In normal fetal development, the scapula moves down the back to rest in its normal position. However, children with the deformity have a scapula that sits too high on one side. Invariably such children will have limitations in movement of the affected shoulder. Problems such as scoliosis often coexist and affected children are at greater risk of developing renal disease and conditions such as Klippel-Feil syndrome. Salaria, Sudesh Sharma","doi":"10.5580/13b","DOIUrl":"https://doi.org/10.5580/13b","url":null,"abstract":"The present study was conducted in the Department of orthopaedics, Government Medical College, Jammu over a period of one year from June 2005 to May 2006 to evaluate the efficacy of percutaneous bone marrow injection in cases of delayed union and non-union. 50 cases of post-traumatic delayed and non-union (out of 50 cases, 38 were of delayed union and 12 were of non-union), irrespective of their age and sex, were selected from Orthopaedic OPD. Forty-six (92%) out of 50 cases had successful union while four had failures. ln case of delayed union, 37 out of 38 (97.37%) patients had successful union and one case did not respond to bone marrow injection. 9 out of 12 (75%) cases of non-union resulted into union where as 3 cases failed to unite. Different methods of treating have evolved over the years from simple decompression in situ to osteotomies and transpositions. Different modalities have been used to investigate the problem. In our report we would like to show that for a complete working and treating diagnosis of cubital tunnel EMG studies with a sulcus view radiograph are sufficient, cheap and quick. More detailed investigations like MRI, CT scan are although more detailed; do not add anything more worthwhile with regards to the management. An Electromyogram (EMG) will confirm the diagnosis of cubital tunnel decompression and a sulcus view will tell us the state of the ulnar bed for the purpose of deciding whether to do a simple decompression or a transposition. INTRODUCTION AND AIM Cubital tunnel syndrome is the second most common compressive neuropathy after carpal tunnel syndrome. Different methods of treatment have evolved over the years, each with its advantages and disadvantages. A study by Heithoff 1 suggests that for most cases a simple decompression is all that is required. Cases where there were additional problems like cubitus valgus, scarred bed, osteophytes, ganglion or a tumour, may require something other than a simple decompression, and a medial condylectomy was suggested. Our hypothesis in this regard is that if we see bony encroachment of the ulnarnerve bed at the elbow, it may need an anterior transposition. However in most cases a simple decompression would suffice. We propose a simple plain radiograph to diagnose bony encroachment of the cubital tunnel. The aim of our study is to assess the value of a simple sulcus view radiograph of the elbow in deciding whether a patient with cubital tunnel syndrome needs either a simple decompression or a more extensive procedure like transposition of the nerve. MATERIAL AND METHODS A prospective study was carried between June 2003 and November 2004. All patients presenting with signs and symptoms suggestive of ulnar nerve entrapment at the elbow were studied. Detailed history and examination was carried out and patients were graded according to their symptoms using the McGowan’s classification. McGowan established the following classification system: Grade I Mild lesions with paresthesia in the ulnar nerve distribution and a feeling of clumsiness in the affected hand; no wasting or weakness of the intrinsic muscles. Grade II Intermediate lesions with weak interossei and muscle wasting. Grade III Severe lesions with paralysis of the interossei and a marked weakness of the hand. Nerve conduction studies were done to confirm the diagnosis. The exclusion criteria were: diabetic neuropathy, previous surgery or significant trauma to the elbow, and those with ulnar nerve neuropathy due to other causes such as cervical or Guyon’s canal entrapment. Virchow described a triad of changes in the blood viscosity, alterations of the vascular endothelium and changes of the laminar blood flow as causative for the manifestation of DVT. Methods: Here we present a retrospective analysis of 8 patient cases with thrombosis of the entire inferior Vena cava and pulmonary embolism orginating from DVT of the internal iliac veins that were in imminite proximity to abscess formations in the ilio-psoatic muscles. Results: The abscesses seem to originate from injections into the ileo-sacral joints and spondylodiscitis. All patients were suffering from spondylarthrosis and degenerative disease of bones and cartilage. Conclusions: As a rare complication following the injection of analgetic drugs or corticosteroids into the gluteus muscles or the ileo-sacral joint devastating infections in the retroperitoneal space may occur. The initial presentation of these abscesses can be difficult to interprete. With the decline in accidents from mining and other heavy industry and increase in Road traffic accidents, this incidences has change. At present out of significant spinal injury approx. 50% involves the cervical spine and approx. 40% of them lead to quadriplegia and approx. 10% of traumatic cord injuries have no obvious radiographic evidence of vertebral injury (SCIWORA). Aim of this retrospective case-series with 25 patients was to assess Neurological and functional outcome in Cervical Spine Injuries. RESULTS: There were 10 patients with lower cervical spine fractures (C3-C7),5 patients with upper cervical fractures,10 patients had SCIWORA injuries. Initial ASIA scale: A:4,B:3,C:5, D:8 & E:5. 15 patients were treated conservatively with cervical tongs and orthotics. Rest of 10 patients treated operatively with anterior cervical platting. Full neurological recovery in 10 patients and 40% of patients had poor neurological outcome , which is significantly higher. 5 patients (20%) were associated with glutei sores or cranial sores. 3 patients had chest infection due to respiratory pneumonitis, 3 patients had major depression (psychological complication).Operative patients were commonly associated with cardio respiratory compromise most probably due to ascending cord edema, in 24% patients. CONCLUSIONS: To conclude cervical spine injury is continuing to be fatal and paralyzing injury with poor and unpredictable neurological outcome. Final neurological outcome is depends on primary neurological damage. SCIWORA type of cervical spine injury and stable cervical spine fractures associated with good neurological outcome. Poor Neurological Outcome is associated with High velocity trauma and unstable, lower cervical spine bony injury. Knee joint is commonly involved in the OA. There is several treatment modalities, such as medical and surgical treatment. Many of patients interested to medical treatment and avoid surgical procedure. One of this treatment, treatment via indomethacin and hyaluronic preparation. This study was carried out in order to evaluate effect of hyalgan and indomethacin in the treatment of OA.Patients and methods: Patients with advanced grade of OA were included in this study. For this purpose, every patients with severe knee pain, limited range of motion, cryptation, joint stiffness, and difficulty in daily activity, were selected. Upright knee x-ray (AP, Lat) were obtained and radiologic changes were recorded. Before treatment, all cases were asked about pain, joint stiffness, knee cryptation, and other and were recorded. All patients received recommendation about activity modification and physiotherapy for quadriceps muscle. Then this patients were divided in two Hyalgan and Indomethacin groups. This study was conducted in the orthopedy clinic of Imam Khomeini Hospital, Ahwaz, Iran. Patients in Hyalgan groups , were received intra-articular injection 20mg weekly for 5 weeks. Patients in indomethacin group, were treated with dosage 25mg three times daily. All patients were evaluated post treatment.Results: in this study 39 patients (60 joints, 32 right and 28 left) underwent hyalgan and 27 patients( 50 joints, 25 right and 25 left) treated by indomethacin. From Hyalgan group 54 knee joint had grade IV and 6 have grade III radiology of OA. In the indomethacin group, 43 knee joints had grade IV and 7 joints had grade III. There is significant difference between two group for post treatment score(HA: 38.45�57.77 vs Indomethacin:32.16�67.29, P=0.00007). In the patients treated with HA, 56.67%, and 28.33% had excellent and good prognosis respectively. In indomethacin group 24.00%, and 34.00% had excellent and good prognosis respectively. Patients treated with HA had better outcome than patients treated with indomethacin( P-value=0.001475). In HA group, satisfaction about treatment was significantly higher than indomethacin group(P-value=0.003045). Pain in resting, morning stiffness, limitation in range of motion, and crypation was significantly lower in patients treated with HA compared to another (P 30yrs. About half of the people aged >65 had evidence of OA.2 In addition to age, sex can also incidence and prevalence of OA. In the aged<50, OA predominately seen in male. After that, incidence and prevalence of OA was higher in female.3 Many patients are interested to non surgical treatment. There are several non surgical treatment modalities for treatment of OA. Rose","doi":"10.5580/15dc","DOIUrl":"https://doi.org/10.5580/15dc","url":null,"abstract":"Objective: To evaluate the results of treatment of patients with acute trauma of the lower extremities using the Ilizarov technique.Method: Retrospective study of eight patients who were treated between 1999 and 2007 for acute trauma to the lower extremities. There were four Type III pilon fractures, five open wounds, three tibial shaft fractures, one comminuted supracondylar femoral fracture, and one tibial plateau fracture (Schatzker VI).Results: There were seven males and one female with an average age of 49 years (range 25 � 74 years). Results were classified in Bone and Functional results. There were six excellent, one good and one poor bone result.
