Material and Methodology: Observational, descriptive, retrospective, cross-sectional study, from 2016 to 2018. Results : 12 pa-tients with acute mediastinitis, predominantly male, with an average of 43 years of age. The causes were perforation by the foreign body and odontogen origin. The most frequent germ was Streptococcus sp. Tho-racotomy and / or cervicotomy were performed in 75% of the cases. The delay to consult was 4.58 days on average and the hospital stay of 19.2 days. Pleural effusion was the only complication found in 33% of patients. The mortality was 25%. Conclusion: In our series. They constitute between 1 and 2% of the tumors of the digestive tract, are characterized by their slow growth and correspond to the periampullary tumors with the best prognosis. There are several therapeutic options, one of them is endoscopic resection. We present the clinical case of a 70-year-old man who consulted due to progressive jaundice, choluria, acolia and abdominal pain. Abdominal CT is suggestive of periampullary neoplasia. Endoscopic retrograde cholangiography showed an irregular and friable proliferative lesion located in the ampulla of Vater, suggestive of ampuloma. Papillotomy, biliary stenting, and papilla biopsy are performed. Biopsy reports adenocarcinoma of the ampulla of Vater. The proposed treatment was endoscopic ampullectomy. The pathologi-cal anatomy of the resected specimen concludes villous adenoma with foci of high grade dysplasia and intramucosal carcinoma without evi-dence of invasion of the submucosa.","PeriodicalId":32918,"journal":{"name":"Cirugia Paraguaya","volume":" ","pages":""},"PeriodicalIF":0.0,"publicationDate":"2019-04-30","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"49110998","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

It frequently affects young adults, from agricultural and livestock regions. Considered endemic in South Amer-ica, with incidence points in Argentina and Uruguay. Low incidence in our country. Presenting most commonly in the liver and lung in its cystic form. Objective: The purpose of this work is to establish the management and treatment of hepatic hydatidosis in patients attending the Hospital Nacional de Itauguá in the period from 2014 to 2018. Material and methods: Observational, descriptive, retrospective cross-sectional study. The data were obtained from the Clinical Histories of patients with Hepatic Hydatidosis from the archives of the Hospital Nacional de Itauguá. Results: We studied 17 patients, of which 64.7% belonged to the female gender, with an average age of 44.8±13.8, were observed 14.06 days of hospitalization on average, 29.4% patients of the study were from the city of Paraguarí, 82.4% would have consulted for abdominal pain, 70.6% used ultrasound as a diagnostic method, 70.6% underwent a lap-arotomy and 23.5% presented a complication postsurgical. Conclusion: The majority of patients with this pathology correspond to women, from urban areas, presenting abdominal pain, diagnosed by ultrasound in whom both were practiced open surgery and a minimally invasive approach.","PeriodicalId":32918,"journal":{"name":"Cirugia Paraguaya","volume":" ","pages":""},"PeriodicalIF":0.0,"publicationDate":"2019-04-30","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"44781504","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
