The usage of transdermal therapeutic systems has substantially driven this growth. The analysis was based on claims data of a German statutory health insurance (2001-2003). Statistical analysis applied univariate comparisons (exploratively only) as well as a multivariate logistic regression models. Patients in the transdermal group were older and the percentage of women was higher than in the oral group. Patients in the transdermal group received their opioids significantly more often from a GP. They had significantly less prescriptions for laxatives and antidepressants. The patients in both groups differed significantly with regard to a number of characteristics. The patient's agreement is therefore necessary for every elective treatment. Patients cannot give meaningful consent until they have received detailed information on their illness, its presumed course, the treatment options and the risks they are exposed to both with and without treatment. This requires provision of information to allow self-determination. The requirement for the patient alone to make decisions arises from the personal rights in our [German] Constitution. If the information provided has been inadequate the doctor is liable for all negative-including life-changing-consequences, regardless of whether or not the risk that has been realised is one that is expressly required to be explained. The Federal Court of Justice requires the doctor to address each patient's personality and their fate when providing information (BGH, NHW 1983, 328, 329). A review of the literature reveals that studies based on large numbers of patients, long-term observations and controlled randomised trials are still lacking. In 2006 a case report of the first patient in Germany to be operated on to allow DBS was published, and we now present a report of this patient's course in the first 6 months after the operation; in addition, a current literature review is discussed. In July 2005 a DBS lead was placed in the left posterior hypothalamic area of this 39-year-old woman with CCH. Stimulation on demand achieved complete suppression of the cluster attacks, and the patient no longer needed medication. After about 8 months a decreasing effect of the stimulation, with only about 50% reduction of cluster attacks, and stimulation-induced side effects were observed. Neither reprogramming of the stimulation parameters nor pharmacological therapy with on-demand and long-term medication reduced the frequency or severity of CCH attacks to the level experienced in the early postoperative stage. Because of intolerable subjective side effects and tension-related pain at the site of the connection cable, in September 2006 the whole system was explanted at the patient's request.DBS in the posterior hypothalamic area is an invasive treatment option for use in cases with CCH that is refractory to any pharmacological therapy. As demonstrated by this case report, it is not possible to give a prognosis concerning its long-term efficacy: despite the initial excellent benefit there can be a reduction and even a loss of the effect of stimulation. The goal of the present pilot study was to evaluate the feasibility of standardized telephone interviews in patients with refractory pain who had been switched to oxycodone in the setting of daily practice.</p><p><strong>Patients and methods: </strong>Once a week for 3 consecutive weeks, 33 female and male patients answered a standardized telephone interview derived from the ICF (International Classification of Functioning) about the impact of pain on their daily activities.</p><p><strong>Results: </strong>After 3 weeks, complete datasets were available for 70% of all participants. With flawless technicalities, the following aspects appeared to be worth improving: non-uniform response categories, limited language choice, and handling instructions to physicians and patients. The efficacy (pain relief and improved limitations in the activities of daily living) and good tolerability of oxycodone were comparable with findings reported in clinical trials. More than 95% of patients accepted the anonymous storage of their data for quality control purposes.</p><p><strong>Conclusion: </strong>Standardized telephone interviews aimed at documenting the evolution of pain are feasible in a daily practice setting. It is probable that 11% of the world's population suffer from migraine. Some recent studies have investigated the prevalence of headache in neurologists and other medical doctors; most of these studies have been performed in the USA. One study in Taiwan investigates how migraine is treated in Taiwanese neurologists. Within Europe, two small observational studies conducted in Italy and Spain are available. A study in Germany analyses the attitudes of headache specialists and neurologists who themselves suffer from migraine to migraine treatment and to the whole concept of migraine. Glossopharyngeal neuralgia is much less common than trigeminal neuralgia and is not well known. Idiopathic neuralgia of the glossopharyngeal nerve sometimes occurs in association with neurovascular compression syndrome of the vagus and trigeminal nerves. The prevalence of migraine is 8% in men and 12-15% in women. Dramatic progress in the areas of epidemiology, pathophysiology, and acute and preventive therapy of migraine has been made over the past 100 years, with triptans being the breakthrough for treating acute migraine attacks. Beta blockers, calcium antagonists, and neuromodulators are available for preventive migraine therapy. Nonpharmacologic treatment also plays an important role in migraine prevention. The questionnaire used in this study was modified from one used in a survey carried out in Germany in 2002. In our version we asked specifically about pain in the musculoskeletal system. In all 228 health care facilities were reviewed, 56.6% of which offer at least one option for pain therapy. In Austria, the majority of patients with pain in the musculoskeletal system are treated by specialists in the departments of anaesthesiology, internal medicine, and orthopaedics. In 17.4% of the clinics in Austria there are plans to extend the pain therapy they offer, but in over half of the hospitals that responded facilities for pain therapy are considered to be vulnerable. The study highlights a significant higher percentage of in-patient pain therapy services in Austria, while in Germany, in contrast, there are more outpatient options for pain therapy.
