рneumoniae carriage in children as they get older and attend organized children's groups for a longer time, along with the predominance within the population of the same pneumococcus strains as those in the vaccine formulation, dictate the need for timely immunization against pneumococcal infection in compliance with the National Vaccination Schedule (vaccination at 2 and 4.5 months of age, revaccination at 15 months of age). Congenital CMVI is quite well studied, always causes alertness of neonatologists, and has a diverse clinical picture after the birth of a child. Certain difficulties in diagnosis are noted in children who have postnatal CMVI. As a rule, the infection is asymptomatic or slightly symptomatic and has no special serious clinical consequences, unlike congenital CMVI, which causes hearing loss and central nervous system damage. It is known that postnatal infection is dangerous for children with impaired immune system. This risk group consists of preterm infants, mainly with very low and extremely low body weight, and full-term infants with severe concomitant congenital and/or acquired pathology. The question of infecting ways for children with postnatal CMVI remains debatable. The article also discusses the various clinical manifestations of postnatal CMVI, which can occur according to the type of CMV-related sepsis-like syndrome, hepatitis, diarrhea, neutropenia. The article highlights the importance of clear laboratory diagnostic criteria during establishing the diagnosis. There are always a lot of questions from the doctor regarding the prescription of specific treatment in CMVI and prevention methods of postnatal CMVI in full-term infants. This article also presents clinical case of postnatal CMVI in full-term infants with combined pathology receiving treatment in the conditions of the Perinatal center of St. Petersburg State Pediatric Medical University. Demchenko","doi":"10.32364/2618-8430-2023-6-3-13","DOIUrl":"https://doi.org/10.32364/2618-8430-2023-6-3-13","url":null,"abstract":"This article summarizes modern scientific data on the efficacy and safety of prescribing physical activity (PA) to children and adolescents with congenital heart defects (CHD), as well as practical recommendations for actual amount of PA in this category of patients, depending on their initial functional status. The preferences and benefits of PA with controlled amount, rather than its limitation in patients with CHD, have been demonstrated. An individual exercise regime can safely be indicated to each patient with CHD according to the team approach and thorough assessment. However, there remains a large gap in the literature regarding recommendations for PA in children with CHD, despite the fact that this research area is one of the priorities worldwide, since PA is a powerful tool in optimizing the health of children with CHD. There are only a limited number of publications with a small sample size, which indicates the need for further research to assess the PA contribution to the recovery of children with CHD, especially with combined defects. Semenova","doi":"10.32364/2618-8430-2023-6-1-45-49","DOIUrl":"https://doi.org/10.32364/2618-8430-2023-6-1-45-49","url":null,"abstract":"Aim: to assess the role of autonomic disorders and undifferentiated connective tissue dysplasia (UCTD) of the heart in children with ventricular extrasystoles (VE) engaged in sports. Patients and Methods: the study included 52 athlete boys with newly diagnosed cardiac arrhythmia in the form of ventricular extrasystoles. The study group (n=34) consisted of children with moderate VE severity co-existing with asympaticotonic type of autonomic dysfunction syndrome (ADS) amid undifferentiated connective tissue dysplasia mostly affecting the heart (UCTDH). The study group was split into two subgroups: subgroup 1 (n=11) – children with monotopic VE persisting at load and subgroup 2 (n=23) – children with occasionally appearing VE which reduced in the vertical position and/or at load. The control group (n=18) consisted of children with moderate VE amid UCTDH without ADS manifestations. A comprehensive clinical and laboratory assessment was performed in all patients. Results: on Holter monitoring (HM), in patients of the study group, subgroup 1, a sinus rhythm was recorded with the presence of 9,000- 15,000 VE per day (single or paired). The power spectral analysis of heart rate variability (HRV) in the subgroup 1 children demonstrated that in most cases (10 (90.9%)) there were low levels of the total power (TP) (р<0.05) and high values of the high-frequency component (HF) (p<0.05) as compared to those in the control group children. Also, a significant decrease in creatine phosphokinase-MB activity in the blood serum was found in children of the study group, subgroup 1, as compared to the reference values (р<0.05). The VE development in the study group, subgroup1, children was caused by the combined exposure to sport loads and the autonomic control of cardiovascular function amid UCTDH. In the study group, subgroup 2, children the laboratory testing did not demonstrate any statistically significant changes in creatine phosphokinase-MB activity in the blood serum as compared to the reference values (р>0.05). On HM, in most of the subgroup 2 children, a sinus rhythm was recorded with the presence of 6,000-9,000 VE per day. The changes in HRV parameters and their spectrogram in 19 (82.6%) cases were common for asympaticotonic type. As compared to the values in the control group children, TP was slightly lower (р>0.05) along with a high HF level (р>0.05). A half of the control group children had a moderate increase in creatine phosphokinase-MB activity in the blood serum (р<0.05), while in other cases its values did not differ considerably from the normal range. On HM, in most cases a normal sinus rhythm with usually single EV, 300-5,000 per day, was recorded. The main measurements of the power spectral analysis of HRV and spectrogram indicators suggested that children of this group had sufficient body energy supply. The presence of VE in the control group children was associated with the impact of sport loads on the heart with dysplasia.
