Case: After conformation of the diagnosis, parents decided not to reveal the condition’s specifics to the patient. At the age of 34 years, she attended our hospital with abdominal swelling, and the following CT scan showed an abdominal mass arising from the ovary of 13 cm x 14 cm size. A laparotomy was done, and a mixture of solid and cystic mass of 20 cm in diameter was found and removed with the ovary. Histopathology of the tumor showed features of testicular seminoma. Modern animal production practices involve regular use of antimicrobials, but abusive use can lead to harmful consequences. The aim of this review was to provide comprehensive information regarding the risk of regular application of antibiotics in food animals, the potential consequences and impact on human health, and options for the prevention of antibiotic resistance in the food chain. Review Results: Although organizations such as WHO, UN, and EU have taken steps to reduce and limit the use of antibiotics in animals, these are proving insufficient in developing countries. In many countries, the antibiotics used in animals appear to exceed that in humans. The abuse in food animals promotes the development of antibiotic-resistant bacteria and resistance genes that can be transmitted to humans through food consumption and direct contact with animal food or environment mechanisms. This can lead to human infections with antibiotic-resistant bacteria, causing treatment failure and, therefore, more severe and longer-lasting illnesses, increased hospitalization rates, more deaths and higher costs to society. Furthermore, as food animals and animal-derived foods are traded globally, they contribute to antibiotic resistance in countries far from where the problem originates. Conclusion: Overcoming the problem of antibiotic resistance requires a multidisciplinary approach with effective action coordination and information exchange between the agricultural, food, veterinary and health sectors. {"title":"STRESS AND HEALTH BEHAVIOR OF HEALTH PROFESSIONS STUDENTS IN BULGARIA","authors":"Petya Boncheva, Klara Dokova","doi":"10.5272/jimab.2024302.5500","DOIUrl":"https://doi.org/10.5272/jimab.2024302.5500","url":null,"abstract":"The purpose of this study is to determine the prevalence of stress among students of various health specialties in Bulgaria and to examine its relationship with the individual and institutional characteristics of the students and with their health behavior. Methods: A survey was conducted among students from all 26 Bachelor and Master of health sciences programmes at a Medical University in Bulgaria. Information on self-assessed levels of health and stress, aspects of the health behavior (physical activity, smoking, alcohol intake, diet and sleep duration), personal (age, gender) and institutional factors (specialty and year of study) was collected with a structured questionnaire. Multinomial logistic regression was performed to evaluate the determinants of the perceived high level of stress. Results: A total of 570 (10% sample) of all students from the university with a mean age of 24 (SD 7.6) years took part in the study. The majority of students (61.7%) experienced a moderate level of stress. Self-reported high level of stress (25.4%) was significantly independently associated with three factors - female gender (OR=2.92, 95% CI=1.09-7.81), low level of self-rated health (OR=6.95, 95% CI=2.17-22.25) and higher (fourth or fifth) year of study compared to first (OR=5.49, 95% CI=1.72-17.59; OR=4.06, 95% CI=1.36-12.16). Conclusions: Stress is an important problem affecting the health of this group of the population. Morphologically, GC is a heterogeneous neoplasm consisting of several histological types. Majority of the patients are diagnosed in an advance stage of the disease. Conventional surgical and systemic treatment has demonstrated limited achievement. Evolution in personalized therapy is a main factor to improve the outcome, but for a successful application, a careful stratification of patients is required. Aim: The purpose of the present paper was to analyze the mandatory clinical and pathological characteristics of patients with GC. Material and methods: A total 74 patients who have undergone total or partial gastric resection, endoscopic and laparoscopic biopsy due to GC were included. Results: The average age of the patients was 69,6 years, with a male predominance in all age groups. Most frequent histological type was intestinal type, according to Lauren and tubular type, according to WHO classifications. 82,3% (p<0,05) of GC located in cardia were diagnosed in males. 37/42 tubular, papillary and mucinous types by WHO classification were classified as intestinal type by Lauren. All signet ring cell and poorly cohesive adenocarcinomas by WHO classification were accordingly assigned as diffuse type by Lauren. 52,2% of the tubular/intestinal type of tumors were graded as G1 or G2, and 93,1% of signet ring and poorly cohesive/diffuse type GC were graded as G3. Significant correlations were observed between perineural invasion and histological type by Lauren (p<0,043), degree of differentiation (p<0,04) and lymphovascular invasion (p<0,000); between а degree of inflammatory reaction and М status (p<0,022). Conclusion: Precise histological typification of GC is a key to better stratification of the patients. Certain histological types are associated with aggressive tumor behavior, as we found diffuse type to correlate with poor differentiation and presence of perineural invasion. The literature describes various operative techniques for the treatment of rhizarthrosis, but the outcomes of the effectiveness of RegJoint™ implant are still scarce. Purpose: The aim of this study is to determine the effectiveness of using the RegJoint™ implant in the operative treatment of rhizarthrosis and the subsequent recovery. Material and methods: In the period 2019-2023, a total of 48 patients diagnosed with rhizarthrosis (46 women and 2 men) of average age 61.4 (±3.2) years went for treatment through the clinical bases of UМHAT \"Kaneff\" - Ruse and UMHAT \"SOFIAMED\"- Sofia. All of them were operated with the bioabsorbable implant - RegJoint™, after which the thumb was immobilized using a fixation splint for 6 weeks. Their average hospital stay was 3.2 (±1.3) days, after which they underwent rehabilitation procedures for 6 months. Results: Functional capacity was assessed using the DASH test, and thumb movements were tested using the Kapanji score three times - preoperatively and at the end of the 6th and 12th postoperative months. The results at the end of 6 months and 1 year period show a significant improvement in terms of pain, swelling and functional status of the operated hand. {"title":"INVASIVE CERVICAL RESORPTION OF UNCOMMON ORIGIN: A CASE REPORT WITH 14-YEAR FOLLOW-UP","authors":"Y. Its occurrence is associated mainly with local predisposing factors, but systemic risk factors may be involved as well. An ICR lesion that develops in the absence of an identifiable cause is termed 'idiopathic'. Case report: This study presents the surgical management and very long-term follow-up results of an \"idiopathic\" Class 3 ICR lesion in the maxillary central incisor. The case provides a unique insight into an uncommon ICR lesion that exhibited an unusual appearance and features. For the repair material, a sandwich technique was applied using mineral trioxide aggregate (MTA) covered with glass ionomer cement (GIC). At the 14-year follow-up, the tooth was symptom-free and functional; however, the disadvantages of both materials were clearly demonstrated. From that perspective, the study highlighted the discoloration potential of the white form of MTA-Angelus and the lack of regenerative properties of the GIC used. Remarkably, 12 years after the surgery, the patient was diagnosed with Familial Mediterranean fever, a rare systemic disease. However, the connection between this genetic disease and the initiation of ICR remains an open question for now. Conclusion: The clinician should be alert for the occurrence of ICR lesions, especially in patients with concomitant systemic disorders. Material/Methods: A “Distress Thermometer” was used to collect information from the patient to assess the degree of distress. Inclusion and exclusion criteria for the patients were applied. Methods for statistical processing of the data and interpretation of the results were used. Results: The study included 225 patients with an average age of 59.6 (+/- 11.4 years). Women are a little over half, 56% (n=126) of the total number of those examined. All patients have a histologically confirmed oncological diagnosis in stage II, III and IV. It was found that there was no relationship between the level of distress and childcare (p= 0.93). In a more in-depth analysis of the results, a significantly higher level of distress was found in patients who encountered difficulties with transportation to the medical facility (p=0.01). A significantly higher level of distress was reported in patients who marked the relationship with their partner as problematic (n=13, 5.8%) (4.7±3.9). Conclusion: The diagnosis and treatment of cancer have more than just a physical impact. Health care should be comprehensive based on the needs of the patient. The device is designed to assess the effect of occlusion on muscle balance and thus indirectly guides the clinician to the necessary changes in occlusion. This paper aims to determine whether muscles influence the indices characterizing occlusion. Material and methods: Thirty-three persons aged 13-43 (mean 21.33) years, 15 men and 18 women who fulfilled the criteria, were included in the study. All subjects had a good state of dental and periodontal health and complete permanent dentition. EMG recordings of the masseter and temporalis muscles were performed during maximum voluntary teeth clenching on cotton rolls and in centric occlusion. All values were standardized as a percentage of the maximum clenching on cotton rolls. Results: Most of the results of the test on cotton rolls are below normal reference values provided by the manufacturer. Within the normal range are only the Global index and IMPACT. When compared to MVC in centric occlusion, some of the indices were higher than when biting on cotton rolls (POC MM, BAR, Global index), which pointed out the effect of the occlusal contact on these tests. Conclusions: The calibration of the Teethan® device gives reliable and reproducible results. Test during MVC in centric occlusion will provide information on which of the indices are influenced by occlusal factors. Material/Methods: We report the clinical course of a 68-year-old woman who sought medical attention regarding a locally advanced breast cancer characterized by exophytic growth. Results: Despite initial systemic treatment, the tumor displayed rapid progression and extensive ulceration with spontaneous bleeding, prompting additional cycles of systemic and radiotherapy. Subsequently, the patient experienced life-threatening acute breast bleeding and hemorrhagic shock, leading to an admission to the Intensive care unit, followed by an emergent modified radical mastectomy. Histological examination of the specimen revealed a high-grade sarcomatoid variant of squamous cell metaplastic breast carcinoma (MBCa) without lymph node (LN) metastasis. Adjuvant radiation therapy was performed, yet a local recurrence was diagnosed, necessitating a second surgical intervention. Despite also receiving adjuvant systemic therapy, the patient presented with a second recurrence with distant metastases in the lungs. A transcatheter arterial embolization (TAE) of the tumor-supplying artery was performed, aiming to impede the tumor vasculature, thus reducing the risk of recurrent life-threatening hemorrhage. However, only a short-term effect was observed, and the patient experienced an unfavorable outcome. Presenting with discomfort and lesions under her decade-long worn partial denture, her condition was indicative of inflammatory papillary hyperplasia and fungal infection. Management involved a combination of systemic and local antifungal treatments, diode laser surgical removal of nodular lesions, fabrication of a new denture, and extensive oral hygiene education. Over a two-year period, a significant decrease in Candida levels was observed, though the persistence of the fungus necessitated ongoing monitoring.
