Her appliances respect rigid scientific fundamentals and provide for the clinician special tools for solving different types of occlusopathies. Clinical case report","authors":"Elba Zambrano de Carrasquero","doi":"10.21595/jfocg.2022.22794","DOIUrl":"https://doi.org/10.21595/jfocg.2022.22794","url":null,"abstract":"X-rays and tomography, in their different modalities, are used as auxiliary diagnostic elements in temporomandibular joint (TMJ) dysfunction, both in static and dynamic situations; however, because they only allow the observation of bone structures, magnetic resonance imaging (MRI) is the ideal method to analyze, among other structures, the articular disc. Among the internal disorders of the TMJ are the abnormal postural relationships between the disc, mandibular head, fossa and articular tubercle, which can even lead to a musculoskeletal condition causing noise, pain and functional limitation. The clinical evaluation of temporomandibular dysfunctions is complex, especially in asymptomatic patients; the Diagnostic Criteria for Temporomandibular Disorders (RDC/TMD), place these entities within the AXIS I protocol, considering that the image is not strictly necessary. However, analyzing the disc situation statically and dynamically, with its surroundings, allows considering the loads that generate dysfunction, leading to the correct diagnosis, and selection of the best treatment. A clinical case is presented, an asymptomatic patient, without functional limitation, who reported a history of occasional mouth opening locking upon awakening. Although robust evidence indicates that these abnormal anatomic variations are associated with breastfeeding difficulties, impaired craniofacial growth, sleep, speech and posture in children, both diagnosis and treatment of oral ties remain controversial. The oral cavity displays considerable morphological variation across individuals. One of these variations includes tight, restrictive connective tissue between oral structures known as tethered oral tie tissues (TOTs). The clinical view regarding these anomalies has evolved with increasing interest not only in tongue-tie (ankyloglossia) but also in lip-tie [1-3]. Ankyloglossia has been considered a risk factor for breastfeeding difficulties [4-16]. Recent evidence indicates that TOTs can be also associated with whole-body consequences, such as reflux, dental malocclusion, and respiratory disorders, ultimately increasing the risk of sleep and speech disorders, and detrimental changes in posture and eating patterns [13, 17-25]. The prevalence of TOTs is highly variable across populations and is still a matter of ample debate. Currently, there is a lack of consensus on diagnosis criteria, best surgical treatment techniques, and pre- and post-surgery care [19, 26, 27]. Yet, the diagnosis and surgical treatment of TOTs have substantially increased in recent years [28-31]. This mini-review will summarize evidence-based data regarding the cascade of consequences of tongue-tie and lip-tie in children and the main signs and symptoms of these anomalies in newborns. It will also discuss the available evidence on treatment options for TOTs, including pre- and post-surgical care that may enable better outcomes and prevention of possible complications. Among the probable causes that have been raised are biochemical alterations in the central and/or peripheral nervous system, which can be seen reflected in an alteration of the rhythmic movements of the masticatory musculature of the stomatognathic system. On the other hand, chewing is recognized as one of the main functions of the stomatognathic system, responsible for maxillofacial growth and determining a rhythm in the movements of the masticatory musculature, depending in turn on a functional demand of the individual, in other words, the type of food consumed and the way to process it in the mouth. Objective: To determine the existence of a relationship between bruxism and the type of masticatory pattern installed in the patient. Methodology: Functional examinations and application of Fonseca questionnaires and bruxism self-report were performed in 27 adult patients, without distinction of sex, who have been diagnosed with possible and probable bruxism. Results: 100 % of the sample of subjects diagnosed with bruxism presented an altered masticatory pattern. Conclusions: The high correlation found in the sample between bruxism and masticatory pattern suggests that it is vital for the success of a bruxism treatment or a DTM to consider the way of processing food, by educating the patient with Masticatory Orientation. Its etiology is multifactorial and may involve several etiological factors, such as deleterious oral habits, hypertrophic tonsils, mouth breathing, dental ankylosis and eruption abnormalities either associated with genetic factors or not. AOB is one of the most challenging treatments in any treatment protocol. A retrospective study of 21 patients with anterior open bite treated with Jaw Functional Orthopedics (JFO) with improvement in overbite was conducted with the aim of understanding mandibular behavior during the treatment. Using SPSS 11.0, using Shapiro-Wilk test all variables showed Normal distribution (p> 0.05), in the comparison between the means at T0 and T1, the t-test for paired samples was used. In order to verify whether cooperative patients had better results than the non-cooperative group, a variable was created. For all tests, a significance level of 5 % was used. A weak statistical correlation (P< 0,1) was found for Bimler’s Factor 8. Change resulted from AOB treatment with FOA in a short time. There was no statistical correlation of Bimler’s Factor 8 when cooperative patients were compared with the non-cooperative group. It could be concluded that in the studied sample there was a weak correlation between overbite improvement during AOB treatment with JFO and mandibular rotation. There are some protocols that have been shown to be effective for open bite treatment. The aim of this investigation was to study the behavior of overbite during AOB treatment with Jaw Functional Orthopedics (JFO). The behavior of overbite was studied retrospectively in 17 patients treated in the Specialization Course in (JFO – ABO Muriaé (Muriaé, MG Brazil). Monthly overbite measurements in millimeters were taken with a pachymeter and the data were stored in an individual paper worksheet. Data obtained before functional appliance installation (T0) and after 12-months of treatment (T1) were compared. Wilcoxon non-parametric test for 2 paired samples was use for statical treatment of the data. P= 0.003 for overbite comparison T1-T0. It could be concluded that Functional orthopedic appliance has shown to be an effective tool for treating anterior open bite in the sample studied. However, in the literature, there are several different landmarks used to trace the occlusal plane and there are also many researchers questioning this parallelism. To evaluate it, we used ancient skulls from Human Sambaqui skeletal remains. The authors choose an ancient population because they were submitted to same environmental conditions, avoiding bias in craniofacial growth epigenetic stimuli. Methodology: For this work we analyzed tomographies of 27 well preserved adult skulls. The program used was Vista Dent Pro 2.1. Camper plane was determined according to the original points described by Peter Camper. We used the four occlusal planes more cited in the literature. Results: None of the occlusal planes evaluated on this study, showed parallelism with Camper plane. The occlusal plane determined by posterior teeth was the one that showed less divergence but still with a high SD and mediana. Conclusion: In Sambaqui ancient population, Camper plane was not a reliable reference to trace occlusal plane.
