Herein, we present a 55-year-old male patient with a unruptured pseudoaneurysm after acute myocardial infarction presenting with severe mitral regurgitation. After resection of the pseudoaneurysm sac, the defect of 5 cm located at the posterolateral aspect of the left ventricle was repaired with a pericardium patch, and it was determined that mitral regurgitation diminished after cardiac geometry was restored and intervention to the mitral valve not required.","PeriodicalId":320229,"journal":{"name":"Cardiovascular Perfusion and Nursing","volume":"3 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2022-06-06","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"132679863","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

A 27-year-old female patient at 35 weeks of pregnancy applied to the cardiology department with a complaint of palpitation. In the control transthoracic echocardiography, a 24¥25 mm mass was detected in the right atrium. The patient had situs inversus totalis and had undergone a robotic atrial septal defect closure operation one year ago. After the cesarean operation, robotic cardiac surgery was performed on the beating heart, and the mass in the right atrium was successfully removed. During the procedure, the perfusion strategy was differently conducted than the normal patient management, and the perfusion device was located differently. Cardiac surgery with minimally invasive robotic surgery in a pregnant female with dextrocardia is a special condition that requires many changes in perfusion methods and protocols, and their coexistence create a difficult process for perfusionists.","PeriodicalId":320229,"journal":{"name":"Cardiovascular Perfusion and Nursing","volume":"1 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2022-06-06","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"126989042","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Patients and methods: Between June 2016 and June 2017, a total of five patients (1 male, 4 females; mean age 67.2 years; range, 55 to 81 years) who received VA-ECMO for PCCS in our clinic were retrospectively analyzed. The ECMO device was implanted in the operating room or in the cardiovascular intensive care unit CICU by an experienced cardiovascular surgeon. Air and thrombus formation and blood flow were monitored by the perfusionists. Daily nursing care included monitorization, control of bleeding, sedation management, pain control, nutrition, control of skin integrity and infection, sponge bath, changing the position of endotracheal tube, dressing replacement, decubitus control, changing sheets, and back hygiene. Results: Two patients who were successfully weaned off VA-ECMO survived to hospital discharge. The median duration of daily nursing care was 62 (range, 45 to 65) min. The longest steps in daily nursing were sponge bath and elevation of the patient. None of the patients suffered from decannulation or displacement of the VA-ECMO cannulas during nursing. Conclusion: We conclude that nurses are fundamental in the care of VA-ECMO patients for the early diagnosis and intervention of procedure-related complications.","PeriodicalId":320229,"journal":{"name":"Cardiovascular Perfusion and Nursing","volume":"40 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2019-04-25","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"128166520","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Working integrated with the electronic patient records, as a part of the clinical process, CDSS serves messages, makes suggestions, and helps applying new information to the patient care by analyzing patient-specific clinical variables. The CDSSs propose evidence-based nursing diagnosis to the nurses, ensure that the patient benefits from an individually designed extensive care service, and encompass drug warnings, drug interactions, food interactions, medical malpractice prevention systems, audio-visual warning systems, and statistical and reporting systems. In this article, we describe CDSSs and present type of CDSSs used in the cardiovascular surgery nursing field with relevant application examples.","PeriodicalId":320229,"journal":{"name":"Cardiovascular Perfusion and Nursing","volume":"30 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2019-04-25","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"115904730","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Upper brachial arterial cannulation was performed to establish cardiopulmonary bypass. Aortic and mitral valves replaced with prosthetic mechanical valves and a 30-mm synthetic graft was interposed to the ascending aorta. Finally, five distal anastomoses were performed for multivessel coronary artery disease over a total 118-min cross-clamp time. After an uneventful recovery, the patient was discharged in the postoperative sixth day without any complication. In conclusion, we believe that high brachial arterial cannulation to establish antegrade selective cerebral perfusion seems to be a safe alternative method.","PeriodicalId":320229,"journal":{"name":"Cardiovascular Perfusion and Nursing","volume":"147 ","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2019-04-25","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"120865150","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

In these cases, if the absence of aortic insufficiency is documented, beating heart surgery is possible via right thoracotomy in the Trendelenburg position. However, venous drainage, cardiopulmonary bypass temperature, electrolyte balance, and cerebral monitorization are the key considerations for the perfusionist. Herein, we discuss the perfusion standards of mitral valve redo surgery in a patient with low ejection fraction and a stuck mitral valve.","PeriodicalId":320229,"journal":{"name":"Cardiovascular Perfusion and Nursing","volume":"10 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2019-04-25","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"128415090","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
