D, Ujade V, Warke Sh, J. a","doi":"10.17727/jmsr.2023/11-2","DOIUrl":"https://doi.org/10.17727/jmsr.2023/11-2","url":null,"abstract":"Background: The present study was undertaken to study the histopathological features of leprosy in skin punch biopsies and to categories them in to various type based on microscopy and bacterial index. The aim of study was to know the role of histopathology in diagnosis of Hansen’s disease and to study the clinicohistopathological correlations of suspected cases of Hansen’s disease. Materials and methods: A retrospective hospital-based study of clinically diagnosed leprosy cases was conducted over a period of one year (April 2021 to March 2022). Lesional skin biopsies obtained were fixed, processed and stained with Haematoxylin and Eosin (H&E) followed by Fite-Faraco staining. The lesions were classified on microscopy as per Ridley-Jopling classification. Results: 63 Skin biopsies were obtained from patients with age range of 14-72 years. Highest incidence was in age group of 31 – 50 years. There was male predominance with male to female ratio of 2:1. Clinically borderline Hansen’s (BB) (25.40%) was the most frequent subtype observed followed by borderline lepromatous (BLH) (19.04%) and lepromatous Hansen’s (LL) (19.04%). On histopathology we found tuberculoid Hansen’s (TT) (28.57%) was the most frequently observed subtype followed by borderline tuberculoid Hansen’s (BT) (19.04%). Fite stain revealed lepromatous bacilli in all cases of LL, BL, histoid and indeterminate Hansen’s; whereas bacilli could be demonstrated in six cases of BB, eight cases of BT and two cases of TT. Conclusion: Early diagnosis of leprosy is clinically difficult as patient present in different clinicopathological form, depending on host immune status. They are misdiagnosed as ovarian/ para ovarian cysts, inguinal hernias, abscess. We report a case of 48-years-old female with gradual painless distension of abdomen. Up on explorative laparotomy cyst was seen arising from right round ligament. Excision of cystic mass was done and pathologically confirmed mesothelial cyst. Establishing accurate per operative diagnosis based on clinical findings before surgery is very important and challenging. {"title":"Effect of vaccination on clinical outcomes in COVID-19 positive chronic kidney disease patients on haemodialysis","authors":"Naseem H, Jyothi E, I. Vaccines were developed against COVID-19 and it was given emergency approval because of the associated high mortality. There are only few studies on the efficacy of vaccine in CKD patients on haemodialysis. Effect of vaccination on clinical outcomes in COVID-19 positive CKD patients on haemodialysis was studied. Materials and methods: In this retrospective study on CKD patients on haemodialysis with confirmed COVID-19 infection, comparison was done on the clinical outcomes between the vaccinated and unvaccinated population. Results: Of the 104 patients, 74 patients were vaccinated against COVID-19 and 30 were unvaccinated. The study population received either covishield (50) or covaxin (24) which were the approved vaccines in India at that time. Among the vaccinated group 15 (20%) needed invasive mechanical ventilation and 16 (53%) in the unvaccinated group (P value 0.001). There were 16 (22%) deaths in the vaccinated group and 15 (50%) in the unvaccinated group with a significantly higher mortality in the unvaccinated group (P value 0.005). 11 (21%) patients on covishiled and 4 (18%) on covaxin needed invasive mechanical ventilation and there were 12 (24%) deaths in the covishield group and 4 (18%) in covaxin group. {"title":"A study of serum cystatin C as a marker of diabetic nephropathy among patients with type 2 diabetes mellitus: A systematic review and meta-analysis","authors":"S. D, P. We utilized a meta-analysis technique to review the literature to evaluate whether serum cystatin C may be used as an early marker of diabetic nephropathy in light of the differing opinions regarding the utility of serum cystatin C as such. PubMed, Google Scholar, Web of Science, EMBASE, and Scopus Database were searched using various key search terms. Studies involving T2DM patients and containing serum cystatin C levels and the estimated glomerular filtration rate (eGFR) data were included. Pooled sensitivity, specificity, positive predictive value, and negative predictive value were also calculated. The pooled sensitivity and specificity of serum cystatin C for predicting DN were 0.87 (95% CI 0.84 - 0.91) and 0.84 (95% CI 0.83 - 0.88) respectively. The pooled positive and negative predictive values of serum cystatin C for predicting DN were 7.035 (95 % CI 4.30 – 11.43) and 0.15 (95% CI 0.09 - 0.21) respectively. The area under the summary receiver operating characteristic (SROC) curve was 0.9548 and the diagnostic odds ratio was 67.10 (95% CI 28.12 - 156.84). Serum cystatin C is a significant biomarker of DN among patients with T2DM. {"title":"Addition of HbA1c to SOFA score for predicting outcomes in diabetic patients with sepsis and septic shock","authors":"P. SOFA (sequential organ failure assessment) score has been validated as a prognostic marker in sepsis and there’s a good correlation between SOFA scoring and mortality of patients. Diabetes is a well-known predisposing factor for sepsis and we studied whether addition of HbA1c to SOFA score was helpful in predicting outcomes in diabetic patients with sepsis and septic shock. H-SOFA max score (HbA1c score added to maximum SOFA measured on any day of hospital stay) was the best predictor for in-hospital mortality. The mean H-SOFA (Max) score in the died (in-hospital mortality) group was 12.21 (±2.78), and in the discharged group was 5.49 (±2.68). Next best predictor was H-SOFA score (HbA1c score added to SOFA score at admission), followed by SOFA score. All the scores had a positive correlation with length of ICU stay. HbA1c (indicator of glycemic control) was helpful in predicting mortality when combined with the SOFA score (but not by itself). Cyclical histological changes in the endometrium reflect the fluctuating hormone levels and enables histological appearance of endometrium to be a reliable parameter to evaluate the cause of infertility. The aim was to study histomorphological patterns of endometrial tissue in cases of infertility. Materials and methods: A cross sectional study on 250 endometrial samples of infertile women both primary and secondary was carried out to find the histomorphological patterns of endometrium in infertile women. Results: 250 endometrial biopsies of patients presenting with infertility were studied. The age ranged from 20 to 45 years, with mean age of 31.47+3.15 years. Majority of the cases were of primary infertility 165 cases (66%) and secondary infertility 85 cases (34%). The various patterns include proliferative phase 76 cases (30.4%), early secretory phase 15 cases (6%), mid secretory phase 15 cases (6%) and late secretory phase 36 cases (14.4%), endometrial polyp 40 cases (16%), endometrial hyperplasia without atypia 52 cases (20.8%), atypical endometrial hyperplasia 2 cases (0.8%), nonspecific endometritis 12 cases (4.8%) and tuberculous endometritis 2 cases (0.8%). Conclusion: Endometrial biopsies provide an insight into the hormonal response of endometrium as well as the local factors of endometrium. It is a primary episodic headache disorder which is due to neurological, gastrointestinal, central nervous system and autonomic changes. The prime aim of the study was to determine the levels of vitamin D associated with the severity of headache in patients with migraine. Methods: A total number of 108 children diagnosed with migraine were evaluated. Levels of vitamin D and 25-hydroxy vitamin D3 were measured by using ELISA method (enzyme-linked immunoassay). Serum vitamin D levels were defined as 30ng/ml. The severity of the headache was determined according to the MIDAS (Migraine Disability Assessment Score)., Results: The mean serum 25-hydroxy vitamin D3 levels of migraine patients were 18.6±4.9 ng/ml. As the level of vitamin D decreased, so the severity of the headache increased, with a higher Migraine Disability Assessment score grade (p 0.05) which shows closer inverse proportional association of vitamin-D levels with migraine. Due to reasons more than one, the exact incidence of the etiology behind unilateral vocal cord palsy has remained a mystery difficult to unfold. The present study was undertaken to determine the clinical presentation and etiopathology of unilateral vocal cord paralysis. The present study was a prospective study undertaken in the Department of Otorhinolaryngology at KIMS Hospital, Secunderabad from June 2018 to May 2020 after the approval of the Ethical Committee among 50 adult patients with a confirmed diagnosis of Unilateral Vocal cord palsy. Patients with the fixity of the vocal cord, laryngeal malignancies, and bilateral Vocal cord palsy/paresis were excluded from the study. A thorough history and ENT, head & neck examination followed by a radiological examination to make an etiological diagnosis of Vocal cord palsy were done. For patients with no cause found, a CT/ MRI imaging was done before labeling them as idiopathic. Most of the patients presented in the 4th and 5th decades of life, with male predominance. The most common presenting symptom was hoarseness of voice (80%) and Left-side vocal cord palsy (56%) is more common. The most common cause was idiopathic (34%) followed by post-surgical trauma, neoplastic, central cause, cardiac, granulomatous disorders, and neurological and post-intubation trauma respectively.
