Haque","doi":"10.3329/jss.v20i2.43812","DOIUrl":"https://doi.org/10.3329/jss.v20i2.43812","url":null,"abstract":"Background: Carcinoma of the breast is one of the most common malignancies in women worldwide. \u0000Objective: The current study was conducted to evaluate the role of Ki-67 as a prognosticmarker in two definite groups of breast cancer patients (ER+ve, PR+ve, HER-ve& triple negative)in Bangladesh perspective. \u0000Methods: Sixty nine female breast cancer patients operated at the surgical oncology departmentof National Institute of Cancer Research & Hospital were selected by non-probabilitysampling method and operated specimens were sent for immunohistochemical study of theER, PR, Her2/neu receptors and Ki-67 protein analysis. Statistical analysis was conductedusing SPSS version 17 for Windows software. P-value 0.05 or less was considered as significant. \u0000Result: The mean age of the patients was 46.96 years with SD of± 13.13 years. Histopathologyreports revealed that 94.2% (65/69) were suffering from duct cell carcinoma (DCC) whilelobular varieties were found in 2 cases. Majority of the patients with ER+, PR+ and Ki-67 +vestatus were between 36-50 years of age. But for Her2/neu positive cases most of patientswere above 50 years of age. In Luminal A category cancer patients 69.7% showed positiveKi-67 expression but in case of triple negative cases this percentage was 87.5%. In developed world about 10% of hospital deathsare due to pulmonary embolism resulting from deep vein thrombosis( DVT). DVT in Asiancountries is not a rare condition. \u0000Objective: The purpose of the study was to find the incidence ,demgraophic characterand risk factors of deep vein thrombosis. \u0000Methods: 150patients undergoing emergency or elective operation of more than one hourof duration was selected for study. Patients evaluated by history, clinical examination andduplex scan. Collected analyzed by computer program. \u0000Results: 102 cases were male and 48 cases were female. The incidence of DVT in malewas 36.3% while among females it was 27.5%. venous thrombosis in age group above 50yrs is quite high, (43.33%) . One-fifth of the DVT patients were smokers. Longer duration ofoperation procedure was correlated with development of DVT and development of DVT waslittle higher in emergency operation than elective one. The occurrence of the condition insome co-morbid states especially diabetes mellitus (36. 7%) and cancer (33.3%) is noteworthy.Out of 51 DVT patients more than 84% developed sub-clinical DVT. \u0000Conclusion: In Bangladesh it is assumed that DVT is rare. GISTsusually present with abdominal mass and GI bleeding but it may also present with manyunusual pattern like obstruction, perforation. Perforation of GISTs is extremely rare presentation.We report a case of pneumoperitoneum due to perforation of small intestinal GIST.Emergency laparotomy followed by resection and end to end anastomosis was done. Aftersurgical treatment lmatinibmesylate was given for prevention of early recurrence. The development of endourological and extracorporeal lithotripsy techniques ledto an increasing number of options for the management of renal calculi. \u0000Objective: To define factors those have a significant impact on the success rate after extracorporealshock wave lithotripsy (ESWL) for treatment of renal stones. \u0000Materials and Methods: Between April 2008 & December 2008, 64 patients with single ormultiple radiopaque renal stones (::s;30mm) were treated with ESWL monotherapy using StortzModulith SLX-F2 lithotriptor.The results of treatment were evaluated after 3 months of followupTreatment success was defined as complete clearance of the stones or presence ofclinically insignificant residual fragments (::s;4mm).The results of treatment were correlated withthe patient characteristics (age, sex, body mass index) and stone features (size, site, nature &radio density). \u0000Results: At 3-months follow-up, the overall success rate is 76%. Among them, repeated ESWLsessions are required in 19 patients (53.9%). Post-ESWL complications are recorded in 8patients (12.5%). Using the x2 test, only three factors have a significant impact on the successrate, namely: stone site, size (the largest diameter of the stone), stone number & BMI (BodyMass Index). The success rate is highest for stones located in the upper calyx (15/15; 100%)and lowest for those located in the lower calyx (10/16; 62.5%) (p=.005). Stones with a largestdiameter ot s 15mm are associated with a success rate of 90.2% (37 /41), compared to 52.2%(12/23) for those with a diameter of >15mm (p=.001). Surgery and anesthesia procedures with a special focus on acute surgical conditions management have been investigated and/or performed on animal models and astronauts/cosmonauts. Although total anesthesia has been successfully performed on animal models it is considered of high risk for humans and regional anesthesia techniques have been the method of choice so far. This analysis would contribute to a better understanding and for identification of those characteristics that are essential for reducing emergency colorectal cancer incidence and for improving the management of such cases. A number of 159 patients that underwent surgery for obstructive, bleeding or perforated colorectal cancer between January 2015 and September 2017 were selected to be included in the study.
